Management of cancer therapy-induced oral mucositis.

The oral mucosa is a common site for collateral damage of cancer therapies, including radiation, cytotoxic medication, and newer targeted therapies. Ulcerative oral mucositis is typically painful and affects oral functions including speech, and oral intake of food and medications, thus impacting the quality of life. Denuded epithelium may also provide access of oral microbial flora to the deeper tissues and the circulation. Prevention of oral mucositis is an ongoing quest currently with relatively few answers. In this chapter, the present authors discuss the evidence and current practice for p

Radiation therapy and chemotherapy-induced oral mucositis.

UNLABELLED: Increasing the intensity of radiation therapy and chemotherapy in the management of cancer has increased the incidence of adverse effects, especially oral mucositis. AIM AND METHODS: a bibliographical review was conducted on the definition of oral mucositis, its clinical findings, the incidence, its etiology, the pathophysiology, associated morbidity, prevention and treatment. RESULTS: current studies define oral mucositis as a very frequent and painful inflammation with ulcers on the oral mucosa that are covered by a pseudo membrane. The incidence and severity of lesions are inf

Research controversies in management of oral mucositis.

The management of mucositis is the subject of many controversies, and the optimal treatment is still not known. Several evaluation scoring systems have been described, but no one of these is appropriate to all clinical situations: a simple scale such as that devised by the WHO can be used routinely, and more sophisticated ones can be implemented by trained experimenters working in research. We have considered the impact of each of the treatments currently available on each stage of mucositis. In attempts at prevention, self-care, in the sense of oral hygiene, must remain atraumatic. It is prob

[The therapeutic management of radiogenic oral mucositis].

BACKGROUND: Acute reactions of oral mucosa are a frequent side effect of radiotherapy, which often necessitates interruption of the treatment. Marked proliferation of tumor stem cells during treatment interruptions may occur in squamous cell carcinomata, which represent the majority of tumors in the head and neck area. Hence a fatal consequence of treatment breaks may be a significant decrease in tumor cure rates. Furthermore, marked acute responses frequently result in increased late sequelae ("consequential damage"). Therefore, amelioration of the mucosal response aiming at avoiding treatmen

[Prevention of oral mucositis during antiblastic chemotherapy].

Oral complications of cancer chemotherapy have been studied in a group of 50 adult patients. A number of 25 patients was treated by means of oral hygiene and prophylactic measures: drugs and topical agents, while the other group of 25 patients had not received stomatological prevention. The goal of the research has showed that prevention measures and elimination of existing or potential sources of infection reduce and minimize mucositis, infection etc.

Chemotherapy-induced oral mucositis in adult leukemia.

Oral mucositis directly attributable to antileukemia chemotherapy occurs in almost 20% of adults undergoing such treatment. Although the mucositis is self-limited when uncomplicated by infection, the attendant extreme discomfort may produce physical and psychologic obstructions to continued anticancer treatment. Because of the ever-present risk of serious infection, drug-induced stomatitis constitutes a threat to the patient that must be kept under constant surveillance and control.

Mucosal microenvironment and mucosal response.

Considerable investigative effort is currently being directed towards the use of oral immunization for the prevention of mucosal infections, including otitis media, in infancy and childhood. The development of immune response to mucosally introduced vaccines or environmental antigens is significantly influenced by the mucosal microflora, enzymatic activity, factors influencing epithelial permeability and the nature of vaccine antigens administered. Studies carried out during the past several years have suggested that antigen uptake, antigen processing, and immune response to environmental anti

Measuring Oral Mucositis of Pediatric Patients with Cancer: A Psychometric Evaluation of Chinese Version of the Oral Mucositis Daily Questionnaire.

OBJECTIVE: Oral mucositis is a frequent clinical condition that has been shown to affect pediatric cancer patients. Oral Mucositis Daily Questionnaire (OMDQ) is one of the few available patient-reported outcome measures to assess the extent and impact of oral mucositis. The objectives of the study were to translate the Mouth and Throat Soreness-Related Questions of the OMDQ into Chinese (OMDQ MTS-Ch) for children and adolescents aged 6-18 years receiving chemotherapy and to evaluate its psychometric properties. METHODS: This was part of a multicenter, prospective cohort study involving two ph

Oral Mucositis in Head and Neck Cancer Patients.

