Opiate antagonist improves neurologic recovery after spinal injury.

The opiate antagonist naloxone has been used to treat cats subjected to cervical spinal trauma. In contrast to saline-treated controls, naloxone treatment significantly improved the hypotension observed after cervical spinal injury. More critically, naloxone therapy significantly improved neurologic recovery. These findings implicate endorphins in the pathophysiology of spinal cord injury and indicate that narcotic antagonists may have a therapeutic role in this condition.

Cerebral vascular diseases.

Strokes remain an important cause of mortality and morbidity despite recent advances in neurologic intensive care. Although support of ventilation and circulation improves the outcome for stroke patients, the efficacy of cerebral resuscitative measures such as hyperventilation, osmotherapy, fluid restriction, steroids, intracranial pressure control, and barbiturates remains unproven. Current specific therapies (anticoagulation, surgery) are useful in carefully selected patients. New therapies now being researched include fluorocarbons, narcotic antagonists, and calcium blockers.

Classification of opioids on the basis of change in seizure threshold in rats.

Twenty opioids have been subdivided into four classes by using flurothyl-induced seizures in rats to measure dose-response relationships, stereospecificity, naloxone sensitivity, and tolerance-cross-tolerance. The data support current theories of multiple opiate receptor types. Since the receptors involved mediate effects that are antagonized, enhanced, or unaffected by naloxone, the model is uniquely suitable for detecting novel narcotic antagonists that can then be used to differentiate opiate receptors in other systems.

Effect of morphine antagonists on drug-induced hypothermia in mice and rats.

Since it has been reported that the narcotic antagonist analgesic, cyclazocine, has thymoleptic activity in man, certain neuropharmacological properties of this compound have been retrospectively analysed in rodents using diprenorphine, naloxone and RX 336-X 336-M (7,8-dihydro-5',6'-dimethylcyclohex-5'-eno 1',2',8',14 codeinone) as reference narcotic antagonists and morphine, desmethylimipramine and d-amphetamine as standard psychotropic agents. From interactional studies with alpha-methylparatyrosine (alpha-MT) in rats, and apomorphine, oxotremorine or reserpine in mice, the calorigenic activ

Double-blind comparison of butorphanol tartrate and meperidine hydrochloride in balanced anaesthesia.

Butorphanol tartrate, a synthetic morphinan which has analgesic agonist and antagonist properties, was compared with meperidine for balanced anaesthesia. The two agents were found to be comparable in efficacy, maintenance of cardiovascular stability, and incidence of side-effects. Butorphanol has the advantage of being non-narcotic and having a lower propensity for addiction. Because of its antagonist properties, there appears to be a limit to its depressant effects on respiration.

Psychopathology and endorphins.

Suggestive evidence exists linking endorphins to the schizophrenic syndrome; narcotic antagonists appear to slightly attenuate some symptoms, attentional performance is improved and CSF opiate-binding substances are reported to be elevated in a sub-group of patients. Many fewer affectively ill patients have been studied and little evidence has accumulated suggesting a relationship between symptoms of affective illness and endorphins although CSF endorphins appear elevated in some manic-depressive patients and "pain patients" with depression have higher CSF endorphins than pain patients without

A method for evaluating analgesic agents in rats.

A method is described for evaluating antinociceptive activity in rats using writhing responses induced by 4% sodium chloride solution. Narcotic and nonnarcotic analgesics, narcotic antagonists except naloxone, and antipyretic and nonsteroidal antiinflammatory drugs were effectively evaluated at relatively low doses using this method. Parenteral ED50 values of these compounds ran parallel to their clinical doses. The method detected not only the analgesic action of the above drugs but also that of methamphetamine. In spite of its high sensitivity, the method was specific in the sense that CNS-d

A simple animal test system to predict the likelihood of a drug causing human physical dependence.

The test system utilizes adult, male, Sprague-Dawley rats to consume their entire daily water intake in a single 60 minute session. The results of administering various doses of more than 70 therapeutic agents 15 minutes prior to the test session permit the classification of drugs into two categories. Group I drugs cause only a dose-dependent reduction in water intake, beginning at a threshold value below which no effect is seen. Group II drugs, upon reaching a threshold value, cause first a dose-dependent increase in water intake to a maximum; additional dosage increments produce a dose-depen

Short-term effects of naltrexone in 155 heroin ex-addicts.

The narcotic antagonist naltrexone was administered for periods of up to 8 months to a total of 155 patients at a dose of 40-200 mg per day. The antagonistic effect of naltrexone was tested by injections of heroin. Eighty milligrams of natrexone was effective for 48hr. The antagonistic effect decreased at 72 hr after the administration of 120-200 mg of naltrexone. Laboratory tests indicated no signs of toxicity. Naltrexone may elicit an increase in blood pressure and opigastric pain. Neither of these side effects appear clinically important. No signs of dependence on naltrexone were detected.

