BACKGROUND: Continuous progression of lesions despite an adequate treatment has been described in tubercular multifocal serpiginoid choroiditis. Reported treatments for this paradoxical response include systemic steroids, immunosuppressive drugs, and intravitreal methotrexate. We describe the use of dexamethasone intravitreal implants in a patient presenting with this condition.
FINDINGS: A 46-year-old woman sought medical attention for scotomas in her left eye. Tests suggested multifocal serpiginoid choroiditis associated with latent tuberculosis infection, and hence, she was started on anti

The paper describes the first reported case of multifocal choroiditis following simultaneous hepatitis-A, typhoid, and yellow fever vaccinations. A 33-year-old male developed sudden onset of flashing lights and floaters in his right eye 3 weeks following hepatitis A, typhoid, and yellow fever vaccinations. Fundus examination and angiography confirmed the presence of multiple peripheral chorioretinal lesions. These lesions demonstrated characteristic morphologic changes over a period of 8 weeks which were consistent with a diagnosis of self-resolving multifocal choroiditis. Vaccine-induced intr

A patient with end-stage renal failure secondary to polycystic kidney disease was seen in the Eye Clinic for a corneal abrasion. Incidental fundal examination revealed bilateral multiple, small, raised, pale yellow sub-retinal nodules. Past medical history of inadequately treated pulmonary tuberculosis was obtained. Following initiation of anti-tuberculous therapy, the choroidal lesions resolved.

PURPOSE: To report a case of idiopathic multifocal choroiditis with transient peripapillary zonal inflammation that was followed with multimodal imaging and explain the mechanism by which chorioretinal atrophy may occur.
METHODS: Observational case report. Review of the clinical examination, ocular imaging, and progression of idiopathic multifocal choroiditis.
RESULTS: A 30-year-old white myopic man presented with complaints of worsening vision and a loss of visual field for 1 month in his left eye. Using multimodal imaging, including wide-field imaging with fluorescein angiography, fundus a

Six patients, consisting of 4 males and 2 females with mean average age fo 29.20 years, with variable picture of multifocal choroiditis were serologically investigated for their association with Epstein-Barr Virus (EBV) infection. IgG and IgM antibodies to EBV-Virus Capsid Antigen (VCA) were assayed by immunoperoxidase method on EBV infected lymphoblast cells (P3HR-1 Burkitt's lymphoma cell line). In these patients definite serological evidence of EBV infection with high titres of IgG and IgM antibodies to EBV-VCA was demonstrated suggestive of active continuing or persisting EBV infection.

We describe a patient with multifocal choroidal lesions affecting the midperipheral fundus, with an atrophic and scattered punched-out pale aspect. Lesions were discovered seven months after an ipsilateral herpes zoster ophthalmicus. Fluorescein angiography findings exhibited a delay of choroidal injection and late moderate staining during the venous phase. The etiological arguments refer to a previous herpes zoster infection. The pathogeny would involve an occlusion of posterior ciliary arteries, lesions of posterior ciliary nerves and/or direct cytopathogenic involvement of chorioretina by n

PURPOSE: This study describes a group of patients with clinical findings of peripheral punched-out lesions and panuveitis who had non-caseating granulomas found by non-directed conjunctival biopsy.
METHODS: A series of 10 patients seen at The University of Iowa Hospitals and Clinics between August 1989 and August 1990 with ocular findings similar to those of multifocal choroiditis with panuveitis, including peripheral punched-out chorioretinal lesions, vitritis, and frequently, cystoid macular edema, was examined. All patients underwent ophthalmic examination, ancillary testing, and non-direc

The prevalence of the HLA-B7 and HLA-DR2 specificities in 17 unrelated patients with multifocal choroiditis and panuveitis, 11 with and six without subretinal neovascularisation, was evaluated and compared with those of two different groups. The first group was 17 patients with subretinal neovascularisation associated with presumed ocular histoplasmosis syndrome, and the second was a group of 105 eye patients with no retinal disease. HLA-DR2 was not found in any patient with multifocal choroiditis and panuveitis, but it was found in 13 patients with presumed ocular histoplasmosis syndrome (p =

7 cases of multifocal choroiditis and panuveitis are reported here (6 females, 1 male). All clinical data were carefully considered. In all cases an aqueous sampling was made for the detection of anti-herpes virus antibodies in aqueous and serum. 3 specificities were tested: herpes simplex (HSV), herpes zoster (HVZ) and cytomegalovirus (CMV). An intraocular synthesis of specific antibodies was found against VZV in 2 cases and against HSV in 1 case. There was another presumptive case for HSV.

