OBJECTIVE: To evaluate the results of myringoplasty in children 4 to 14 years old at the time of surgery.
DESIGN: Retrospective analysis of case notes for 100 consecutive children who had myringoplasty in a teaching hospital serving as a primary care and referral center.
METHODS: Between March 1994 and March 1999, patients 14 years or younger at the time of surgery were identified by the computer database. There were 118 procedures performed in 100 patients (18 had a second procedure performed in the contralateral ear at a later date). Twenty-three patients were excluded because they underwe

An analysis of the results of 100 temporalis fascia underlay myringoplasties in children between the ages of 2-17 years is presented. The follow-up period in this retrospective review was from 6 to 24 months. There was no statistically significant correlation between the age of patient, a history of previous ear surgery, previous general anaesthesia, previous adenotonsillar surgery, preoperative otorrhoea, impaired eustachian tubal function, individual surgeon performing the surgery, size of perforation or operative technique and the outcome of the surgery with respect to graft take rate. A series of 365 cases of tympanic membrane repair by the same surgeon is analysed, comparing the results of different techniques and grafting materials. A detailed study of 206 of these cases employing the underlay technique with temporal fascia is reported. In this group a graft take rate of 91.4 per cent in dry ears, and of 89.3 per cent in all cases, was achieved. Various factors liable to influence the success rate are analysed and discussed. No cases of conductive or sensori-neural hearing loss resulting from the operation were encountered.

Fat myringoplasty is a simple office technique for repairing tympanic membrane perforations. It involves wedging a piece of fat from the ear lobule into the perforation. A search of the literature failed to reveal any controlled studies on the efficacy of this method, which may allow patients to avoid more invasive procedures. Controlled perforations in guinea pig tympanic membranes were treated with fat myringoplasties, paper patches, or no intervention. Four weeks following the procedures, ears were examined and photographed under the microscope and studied histologically. The fat myringopla

We reviewed the records of 472 patients who had undergone myringoplasty during an 11-year period at the Otologic Medical Group. There was a statistically significant relationship between the size of the perforation and the degree of hearing impairment, but no relationship between the degree of impairment and the complaint of tinnitus. The majority of operations were performed using the lateral surface fascia grafting technique. Those 168 operations performed postauricularly were successful in closing the tympanic membrane perforation in over 97% of the cases and in reducing the conductive defi

BACKGROUND: Myringoplasty is a common procedure performed by otolaryngologists, particularly in the pediatric population. Tympanic membrane (TM) perforations can be caused by a variety of factors including retained tympanostomy tubes, infection and trauma. First described in 1878, myringoplasty has evolved to include many different materials for repair. Current materials used for myringoplasties include fascia, perichondrium, gelfoam, fat and paper. Multiple studies have looked at the success rates for these different materials. Fat graft myringoplasties in particular offer the advantage of a

The objectives of the study are to evaluate the feasibility, results and complications of the endoscopic transcanal cartilaginous myringoplasty. Thirty patients with a tympanic membrane perforation underwent a transcanal endoscopic cartilaginous myringoplasty, between June 2008 and January 2010. Three patients had a residual perforation at 2 months after surgery. At 1 year, the perforation was closed for 29 patients (96 %). There was no case of blunting, lateralization of the tympanic membrane or ossicular injury. Two patients had an iatrogenic superficial cholesteatoma in the tympanic membran

Day-case surgery is preferred for adults, allowing post-operative fast recovery in family environment and support. Myringoplasty using the traditional method of underlay temporalis fascia or tragal perichondrium is usually performed as an in-patient. From 2003 to 2004, 22 myringoplasty procedures were performed in a dedicated day surgery unit at the Hospital Melaka. We report the retrospectively review of the outcome results of these procedures. None of the patients need admission overnight. There were no surgical or anesthetic complications noted and this series suggests that day-case surgery

A retrospective study of patients who underwent myringoplasty as a day-case procedure in two freestanding day-surgery units during a five-year period was carried out. Their case notes were identified and details including age, site and size of perforation, grade of surgeon, surgical approach, graft material, post-operative dressings, overnight stay, complications, and outcome were noted and analysed. The total number of patients including both children and adults who had myringoplasty as a day case was 144. Of these, 125 patients with 143 procedures were included in this study. Their ages rang

Paediatric revision myringoplasty has received little attention. This study addressed the issue exclusively and reviewed the short- and long-term results of surgery in children between the ages of five and 15. Twenty-six out of 38 operated ears (68.4 per cent) were initially intact. The causes of immediate failure in decreasing order were: infection with graft necrosis, complete no-take of the graft and poor anterior adaptation of the graft. Age, size and site of perforation and surgeon's experience did not significantly affect the initial outcome of surgery. Six ears developed delayed re-perf

Day surgery has an ever-increasing role to play in otology. In this study of myringoplasty, tragal perichondrial grafts were placed permeatally as a day case procedure in 32 patients. These are compared with a control group, matched for age and for the size of their perforations, in whom temporalis fascia was grafted via an endaural or postaural incision. The success rate was 94% in the perichondrial group as compared with 84% in the control group (no significant difference P > 0.05). Day case myringoplasty produced less morbidity for the patients and a 40% reduction in theatre time as well as

OBJECTIVES: Where various surgical options exist for solving the same problem, and the results are similar, Occam's razor may be a suitable way of deciding which technique to use. The postaural [retroauricular] approach in myringoplasty is favoured generally, for its reported better overview of the middle ear. However, due to increasing interests in both patients' comfort and a more efficient use of operating time, transmeatal approach is a good alternative. The aim of this study is to investigate the difference in success rates between these two approaches and the factors that contribute to s

OBJECTIVE: A theoretical risk of iatrogenic sensorineural hearing loss (HL) during surgery has induced a reluctance to perform bilateral myringoplasty/tympanoplasty type I among some otosurgeons. This paper presents results of bilateral surgery in 26 patients.
MATERIAL AND METHODS: Twenty-six patients with bilateral, dry tympanic membrane perforations caused by chronic otitis media were selected prospectively for bilateral myringoplasty/tympanoplasty type I (52 ears) at a tertiary referral center. All patients had a HL corresponding to the size and localization of the perforation (no suspicio

The results after simultaneous bilateral fat graft myringoplasties in children have not been previously reported in the literature. We report on 28 children who underwent bilateral fat graft myringoplasties as outpatient procedures under general anesthesia. The success rate was 91% at final follow-up, with only 16% of children requiring insertion of pressure-equalizing (PE) tubes following surgery. No complications were documented. We concluded that bilateral fat graft myringoplasties are safe to perform, with an excellent success rate, and are suitable as outpatient procedures.
