Myasthenia gravis is a neuromuscular disease associated with antibodies against components of the neuromuscular junction, most often against the acetylcholine receptor (AChR). Although several mechanisms have been postulated to explain how these autoantibodies can lead to the pathology of the disease, convincing evidence suggests that destruction of the receptor-bearing postsynaptic membrane by complement membrane attack complex is of central importance. In this review, evidence for the importance of complement, and possible relationships between autoantigen, autoantibodies, complement activat…

The diagnosis of autoimmune Myasthenia Gravis (MG) remains clinical and rests on the history and physical findings of fatigable, fluctuating muscle weakness in a specific distribution. Ancillary bedside tests and laboratory methods help confirm the synaptic disorder, define its type and severity, classify MG according to the causative antibodies, and assess the effect of treatment objectively. We present an update on the tests used in the diagnosis and follow-up of MG and the suggested approach for their application.

Introduction: This study aimed to analyze the clinical features of myasthenia gravis (MG) in combination with the afterdischarges and compare the characteristics of afterdischarges in MG with different serum antibodies. Methods: Ninety-two patients with MG were analyzed retrospectively. The afterdischarges were investigated using motor nerve conduction examination, F-wave examination, and repetitive nerve stimulation (RNS). Results: Afterdischarges were observed after the M wave in 14 of 92 patients. Three of these 14 patients tested positive for the muscle-specific tyrosine kinase antibody (M…

Myasthenia gravis is an autoimmune disease characterised by fluctuating muscle weakness, which worsens during activity. It affects particularly scapular and pelvic girdles, axial and bulbar muscles. Myasthenia gravis is twice more frequent in women and symptoms often appear in the second and third decade of life. Thus, a growing number of women affected by this condition become pregnant. To minimise the effects of myasthenia gravis on pregnancy and the newborn, and to avoid myasthenia crisis in the post-partum, the pregnancy must be planned as far as possible. During pregnancy, treatment must …

Myasthenia gravis (MG) is one of the autoantibody-mediated neuroimmunological diseases. Autoantibodies to the muscle-type nicotinic acetylcholine receptor (AChR) were detected in more than 80% of MG patients. The gene structure and pathogenicity of some AChR antibodies produced in MG have already been identified and elucidated, respectively. Therefore, the AChR antibody is similar in nature to innovative drug development targets with respect to MG treatment. Here, we discuss the development of molecular target drugs for AChR antibody-positive MG.

In a patient with myasthenia gravis, a life-threatening myasthenic crisis can occur, a complication of myasthenia gravis. The crisis can be triggered by, among other things, emotional stress, pain, infections and a reaction to medication. The policy for invasive dental treatment in a patient with myasthenia gravis must be aimed at preventing a crisis. Prior to the intervention, consultation with the neurologist in charge is necessary because sometimes optimisation of the medication or preventive measures are required. Treatment of a myasthenia gravis patient with amide local analgesic is possi…

In recent years, thymectomy has become a widespread procedure in the treatment of myasthenia gravis (MG). Likelihood of remission was highest in preoperative mild disease classification (Osserman classification 1, 2A). In absence of thymoma or hyperplasia, there was no relationship between age and gender in remission with thymectomy. In MG treatment, randomized trials that compare conservative treatment with thymectomy have started, recently. As with non-randomized trials, remission with thymectomy in MG treatment was better than conservative treatment with only medication. There are four majo…

Myasthenia gravis (MG) is a chronic autoimmune neuromuscular disease and is the most common disorder of neuromuscular transmission. MG is caused by a defect in the transmission of nerve impulses to muscles in which communication from nerves to muscles is interrupted at the neuromuscular junction. This interruption can cause significant impact to muscle functions, which can have serious consequences for a pregnant woman, especially during labor. This brief article, which is meant to be used as an easy-reference tool in the clinical setting, examines the disease process and its effect on the ant…

A century has passed since the discovery of the first neurotransmitter, acetylcholine, in 1914. Extensive research on acetylcholine and its function has provided us with important options to treat the formerly intractable neuromuscular disease myasthenia gravis (MG). In the first days of treatment for MG, acetylcholine esterase inhibitors are administered, which is only a symptomatic therapy. After overwhelming developments in immunology, the immunological mechanisms of MG at the molecular or genetic level were recognized. Destruction of the acetylcholine receptors through autoimmune attacks p…

Myasthenia gravis (MG) is an autoimmune disorder of the neuromuscular transmission, caused by auto antibodies against the nicotinic acetylcholine receptor. These antibodies of the IgG isotype are detected in 80-90% of generalized MG and in 50-70% of ocular MG. Seronegative MG is caused by humoral factors. Prevalence of MG lies between 1 in 10.000 and 1 in 50.000, with 2/3 of affected individuals being female. In the case of maternal myasthenia gravis, both the mother and the child may develop myasthenia symptoms with varying degrees of weakness and progressive fatigability of the skeletal musc…

The international journal of tuberculosis and lung disease : the official journal of the International Union against Tuberculosis and Lung Disease2013eng
SETTING: The relationship between myasthenia gravis (MG) and tuberculosis (TB) has not been determined.
OBJECTIVE: To evaluate the relationship between MG and TB.
DESIGN: A national survey conducted between 2000 and 2006 included 2317 patients with MG identified from the Taiwan National Health Insurance database. The incidence rate ratio of TB in these patients was compared with those in 23 170 randomly selected age-, sex- and comorbidity-matched controls without MG.
RESULTS: The risk of TB was higher in the MG cohort (adjusted hazard ratio [aHR] 1.96, 95% confidence interval [CI] 1.22-3.16…

Acquired myasthenia gravis is a relatively uncommon disorder, with prevalence rates that have increased to about 20 per 100,000 in the US population. This autoimmune disease is characterized by muscle weakness that fluctuates, worsening with exertion, and improving with rest. In about two-thirds of the patients, the involvement of extrinsic ocular muscle presents as the initial symptom, usually progressing to involve other bulbar muscles and limb musculature, resulting in generalized myasthenia gravis. Although the cause of the disorder is unknown, the role of circulating antibodies directed a…

Myasthenia gravis (MG) is an antibody-mediated, neuromuscular transmission disorder, which ranges in clinical manifestations of ocular myasthenia that may be visually disabling to myasthenic crisis with patients suffering life-threatening respiratory insufficiency. MG also has pathophysiologic subgroups based on presence or absence of acetylcholine receptor or muscle-specific kinase antibodies and presence of thymoma. Cholinesterase inhibitors partially improve weakness, but the vast majority of patients require therapies that moderate the autoimmune attack. Mortality of MG has been reduced ov…

Myasthenia gravis (MG) is a disease affecting the nicotinic acetylcholine receptor of the post-synaptic membrane of the neuromuscular junction, causing muscle fatigue and weakness. The myasthenic patient can be a challenge to anesthesiologists, and the post-surgical risk of respiratory failure has always been a matter of concern. The incidence and prevalence of MG have been increasing for decades and the disease is underdiagnosed. This makes it important for the anesthesiologist to be aware of possible signs of the disease and to be properly updated on the optimal perioperative anesthesiologic…

Ocular myasthenia gravis (OMG) has no agreed-upon diagnostic and management criteria, leading to misdiagnoses and often misguided treatments. The purpose of this minireview is to provide guidance to clinicians who encounter possible OMG patients. We cite relevant literature and the recommendations for diagnosis and therapy based on the authors' extensive experience in OMG. This report provides sound recommendations based on the authors' successes and failures, coupled with relevant literature in the myasthenia gravis field.
