[Intraoperative facial nerve monitoring in parotid surgery].

Neurophysiological monitoring of cranial motor nerves has proved to be of value in cerebellopontine and skull base surgery. Unfortunately, facial nerve monitoring has been used infrequently for routine parotid gland surgery because suspicion of expense, possible unreliability and the requirement for extra personnel. This study presents clinical experience at the University of Erlangen with facial nerve monitoring during parotid gland surgery done by residents. Advantages are also emphasized for the experienced ENT-surgeon for use during revision parotidectomy. In 35 consecutive patients with b

Intraoperative evoked potential monitoring in acetabular surgery.

The effectiveness of intraoperative sciatic nerve monitoring was evaluated for 88 consecutive patients undergoing open reduction and internal fixation for acetabular fractures. Intervention outcomes and pre and postoperative electrophysiologic status were compared to postoperative functional findings. Only 2% of the patients demonstrated iatrogenic sciatic nerve palsies. Functional and evoked potential findings were in agreement for 89% of the patients with postoperative palsies, while 26% of the functionally normal patients showed abnormal evoked potentials. Intervention occurred in 55 surger

Intraoperative monitoring for tethered spinal cord syndrome.

Functions of the lower extremities and bowel and bladder must be monitored during releasing surgery for tethered spinal cord syndrome, because neural elements are embedded in lipoma or anomalous tissues. Evoked muscle action potentials of the external anal sphincter muscle, external urethral sphincter muscle, and lower extremity muscles can indicate promptly whether or not neural elements are involved before the surgeon releases or cuts the tissue for cord release. It is also important to monitor vesical pressure because of different types of innervation from the external urethral and anal sph

Hypnosis for monitoring intraoperative spinal cord function.

In such procedures as the Harrington operation for idiopathic scoliosis, evaluation of spinal cord function is advisable immediately after the back is straightened. The authors have used hypnosis for this procedure, as an adjunct to anesthetic management. Patients are awakened on the table, given lower extremity tests, and are then reanesthetized. The incidence of postoperative pain and other discomfort is also reduced by hypnosis.

Parathyroidectomies using intraoperative parathormone monitoring: when should we stop measuring intraoperative parathormone levels?

Intraoperative parathyroid hormone monitoring (IOPM), in use for the last 15 years, has facilitated focused parathyroidectomy. We undertook this study to determine if a drop in IOPT hormone levels below 50 per cent of baseline were sufficient to terminate the procedure. We conducted a retrospective chart review (January 2007 to September 2010) of 104 patients who underwent initial parathyroidectomies with IOPM by general surgeons for primary hyperparathyroidism. Patients were followed up for serum calcium levels (range, 6 to 48 months). The number of specimens excised was significantly decreas

Intraoperative Neurophysiological Monitoring During Spinal Cord Tumor Surgery.

Intramedullary spinal cord tumor surgery has evolved through major technological advances. From early attempts in the 19th century to modern methods, innovations like the operative microscope, bipolar coagulation, and MRI have transformed practice. The introduction in the 1980s of intraoperative neurophysiological monitoring enabled real-time spinal cord functions assessment through the application of monitoring techniques. In this article, we will review the state of the art of the different techniques, the alarm criteria and related recovery strategies, current controversies and future consi

Intraoperative monitoring of trigeminal neuralgia: a technical note.

Trigeminal neuralgia causes excruciating pain in patients. Microvascular decompression is indicated for drug-resistant s trigeminal neuralgia. Unlike facial spasms, any part of the nerve can be the culprit, not only the root entry zone. Intraoperative monitoring does not yet exist for trigeminal neuralgia. We successfully used intermittent stimulation of the superior cerebellar artery during surgery and confirmed the disappearance of the trigeminal nerve motor branch reaction after the release of the compression. Intermittent direct stimulation of the culprit blood vessel using the motor branc

Intraoperative neurophysiological monitoring and spinal cord stimulator implantation.

INTRODUCTION: Spinal cord injury (SCI) is one of the most dreaded complications after spinal cord stimulation (SCS) implantation surgery. As a result, intraoperative neurophysiological monitoring (IONM) has been proposed to avoid accidental damage to nervous structures under anesthesia and confirm positioning for optimal stimulation. Our study uses a large administrative claims database to determine the 30-day risk of SCI after SCS implantation. METHODS: This retrospective cohort study used the IBM MarketScan Commercial and Medicare Supplemental Databases from 2016 to 2019. Adult patients und

Intraoperative Monitoring of Scoliosis Surgery in Young Patients.

Intraoperative neurophysiologic monitoring has added substantially to the safety of spinal deformity surgery correction since its introduction over four decades ago. Monitoring routinely includes both somatosensory evoked potentials and motor evoked potentials. Either modality alone will detect almost all instances of spinal cord injury during deformity correction. The combined use of the two modalities provides complementary information, can permit more rapidly identification of problems, and enhances safety though parallel redundancy should one modality fail. Both techniques are well establi

[Intraoperative Monitoring during Microvascular Decompression for Hemifacial Spasm].

Brainstem auditory evoked potential(BAEP)and abnormal muscle response(AMR)monitoring are widely used during microvascular decompression(MVD)for hemifacial spasm(HFS). In BAEP monitoring, the intraoperative findings of wave V do not necessarily predict postoperative hearing function. However, if a warning sign as significant as wave V change appears, the surgeon must abort the operation or inject artificial cerebrospinal fluid into the VIII nerve. During MVD for HFS, BAEP monitoring must be performed to preserve hearing function. AMR monitoring is useful in identifying the offending vessels com

Intraoperative Cochlear Nerve Monitoring in Vestibular Schwannoma Microsurgery.