Multifocal choroiditis is a syndrome that simulates the presumed ocular histoplasmosis syndrome but, in addition, includes vitritis and anterior uveitis. Because of its relatively recent acceptance as a distinct clinical entity, there is little information available on the long-term natural history of this condition. We managed three cases of multifocal choroiditis with recurrent active inflammation. Two patients had new peripheral chorioretinal lesions in previously normal retina, and the other had both peripheral chorioretinal scars and new swelling of the disc.

BACKGROUND: Multifocal choroiditis with panuveitis is a rare disease. The educational merit of this case presentation results from the good documentation and the impressive ocular fundus pictures.
CASE PRESENTATION: We illustrate the 3-year course of disease in a 22-year-old myopic white woman with multifocal choroiditis with panuveitis and secondary choroidal neovascularization. The activity of the disease was evaluated clinically by optical coherence tomography and fluorescein angiography. Choroidal neovascularization was treated by intravitreal bevacizumab (2.5 mg/0.1 ml). PURPOSE: To evaluate the characteristics of multifocal choroiditis and panuveitis (MCP) and punctate inner choroidopathy (PIC) using multimodal imaging.
METHODS: This is a retrospective, consecutive, observational case series of 38 eyes of 22 patients. Each eye of patients with multiple yellow-white idiopathic inflammatory lesions in the fundus was classified as having MCP or PIC using standard diagnostic criteria in a masked fashion. The features of these eyes as determined from color fundus photography, spectral-domain optical coherence tomography, fundus autofluorescence, and angiography w

PURPOSE: The study aims to evaluate a series of patients with initial diagnosis of ocular histoplasmosis syndrome (OHS) with progression and response to treatments consistent with multifocal choroiditis (MFC).
METHODS: Retrospective review of nine patients referred for management of recurrent OHS lesions. Serology panel was conducted to rule out autoimmune and infectious causes.
RESULTS: Clinical examination revealed multiple small, punched-out peripheral chorioretinal scars, and peripapillary atrophy. Histoplasma antigen/antibody was negative in all patients. Fluorescein angiography and opt

We studied nine patients with the picture of presumed ocular histoplasmosis. All patients had the characteristic triad: punched out peripheral lesions, peripapillary atrophic scar and disciform process in the posterior pole. All patients had negative test for histoplasmosis (electrosyneresis). Four patients were observed for a long period (8, 8, 7 and 4 years). In two cases we could observe the first step of the basic choroidal lesions. One of these patients fundus was previously normal. Age, female to male ratio and in one case mild inflammatory lesions were similar to a new entity: multifoca

PURPOSE: We report a case of an atypical multifocal choroiditis with retinal vasculitis associated with positive West Nile serology.
METHODS: Retrospective case report.
RESULTS: A healthy 18-year-old man presented with floaters in the left eye that started while backpacking in Wyoming. Examination and testing revealed numerous chorioretinal lesions in the left eye, as well as retinal vasculitis and evidence of branch retinal vein occlusion. Serology was positive for West Nile Virus.
CONCLUSION: West Nile virus may be a trigger for development of multifocal choroiditis in susceptible individ

A woman in her late 50s presented with on-and-off redness and diminution of vision in her left eye for 6 months. Her best corrected visual acuity was 20/40 in the right eye and hand motion in the left eye. Anterior segment examination revealed a greyish-white lesion extending from 3 to 6 o'clock hours posterior to the iris and protruding into the anterior chamber. Left eye B-scan ultrasonography showed a multifocal choroidal lesion, a smaller one involving the posterior pole, and a larger lesion involving the complete nasal quadrant and anteriorly extending to the ciliary body and iris.
