Patient acuity as a determinant of paramedics' frequency of being exposed to physically demanding work activities.

BACKGROUND: The purpose of this investigation was to examine if paramedics' frequency of being exposed to highly physically demanding activities, or their perception of physical, clinical, and emotional demands were altered by patients' acuity level, operationalized using the Canadian Triage and Acuity Scale (CTAS). METHODS: Physical demands descriptions (PDD) were compiled from thirteen services across Canada. The observation sessions took place during a minimum of two full-shift (12-h) ride-outs at each service. Data were obtained from 53 ride-outs, which included a total of 190 calls. RES

Manual handling and back pain among health care professionals in neurological inpatient and outpatient settings: a mixed methods study.

BACKGROUND: The number of patients with neurological disorders and severe disability is increasing globally. These patients often need help with positioning and the amount of support varies with their level of impairment. High rates of work-related musculoskeletal disorders are observed among healthcare professionals (HCP) with patient contact due to injuries during manual handling. There is insufficient research on manual handling by nurses and other HCP. OBJECTIVES: The primary aim of this study was to explore manual handling strategies by HCP in neurological inpatient and outpatient settin

Effects of transfer board use on physical load and transfer time in wheelchair-to-bed and bed-to-wheelchair transfers in simulated lower limb disability.

This study evaluated the impact of a transfer board on reducing physical demands during wheelchair-to-bed and bed-to-wheelchair transfers. A quasi-experimental pre- and post-intervention design was conducted with 23 university students simulating individuals with disabilities after training in transfer board use. Dependent variables included cardiovascular load (heart rate), transfer time, and perceived effort (Borg scale). Using the transfer board significantly reduced cardiovascular load from 19.2% to 11.2% (p = 0.001), decreased transfer time by 23.1% (p = 0.03), and lowered perceived effor

TransKinect: a computer vision and machine learning clinical decision support system for automatic independent wheelchair transfer technique assessment.

BACKGROUND: Physical and occupational therapists provide routine care for manual wheelchair users and are responsible for training and assessing the quality of transfers. These transfers can produce large loads on the upper extremity joints if improper sitting-pivot-technique is used. Methods to assess quality of transfers include the Transfer Assessment Instrument, a clinically validated tool derived from quantitative biomechanical features; however, adoption of this tool is low due to the complex usage requirements and speed of typical transfers. OBJECTIVE: The objective of this study is to

To log-roll or not to log-roll - That is the question! A review of the use of the log-roll for patients with pelvic fractures.

Pelvic fractures are one of the most serious injuries in polytrauma - massive haemorrhage is a major complication and the leading cause of death. The assessment and early management of these injuries in the Emergency Department (ED) focuses on the recognition of haemodynamic instability and should follow Advanced Trauma Life Support (ATLS) guidelines. The log-roll technique, although an accepted practice in trauma care, has been reported to be inadvisable when a pelvic fracture is suspected. This paper, which includes a case study, briefly explains the pathophysiology and initial management of

Feasibility of optimizing muscle activation of individuals with spinal cord injuries to reduce caregiver effort in assisted transfers.

BACKGROUND: Spinal cord injury (SCI) significantly affects the quality of life of affected individuals and their caregivers. Assisted transfers after SCI are physically demanding and one of the leading causes of musculoskeletal injuries among caregivers. While mechanical lift systems can reduce injury risk, their adoption is limited by high costs, large size, and portability. Functional neuromuscular stimulation (FNS) presents a promising alternative for facilitating transfers by activating the paralyzed muscles below the level of injury. However, determining the correct and optimal stimulatio

Chair Rise Peak Power in Daily Life Measured With a Pendant Sensor Associates With Mobility, Limitation in Activities, and Frailty in Old People.

The aim of this study was to analyze the clinical relevance of sensor-based daily life chair rise performance measured in old people. A pendant-sensor was worn during standardized tests and in daily life to detect chair rise transfers and analyze transfer peak power. Linear correlations between mean, median, 25th, and 75th percentile transfer peak powers in daily life and mean peak power in standardized tests were evaluated with Pearson correlation ( r). Associations between transfer peak powers in different experiments and outcomes of a clinical mobility test [timed-up-and-go (TUG)], a test o

Management of a 3-year-old with an unstable C6-C7 diastasis without quadriplegia.

BACKGROUND: Caring for pediatric spine trauma patients places spine surgeons in situations that require unique solutions for complex problems. Recent case reports have highlighted a specific injury pattern to the lower cervical spine in very young children that is frequently associated with complete spinal cord injury. METHODS: This report describes the presentation and treatment of a C6-C7 dislocation in a 3-year-old patient with an incomplete spinal cord injury. The highly unstable cervical injury and the need to prevent neurologic decline added complexity to the case. RESULTS: A multi-sur

Hanger-lifting procedure in knee arthroscopy.

