Non-tuberculous mycobacteria, particularly Mycobacterium avium complex (MAC), have become significant causes of pulmonary infections, especially in immunocompromised individuals. MAC lung disease presents a diagnostic challenge due to its clinical overlap with other pulmonary conditions, including Mycobacterium tuberculosis infections. The need for microbiological confirmation and radiological evaluation complicates early diagnosis. New molecular diagnostic methods, such as PCR, have improved detection but are not universally accessible. Treatment for MAC lung disease typically involves a mult

Mycobacterium avium complex (MAC) is a ubiquitous soil pathogen that is an uncommon cause of diseases in immunocompetent patients. In this case, we describe the presentation of an otherwise healthy man in his 50s presenting with months of malaise and severe hip pain, with aspiration initially yielding no bacteria and presumed fastidious infection. He was treated with irrigation and debridement, surgical stabilisation of the femoral neck and conventional broad-spectrum antibiotics with final cultures diagnostic of MAC osteomyelitis. This case serves to demonstrate the importance of clinical sus

BACKGROUND: Macrolide-resistant Mycobacterium avium complex (MAC) disease is very difficult to cure. Macrolide-resistance emerges in patients and is largely preventable by appropriate screening and treatment practices.
METHODS: Patients with macrolide-resistant MAC isolates between March 2019 and March 2022 were retrieved from the mycobacteriology reference laboratory database at Radboudumc, Nijmegen, the Netherlands. Clinical consultation reports were extracted from the database to assess the cause of macrolide resistance.
RESULTS: Sixteen patients with macrolide-resistant MAC disease were

Opportunistic infections are the result of infection by bacteria, viral, and fungal sources potentially leading to severe disease and death. These infections are a cause of significant morbidity and mortality among individuals with profound immunosuppression, namely human immunodeficiency virus (HIV), and organ transplant recipients on medications used to prevent organ rejection. Mycobacterium avium complex (MAC) is one of the most prevalent pathogens worldwide as it is found ubiquitously in water, food, and soil and is commonly a source of disseminated disease among the immunocompromised. BACKGROUND: Hemoptysis is a frequent and sometimes fatal complication of non-tuberculous mycobacterial (NTM) lung disease. The risk factors for hemoptysis are not well understood. In the current study, potential risk factors for hemoptysis were investigated in patients with Mycobacterium avium complex (MAC) lung disease, which is the most common NTM in Japan.
METHODS: Medical records from the Kinki-Chuo Chest Medical Center were reviewed. Consecutive patients with MAC lung disease diagnosed in 2014 and followed up for more than 1 year in the hospital were included in the study. Hemoptysis was

Disseminated atypical Mycobacterium infection is a well-known opportunistic infection in HIV-infected patients with advanced immune deficiency before the introduction of combination antiretroviral therapy. Although the disseminated infection is now rare, few cases of localised infections are reported. A 38-year-old man was diagnosed with HIV infection during asymptomatic sexual health screening. Although he was asymptomatic on diagnosis, he had advanced immunodeficiency; therefore, combination antiretroviral therapy was started immediately. After 5 months of treatment, he developed pericardial

BACKGROUND: Mycobacterium avium complex (MAC) pulmonary disease (PD) is often difficult and complicated to diagnose or to discriminate from follicular bronchitis, bronchiectasis, or other conditions associated with rheumatoid arthritis (RA) lung in the clinical setting.
OBJECTIVE: We investigated whether a serologic test for anti-glycopeptidolipid (GPL) antibody was useful for distinguishing MAC-PD from RA lung in diagnosis.
METHODS: Serum IgA antibody to MAC-specific GPL core antigen was measured by an enzyme immunoassay. Antibody levels were measured in sera from 14 RA patients with MAC-PD

BACKGROUND: Mycobacterium avium-intracellulare complex (MAC) is a ubiquitous pathogen found in soil and water. Environmental exposure is the primary route for MAC infection. However, specific environmental risk factors have been poorly determined in immunocompetent patients with pulmonary MAC disease.
METHODS: A case-control study was performed with 106 patients with pulmonary MAC disease (men [women], 23 [83]; age, 64.3 ± 9.2 years) and 53 age-matched control patients with bronchiectasis but not pulmonary MAC infection (men [women], 7[46]; age, 63.0 ± 11.0 years). All participants completed

INTRODUCTION: Mycobacterium avium complex (MAC) is an increasingly recognized cause of pulmonary disease in immunocompetent individuals. An acute form of MAC lung disease, MAC-associated pneumonitis, has generally been associated with the use of hot tubs. There is controversy in the literature about whether MAC-associated pneumonitis is a classic hypersensitivity pneumonitis or is a direct manifestation of mycobacterial infection.
CASE PRESENTATION: We report the second case in the literature of MAC-associated pneumonitis not related to the use of hot tubs. The source of MAC in a 52-year-old