Oral mucositis (OM) is a common side effect of radiation therapy for head and neck cancer (HNC). Despite the medical advances in cancer therapy, OM is still virtually inevitable in patients being irradiated for neoplasms of the head and neck. The initial signs of oral mucositis typically manifest after cumulative doses between 15 and 20 Gy, with ulceration formation by 30 Gy and reaching peak severity in the week after radiation treatment completion (generally 60-72 Gy in management of HNC), then resolving over the 3-4 weeks following treatment completion. Severe oral mucositis (SOM), defined

Ocular manifestations of mycoplasma-induced rash and mucositis.

A 13-year-old girl presented with a 5-day history of fever and cough followed by new-onset oral ulcers and conjunctival injection. Clinical examination revealed bilateral 360-degree subconjunctival hemorrhages, which later evolved to corneal epithelial defects, pseudo-membrane formation, and extensive oral mucosal ulceration. Mycoplasma pneumoniae serum IgG and IgM were positive. Treatment with topical prednisolone acetate, moxifloxacin, preservative-free artificial tears, and erythromycin ointment was initiated. A self-retaining amniotic membrane was placed. The ocular and oral lesions resolv

Severe Mycoplasma pneumoniae-associated mucositis treated with immunoglobulins.

UNLABELLED: Mycoplasma pneumoniae-associated mucositis (MPAM), previously labelled as atypical Stevens-Johnson syndrome (SJS), SJS with minimal or no skin manifestations, is a rare non-respiratory manifestation of Mycoplasma pneumoniae infection. The nineteen cases described so far in children and young adults were characterized by a high male gender prevalence (16/19) and a good response to appropriate antibiotic treatment and supportive care in the majority of patients. We describe a case of MPAM in a previously healthy girl, who improved after a 0.5 g/kg daily dose of intravenous immunoglob

Pathobiology of oral mucositis: novel insights and opportunities.

Oral mucositis is a common and debilitatingly painful side effect of many forms of chemotherapy and radiation therapy. The erythematous, atrophic, and ulcerative lesions that develop are a consequence of epithelial damage and death mediated through a complex series of molecular and cellular events. The consequences of mucositis are far-reaching and include chemotherapy dose reductions, breaks in radiation treatment, cessation of cancer therapy, reliance on parenteral nutrition, administration of narcotics, hospitalization, and morbidity. In this review, the underlying molecular and cellular pa

Allopurinol gel mitigates radiation-induced mucositis and dermatitis.

It has not been verified whether allopurinol application is beneficial in decreasing the severity of radiation-induced oral mucositis and dermatitis. Rats were divided into 4 groups and received 15 Gy irradiation on the left whisker pad. Group 1 received only irradiation. Group 2 was maintained by applying allopurinol/carrageenan-mixed gel (allopurinol gel) continuously from 2 days before to 20 days after irradiation. Group 3 had allopurinol gel applied for 20 days after radiation. Group 4 was maintained by applying carrageenan gel continuously from 2 days before to 20 days after irradiation.

Topical photodynamic therapy for brachytherapy-induced oral mucositis.

Radioactive iodine-125 seed implantation has emerged as an effective treatment approach for locally advanced oral cancer. Nevertheless, some brachytherapy associated side reactions were reported, even at a relatively low initial radiation treatment volume. Radiogenic oral mucositis has been a concerning side effect of this treatment method. Photodynamic therapy (PDT) is a potential viable therapeutic strategy of oral mucositis. Herein, we report a case of 73-year-old male patient with ventral tongue and floor-of-the mouth cancer treated by iodine-125 implantation. Thereafter, this patient expe

Lyophilized Symbiotic Mitigates Mucositis Induced by 5-Fluorouracil.

Mucositis is an adverse effect of cancer chemotherapies using 5-Fluorouracil (5-FU). It is characterized by mucosal inflammation, pain, diarrhea, and weight loss. Some studies reported promising healing effects of probiotic strains, when associated with prebiotics, as adjuvant treatment of mucositis. We developed a lyophilized symbiotic product, containing skimmed milk, supplemented with whey protein isolate (WPI) and with fructooligosaccharides (FOS), and fermented by Lactobacillus casei BL23, Lactiplantibacillus plantarum B7, and Lacticaseibacillus rhamnosus B1.
