A new era in head and neck reconstruction has been opened with the use of myocutaneous flaps. The pectoralis major myocutaneous flap is considered the most reliable of its kind but flap failures still occur secondary to tension and infection. The modified pectoralis major myocutaneous paddle flap has been used in 75 cases since 1982 for the closure of major defects of the oral cavity, oro- and hypopharynx and neck. Although almost all of the patients had a stage IV tumour, we had good aesthetic and functional results. There was no total flap necrosis. Three cases of partial necrosis underwent spontaneous healing. The details of the operative techniques are compared with other reconstructive methods, and the advantage of the pectoralis major flap is discussed.

A long, U-shaped myocutaneous flap was designed to construct mediastinal tracheostomy in patients with a short tracheal stump. The technique was used in 1 patient who had tracheal carcinoma and whose tracheal stump was 1.5 cm and in 2 patients whose posterior part of the tracheal stump was 3 cm and 5 cm, respectively, and who had advanced cervicothoracic esophageal carcinoma. The flap provides a cosmetic effect and its construction saves time in the surgery of long-segment tracheal carcinomas and locally advanced cervicothoracic esophageal carcinomas.

The frontalis musculocutaneous island flap offers advantages of immediate repair for small to medium-sized forehead defects (0.5-5 x 0.5-5 cm). Based on the supraorbital artery, the flap is both adaptable and dependable. The nasalis myocutaneous sliding flap ("nasalis flap") is an axial island flap based on the branch of the angular artery that supplies the nasalis muscle. The technique for performing this flap for the repair of defects up to 1.5 cm in diameter on the supratip area of the nose is described in detail. Twenty-four cases with supratip defects following Mohs surgery repaired with the nasalis flap are reported. Flap survival was 100% and final cosmetic results were excellent.