Monitoring the cochlear nerve during vestibular schwannoma (VS) microsurgery depends on the hearing status and surgical approach. Traditional hearing preservation VS microsurgery relies on acoustically driven auditory brainstem response (ABR) and cochlear nerve action potential. Both modalities have advantages and disadvantages that need to be understood for proper implementation. When hearing is lost or the approach violates the otic capsule, electrically evoked monitoring methods may be used. Evoked ABR (eABR) is feasible and safe but may be limited by artifact. Combining eABR with near-fiel

Intraoperative Neurophysiological Monitoring for Intradural Extramedullary Spinal Tumours.

STUDY DESIGN: Retrospective cohort study. OBJECTIVES: Intraoperative neurophysiological monitoring (IONM) is widely used in spinal neurosurgery, particularly for intramedullary tumours. However, its validity in intradural extramedullary (IDEM) spinal tumours is less clearly defined, this being the focus of this study. METHODS: We compared outcomes for patients that underwent resection of IDEM tumours with and without IONM between 2010 and 2020. Primary outcomes were postoperative American Spinal Injury Association (ASIA) scores. Other factors assessed were use of intraoperative ultrasound, d

Intraoperative mapping and monitoring during brain tumor surgeries.

Many different methodologies and paradigms are available to guide surgery of supratentorial tumors with the aim to preserve quality of life of the patients and to increase the extent of tumor resection. Neurophysiologic monitoring techniques (such as different evoked potentials) may help to continuously assess functional integrity of the observed systems and warn about vascular injury. For neurophysiologic mapping methods, the focus is not only to preserve cortical sites, but also to prevent injury to subcortical pathways. Therefore, cortical mapping is not enough but should be combined with s

SEP Montage Variability Comparison during Intraoperative Neurophysiologic Monitoring.

Intraoperative monitoring is performed to provide real-time assessment of the neural structures that can be at risk during spinal surgery. Somatosensory evoked potentials (SEPs) are the most commonly used modality for intraoperative monitoring. SEP stability can be affected by many factors during the surgery. This study is a prospective review of SEP recordings obtained during intraoperative monitoring of instrumented spinal surgeries that were performed for chronic underlying neurologic and neuromuscular conditions, such as scoliosis, myelopathy, and spinal stenosis. We analyzed multiple mont

Intraoperative mapping and monitoring in supratentorial tumor surgery.

With the increasing strive for complete resections of supratentorial low or high-grade gliomas, the necessity for intraoperative electrical stimulation methods to map and monitor functional important cortical or subcortical brain areas increased. While many brain functions can be assessed during awake surgery using electrical stimulation, motor function as well as visual function can be mapped and monitored in the asleep patient, as well. Several different methods reviewed in this article are available which lead to an improved functional outcome while increasing the extent of tumor resection

Intraoperative neurophysiologic monitoring: are we really that bad?

STUDY DESIGN: Retrospective review of a prospectively collected, multicenter database. OBJECTIVE: To assess the rates of new neurologic deficit (NND) associated with spine surgery. SUMMARY OF BACKGROUND DATA: New neurologic deficit is a potential complication of spine surgery but previously reported rates are often limited by small sample size and single-surgeon experiences. METHODS: The Scoliosis Research Society morbidity and mortality database was queried for spinal surgery cases complicated by NND from 2004 to 2007, including nerve root deficit, cauda equina deficit, and spinal cord def

Neurophysiologic intraoperative monitoring in children with Down syndrome.

OBJECTIVE: Neurophysiological monitoring during complex spine procedures may reduce risk of injury by providing feedback to the operating surgeon. This tool is a well-established and important surgical adjunct in adults, but clinical data in children are not well described. Moreover, to the best of our knowledge, neurophysiologic intraoperative monitoring data have not been reported in children with neurodevelopmental disorders, such as Down syndrome, who commonly present with craniocervical instability requiring internal fixation. The purpose of this study is to determine the reliability and

An optimal algorithm for intraoperative parathyroid hormone monitoring.

BACKGROUND: A minimally invasive approach to primary hyperparathyroidism is equivalent to bilateral exploration when intraoperative parathyroid hormone (IOPTH) monitoring is used. The optimal strategy for the monitoring has been debated. HYPOTHESIS: There exists an optimal strategy for IOPTH monitoring. DESIGN: Retrospective study. SETTING: Tertiary referral hospital. PATIENTS AND METHODS: A total of 1882 patients underwent parathyroidectomy for primary hyperparathyroidism with IOPTH monitoring. Successful exploration was defined as a 50% or more decline in IOPTH level from baseline and a

Intraoperative fluid management guided by oesophageal Doppler monitoring.

PROBLEM: Fluid management during major surgery poses a challenge to the surgical team as postoperative complications are often related to giving the wrong amount of intravenous fluid. Postoperative morbidity can be reduced by using the oesophageal Doppler cardiac output monitor to individualise fluid administration, but this technology has not been widely adopted. DESIGN: A campaign for adopting this technology in major surgical specialties explored clinical and managerial barriers throughout the procurement and implementation process. We compared patient outcomes 12 months before implementat