The enflurane sparing effect of morphine, butorphanol, and nalbuphine.

The potencies of morphine and of the narcotic analgesic agonist-antagonists butorphanol and nalbuphine in terms of their ability to decrease enflurane MAC were studied. Following the determination of control MAC for enflurane in each dog, an intravenous bolus dose of either butorphanol tartrate, nalbuphine hydrochloride, morphine, or placebo was administered and enflurane MAC was redetermined. A higher dose of the same drug was then administered and enflurane MAC was redetermined up to a total of four doses in each animal. The successive doses for morphine and nalbuphine were 0.5, 1.5, 5.0, an

Primary care of parenteral substance abusers.

Parenteral substance abuse is widespread. Although treatment for opiate dependence and cocaine abuse takes place in specialized programs, abusers frequently present in primary care settings for management of associated health problems. This article reviews the two drugs that are most commonly abused parenterally (heroin and cocaine); treatment options, including methadone maintenance and narcotic antagonists; and the primary care and health maintenance issues of this population. Psychological aspects of caring for these patients are discussed. Physical findings and diagnosis and management of

A unit for delivering multiple drug treatment and prevention services.

A diverse group of drug abusers in New Haven has benefited from a multifaceted treatment program at the Connecticut Mental Health Center's drug dependence unit. The unit is also involved in research, training, and community education: Treatment approaches include methadone maintenance, long- and short-term therapeutic communities, a day program for adolescents, and a low-intervention program using narcotic antagonists. Out of approximately 2400 applicants, 1600 have entered treatment, and 550 are considered drug-free program graduates. A long-term follow-up study is in progress to validate and

Antihistaminic-opioid combination: effect on locomotor activity in mice.

Various laboratory studies have been recently prompted by reports indicating abuse by heroin addicts of the narcotic agonist-antagonist pentazocine combined with the antihistaminic tripelennamine. For what concerns the effects on locomotor activity, it was demonstrated that three histamine H1 receptor antagonists, chlorpheniramine, diphenhydramine and tripelennamine enhance morphine-, but not amphetamine and scopolamine-induced hyperactivity in mice. These results suggested that antihistaminics may specifically interfere with locomotor effects of opioids. Such hypothesis was strengthened by fu

Opiate antagonists: a role in the treatment of hypovolemic shock.

The opiate antagonist naloxone has been used to treat shock following acute blood loss in conscious rats. Naloxone treatment rapidly increased mean arterial pressure and pulse pressure in this new shock model. More importantly, these blood pressure changes were sustained and survival was significantly increased with maloxone as compared with placebo treatment. From these findings, it may be inferred that endorphins may play a role in the pathophysiology of hypovolemic shock. It is suggested that narcotic antagonists may prove to be of therapeutic value in the treatment of shock.

Mass Spectrometric Imaging of the Brain Demonstrates the Regional Displacement of 6-Monoacetylmorphine by Naloxone.

Overdose is the main cause of mortality among heroin users. Many of these overdose-induced deaths can be prevented through the timely administration of naloxone (NLX), a nonselective mu (μ)-, kappa (κ)-, and delta (δ)-opioid receptor antagonist. NLX competitively inhibits opioid-overdose-induced respiratory depression without eliciting any narcotic effect itself. The aim of this study was to investigate the antagonistic action of NLX by comparing its distribution to that of 6-monacetylmorphine (6-MAM), heroin's major metabolite, in a rodent model using mass spectrometric imaging (MSI) in combi

Gastrointestinal Complications in Patients Who Undergo Radical Cystectomy with Enhanced Recovery Protocol.

Gastrointestinal (GI) complications are among the most common complications following radical cystectomy and urinary diversion. The most common is postoperative ileus, although its precise pathophysiology is not completely understood. Enhanced recovery after surgery (ERAS) protocols include evidence-based steps to optimize postoperative recovery and shorten hospital stay, mainly through expedited GI function recovery. They include avoiding bowel preparation and postoperative nasogastric tube, early feeding, non-narcotic pain management, and the use of cholinergic and mu-receptor opioid antagon

Effects of naloxone on systemic and cerebral responses to experimental concussive brain injury in cats.

This study examined the effects of bolus injections of naloxone hydrochloride, a specific narcotic antagonist, on systemic cardiovascular function, intracranial and cerebral perfusion pressures, blood gas status, and cortical encephalograms (EEG's) in 38 cats after two different grades of experimental brain injury. Naloxone had no prolonged effects on uninjured control animals. However, as compared to a saline-injected control group, naloxone significantly reversed the hypotension and reduction in pulse pressure seen after higher grades of injury. These changes persisted for at least 60 minute
