Measuring Blunt Force Head Impacts in Athletes.

INTRODUCTION: Although concussion continues to be a major source of acute and chronic injuries, concussion injury mechanisms and risk functions are ill-defined. This lack of definition has hindered efforts to develop standardized concussion monitoring, safety testing, and protective countermeasures. To overcome this knowledge gap, we have developed, tested, and deployed a head impact monitoring mouthguard (IMM) system. MATERIALS AND METHODS: The IMM system was first calibrated in 731 laboratory tests. Versus reference, Laboratory IMM data fit a linear model, with results close to the ideal li

A thirty-year review of oral appliances used to manage microstomia, 1972 to 2002.

Patients who have sustained a facial burn often contend with cosmetic and functional challenges, including microstomia. Many devices have been created by a variety of professionals to provide oral stretching. These devices can be classified as intraoral or extraoral and deliver a stretch either horizontally, vertically, or circumorally. The purpose of this article is to present a comprehensive review of oral-stretching devices.

Oral injury and mouthguard usage by athletes in Japan.

The use of mouthguards in contact sports effectively prevents oral injury and preserves oral structures. We investigated oral injuries and awareness concerning mouthguards among Japanese high school soccer and rugby players. Athletes were asked a series of questions concerning their history of oral injury while participating in sports, and their pattern of mouthguard use was determined. The data were evaluated statistically using chi-square analysis. The incidence of oral injuries was 32.3% for soccer athletes and 56.5% for rugby athletes, with 0.8% and 24.1% of the respective groups having mo

On-field instrumented mouthguard coupling.

Instrumented mouthguard (iMG) sensors have been developed to measure sports head acceleration events (HAE) in brain injury research. Laboratory validation studies show that effective coupling of iMGs with the human skull is crucial for accurate head kinematics measurements. However, iMG-skull coupling has not been investigated in on-field sports settings. The objective of this study was to assess on-field iMG coupling using infrared proximity sensing and to investigate coupling effects on kinematics signal characteristics. Forty-two university-level men's ice hockey (n = 21) and women's rugby

Laboratory Validation of Instrumented Mouthguard for Use in Sport.

Concussion is an inherent risk of participating in contact, combat, or collision sports, within which head impacts are numerous. Kinematic parameters such as peak linear and rotational acceleration represent primary measures of concussive head impacts. The ability to accurately measure and categorise such impact parameters in real time is important in health and sports performance contexts. The purpose of this study was to assess the accuracy of the latest HitIQ Nexus A9 instrumented mouthguard (HitIQ Pty. Ltd. Melbourne Australia) against reference sensors in an aluminium headform. The headfo

Influence of intra-oral maxillary sports mouthguards on the airflow dynamics of oral breathing.

PURPOSE: Mouthguards worn during sporting competition may influence oral airway flow dynamics and potentially increase airflow resistance during mouth breathing. METHODS: We measured oral airflow resistance (RO) in 10 normal subjects (four men, six women, age 29 +/- 3 yr, mean +/- SEM) wearing two different custom-made maxillary mouthguards. RESULTS: During tidal mouthpiece breathing (jaw position controlled), inspiratory R(O) (at (1.4 L x s(-1)) increased from 0.22 (0.15-0.46) cm H2O x L(-1) x s(-1) (median and interquartile range) to 0.47 (0.24-0.52) cm H2O x L(-1) x s(-1) with mouthguard

Clinical protocol for fabricating a custom mouthguard for a completely edentulous boxer.

An edentulous patient presented for fabrication of new complete dentures. After completion of treatment, he disclosed his participation in professional boxing, which required the mandatory use of a mouthguard. As a documented technique for the fabrication of sports mouthguards for completely edentulous athletes was not found, a custom protocol was developed to meet his functional, protective, and regulatory needs. This clinical report describes the step-by-step technique and clinical rationale for creating a custom mouthguard by duplicating maxillary and mandibular complete dentures to provide

Dental injuries, temporomandibular disorders, and caries in wrestlers.

The aim of this study was to examine the prevalence of dental injuries, temporomandibular disorders (TMD), and dental caries in a group of champion wrestlers. Twenty-six male wrestlers, with a mean age of 23 yr, and an age-matched control group participated in the study. A questionnaire was used with questions on trauma, frequency of headache, intensity of practicing sports, use of sugar-containing "sports drinks", use of mouth guards, and previous TMD problems. Four bitewing radiographs were taken in all subjects. In addition, three intraoral apical radiographs of maxillary and mandibular fro

Real-Time Continuous Tongue Pressure Measurement With Mouthguard-Type Pressure-Sensing Device.

OBJECTIVE: It is well-established that occlusion and dental arch form are related to the morphology and function of the oral soft tissues. Oral soft tissue dynamic assessment is important for elucidating the causes of malocclusion and developing effective treatment methods. We previously developed a small mouthguard-type sensing device for measuring oral soft tissue pressure; however, its continuous measurement performance had not been thoroughly evaluated. This study assessed the device's ability to continuously measure tongue pressure applied to the palatal surfaces of the maxillary anterior

Biofidelity and Limitations of Instrumented Mouthguard Systems for Assessment of Rigid Body Head Kinematics.

Instrumented mouthguard systems (iMGs) are commonly used to study rigid body head kinematics across a variety of athletic environments. Previous work has found good fidelity for iMGs rigidly fixed to anthropomorphic test device (ATD) headforms when compared to reference systems, but few validation studies have focused on iMG performance in human cadaver heads. Here, we examine the performance of two boil-and-bite style iMGs in helmeted cadaver heads. Three unembalmed human cadaver heads were fitted with two instrumented boil-and-bite mouthguards [Prevent Biometrics and Diversified Technical Sy

Validation and Comparison of Instrumented Mouthguards for Measuring Head Kinematics and Assessing Brain Deformation in Football Impacts.

Because of the rigid coupling between the upper dentition and the skull, instrumented mouthguards have been shown to be a viable way of measuring head impact kinematics for assisting in understanding the underlying biomechanics of concussions. This has led various companies and institutions to further develop instrumented mouthguards. However, their use as a research tool for understanding concussive impacts makes quantification of their accuracy critical, especially given the conflicting results from various recent studies. Here we present a study that uses a pneumatic impactor to deliver imp

[Sports and orofacial injuries].

Many people enjoy sports, it is healthy and relaxing. There is, however, an inherent risk of sustaining injuries and fractures of the maxillofacial skeleton. Adequate diagnostics and treatment are mandatory to minimize the possible long-term consequences of injuries. Dentists may become primary involved in the diagnosis of such traumas, particularly when injuries are limited to the orofacial region. In addition, they can play an important role in caring for sportsmen with orofacial injuries, for instance by endodontic, restorative, and prosthodontic treatments. This paper discusses the assessm

Accuracy of Instrumented Mouthguards During Direct Jaw Impacts Seen in Boxing.

PURPOSE: Measuring head kinematics data is important to understand and develop methods and standards to mitigate head injuries in contact sports. Instrumented mouthguards (iMGs) have been developed to address coupling issues with previous sensors. Although validated with anthropomorphic test devices (ATDs), there is limited post-mortem human subjects (PMHS) data which provides more accurate soft tissue responses. This study evaluated two iMGs (Prevent Biometrics (PRE) and Diversified Technical Systems (DTS) in response to direct jaw impacts. METHODS: Three unembalmed male cadaver heads were p