We propose a unique arthroscopic technique, the "hanger-lifting procedure." Unlike conventional arthroscopy, the space in which the arthroscope is placed is not a joint space filled with water but a subcutaneous space filled with air. The space is kept lifted by a semi-loop-shaped hanger and a retraction system by use of a wire. In general, arthroscopes are unable to be applied outside the joint because of the lack of a cavity. However, this method can provide extra-articular visualization of the knee in addition to standard intra-articular visualization. This approach is useful for lateral re

Exploring repositioning movements in sitting with 'at risk' groups using accelerometry and interface pressure mapping technologies.

BACKGROUND: Despite high quality guidelines underpinning pressure ulcer care (NPUAP/EPUAP/PPPIA, 2014), pressure ulceration still poses a significant financial impact on health care services in treatment and staff costs as well as having a profound effect on the health and quality of life of individuals experiencing them. Repositioning is a key preventative technique recommended by occupational therapists and other health care professionals. The frequency and quality of repositioning movements performed by individuals, however, can be difficult to determine. This paper explores the use of tech

Measuring dynamic stability requirements during sitting pivot transfers using stabilizing and destabilizing forces in individuals with complete motor paraplegia.

Dynamic stability requirements have never been quantified when long-term manual wheelchair users transfer themselves in a seated position from an initial surface to a target surface, a functional task commonly referred to as sitting pivot transfers (SPTs). Ten individuals with spinal cord injury (SCI), who rely on a manual wheelchair for mobility, underwent a comprehensive biomechanical SPT assessment. SPTs performed toward a target seat of same height (even) and a seat 10cm higher than the initial seat (uneven), repeated three times for each task, were assessed. A dynamic equilibrium model, c

A Novel and Simple Modification for Management of Distal Ureter During Laparoscopic Nephroureterectomy Without Patient Repositioning: A Bulldog Clamp Technique and Description of Modified Port Placement.

BACKGROUND AND PURPOSE: To describe a novel and pure laparoscopic approach using a custom-made bulldog clamp with modified port placement for management of distal ureter during laparoscopic nephroureterectomy. PATIENTS AND METHODS: Between October 2013 and December 2014, 31 patients found to have upper tract urothelial carcinoma were treated using this technique. After finishing a standard laparoscopic transperitoneal nephrectomy in a 45° to 60° recumbent position, an additional 12-mm trocar was inserted at the lower abdomen to allow the surgeon to continue dissecting the ureter caudally towa

Evaluation of a passive back-support exoskeleton during in-bed patient handling tasks.

This study evaluated the effects of a back-support exoskeleton on the trunk and hip joint angles, lower back muscle activity and heart rate during four patient handling tasks: assisting a patient from sitting to lying, laterally repositioning the patient and turning the patient in two directions. Eight participants performed these tasks with and without the exoskeleton. Results demonstrated a significant reduction in the lower back muscle activity, but less pronounced effects for other tasks involving minimal trunk flexion. Hip flexion angles were reduced for all tasks when the exoskeleton was

How to perform extracorporeal photopheresis via port catheter.

BACKGROUND: Extracorporeal photopheresis (ECP) is commonly performed via peripheral venous access catheter. If this is not possible, a tunneled central venous catheter may be considered. However, this access mode may be associated with high complication rates. Port catheters and permanent arteriovenous fistulas in principle could serve as alternative options; however, treatment opinions are limited. STUDY DESIGN AND METHODS: We report on four adult patients with poor peripheral vein conditions in whom we performed ECP via port catheter. RESULTS: In three of four patients, problems arising fr

An ergonomic assessment of using laterally-tilting operating room tables and friction reducing devices for patient lateral transfers.

Patient lateral transfers between two adjacent surfaces pose high musculoskeletal disorder risks for nurses and patient handlers. The purpose of this research was to examine the ergonomic benefits of utilizing the laterally-tilting function of operating room (OR) tables during such transfers - along with different friction-reducing devices (FRD). This method allows the patient to slide down to the adjacent surface as one nurse guides the transfer and another controls the OR table angle with a remote control. Sixteen nursing students and sixteen college students were recruited to act as nurses

Manual handling of patients: role of kinesiophobia and catastrophizing in health workers with chronic low back pain.

BACKGROUND: Biomechanical overload due to patients' manual handling represents a relevant contributor to chronic low back pain (LBP). Fear of movement (also known as kinesiophobia) and catastrophising may influence the development of chronic complaints and lower performances also in working environments, despite these issues are poorly investigated. AIM: The aim of this study is twofold: 1) to evaluate the levels of kinesiophobia and catastrophizing in a sample of health personnel with chronic LBP and employed in activities specifically requiring patients' manual handling; 2) to appraise the