BACKGROUND: To describe long term outcomes of Mycobacterium avium complex (MAC) immune reconstitution inflammatory syndrome (IRIS).
METHODS: Cases of MAC IRIS were retrospectively identified at four HIV clinics (Michigan, Maryland, Rhode Island, and Indiana) from 1996-2004. Patients were included if they were initially diagnosed with AIDS and found to have evidence of focal MAC infection documented by tissue culture or PCR after initiating HAART, and at least 6 months of follow up.
RESULTS: Among the 20 patients included, the mean age was 40 years, mean CD4 cell count was 24/mm3 at pretreatm

Mycobacterium avium complex (MAC) is ubiquitous. It is found in various freshwater and saltwater sources around the world, including hot water pipes. Although the organism was identified in the 1890s, its potential to cause human disease was only recognized 50 years later. Only a minority of people exposed to the organism will acquire MAC lung disease, usually those with underlying lung disease or immunosuppression. MAC may, however, cause progressive parenchymal lung disease and bronchiectasis in patients without underlying lung disease, particularly in middle-aged and elderly women. Prelimin

Since the emergence of AIDS, disseminated Mycobacterium avium complex (MAC) infection has become a growing cause of morbidity and mortality in this group of patients. Our objective was to study the incidence and the clinical and microbiological features of MAC infection in HIV-positive patients as well as the response to a therapy regimen combining clarithromycin and ethambutol. At our hospital, the first patient with disseminated MAC infection was diagnosed in 1988. Since then, 54 HIV-positive patients with MAC infection have been diagnosed (30/1,000 HIV-positive patients). MAC represented 12

To study the pathogenesis of Mycobacterium avium complex (MAC) bacteremia, the extent of organ involvement was determined in a retrospective cohort of 44 AIDS patients with MAC bacteremia and complete autopsies between 1988 and 1992. Clinical and microbiologic histories were reviewed and lymph nodes, spleen, liver, bone marrow, small intestine, and colon from each autopsy were systematically evaluated for the presence of mycobacteria or foamy histiocytes. Of the patients, 30% had no histologic evidence of MAC. In the remaining 70%, reticuloendothelial and gastrointestinal involvement was most

Six patients with bone marrow cultures yielding Mycobacterium avium complex were encountered at the Mount Sinai Hospital between 1969 and 1976. One additional isolate of the same mycobacterial species was recovered from splenic cyst fluid of a seventh patient. Because none of the patients had illnesses apparently due to M. avium complex, the isolates were unexpected and unexplained. Six of the seven patients had other acute or chronic infectious processes, occurring alone or superimposed on a preexisting disease. These patients were therefore unusual, because nontuberculous mycobacteria have p

Mycobacterium avium complex (MAC) is a nontuberculous mycobacteria that cause chronic pulmonary infections, particularly in individuals with underlying structural lung disease. Despite prolonged multidrug therapy, treatment outcomes for MAC pulmonary disease remain suboptimal, posing ongoing clinical challenges. Although β-lactams have demonstrated efficacy against Mycobacterium abscessus, β-lactams are not currently recommended for MAC, in part due to limited supporting data. Since M. abscessus and MAC have similar cell wall structures and peptidoglycan synthesis, we hypothesized that β-lacta

BACKGROUND: This case report describes an uncommon Mycobacterium avium infection mimicking lymphoma in a dog.
CASE PRESENTATION: A 1.5-year-old female Miniature Schnauzer presented with persistent lymphadenopathy, fever, and diarrhea, which were refractory to conventional therapy. Hematological and biochemical analyses revealed anemia, thrombocytopenia, elevated liver enzymes, creatine kinase, glutamate dehydrogenase, lactate dehydrogenase, hyperkalemia, low IgG levels, and euthyroid sick syndrome. Ultrasonography confirmed generalized visceral, retroperitoneal, and peripheral lymphadenopathy

We performed bedaquiline broth microdilution susceptibility testing using Clinical and Laboratory Standards Institute (CLSI) guidelines on 103 respiratory isolates of Mycobacterium avium complex (MAC), including multidrug-resistant isolates. Approximately 90% of isolates had bedaquiline MICs of ≤0.008 μg/ml, and 102/103 isolates had MICs of ≤0.015 μg/ml. Bedaquiline has excellent potential for use in patients with MAC infections, although for reasons of its metabolism by the cytochrome P450 system, it should not be given with rifampin.
