Tuberculosis, injecting drug use and integrated HIV-TB care: a review of the literature.

BACKGROUND: People who inject drugs (PWID) are at increased risk of tuberculosis (TB) and reduced retention in treatment. There is a need to document strategies for integrated delivery of HIV, TB and drug dependency care. METHODS: This article reviews the literature on rates of TB mono- and co-infection, and published and grey literature descriptions of TB and HIV-TB care, among PWID. RESULTS: Latent TB infection prevalence was high and active disease more common among HIV-positive PWID. Data on multidrug-resistant TB and co-infections among PWID were scarce. Models of TB care fell into six

Estimating the number of the people who inject drugs in Bali, 2010.

INTRODUCTION AND AIMS: A population estimation study was performed in 2009-2010 to understand the apparent decrease in people who inject drugs (PWID) attending needle syringe programs (NSPs) and to calculate the number of PWID in Bali for service planning. DESIGN AND METHODS: The multiplier method was applied to the results of a survey of 125 PWID using data from 16 institutions to calculate benchmarks for five events. Respondents were also asked to nominate other PWID and the services they used and to name PWID who had died in the previous 3 years. RESULTS: There was consistency in the esti

Unstable housing as a factor for increased injection risk behavior at US syringe exchange programs.

OBJECTIVE: To assess variation in injection risk behavior among unstably housed/homeless injecting drug users (IDUs) across programs in a national sample of US syringe exchange programs. METHODS: About 23 syringe exchange programs were selected through stratified random sampling of moderate to very large US syringe exchange programs operating in 2001-2005. Subjects at each program were randomly sampled. Risk behavior interviews were collected using audio-computer assisted self-interviewing (A-CASI). "Unstable housing/homelessness" was operationally defined as having lived "on the street or in

Updates on syringe coverage and service uptake among needle and syringe programs in the United States, 2019-2020.

BACKGROUND: As injection drug use has increased in the US, so too has the prevalence of receptive syringe sharing. Since the 1980s, Needle and Syringe Programs (NSPs) have been an important source of clean injection equipment and disposal of used syringes. This study reports national syringe coverage and examines the impact of program attributes on organizational-level service uptake, defined as number of syringes distributed per participant contact per year. METHODS: In 2019 and 2020, we administered an annual cross-sectional survey to NSPs operating in the US (n = 260). A national estimate

Longitudinal Description of State Laws Governing Syringe Services Programs and Syringe Possession and Distribution, 2010-2024.

This article reports new legal data on state syringe services program (SSP) and drug paraphernalia laws in the United States. Scientific legal mapping methods were used to develop a longitudinal legal dataset capturing state-level SSP and drug paraphernalia laws in effect from January 1, 2010, through June 1, 2024. During that period, the number of states with a legal mechanism allowing for syringe distribution from SSPs nearly doubled. However, legal barriers to SSP operations remain in many states.

Disgust or Dignity? The Moral Basis of Harm Reduction.

Harm reduction has been advocated to address a diverse range of public health concerns. The moral justification of harm reduction is usually presumed to be consequentialist because the goal of harm reduction is to reduce the harmful health consequences of risky behaviors, such as substance use. Harm reduction is contrasted with an abstinence model whose goal is to eradicate or reduce the prevalence of such behaviors. The abstinence model is often thought to be justified by 'deontological' considerations: it is claimed that many risky behaviors are morally unacceptable, and therefore that we ha

Addressing policy barriers to scaling up needle and syringe programmes: a global call to action.

Needle and syringe programmes (NSPs) are effective, affordable solutions for preventing the transmission of blood-borne viruses among people who inject drugs. Yet, global NSP coverage remains extremely low; only 2% of people who inject drugs live in countries with high coverage, and many low-income and middle-income countries do not have NSPs. This Health Policy reports outputs from an international working group who used implementation science approaches to prioritise barriers and co-design solutions to scale up NSPs across three domains: global policy, national policy, and procurement. We pr

Potential reasons for the decline of new HIV cases among people who inject drugs (PWID) in Kyrgyzstan.

There has been a declining trend in the number of reported cases of human immunodeficiency virus (HIV) among people who inject drugs (PWID) in Kyrgyzstan. The local HIV and public health community has suggested that this decline may be driven by interventions targeting HIV transmission among PWID, changes in drug use patterns, or possible underreporting of new HIV cases. The present study aims to examine these hypotheses using a deterministic compartmental model. Three intervention scenarios were evaluated to assess their impact on PWID population trends, HIV incidence, and case reporting. Sce

Critical studies of harm reduction: Overdose response in uncertain political times.

North America continues to witness escalating rates of opioid overdose deaths. Scale-up of existing and innovative life-saving services - such as overdose prevention sites (OPS) as well as sanctioned and unsanctioned supervised consumption sites - is urgently needed. Is there a place for critical theory-informed studies of harm reduction during times of drug policy failures and overdose crisis? There are different approaches to consider from the critical literature, such as those that, for example, interrogate the basic principles of harm reduction or those that critique the lack of pleasure i

Harm reduction in the USA: the research perspective and an archive to David Purchase.

The history of harm reduction in the USA has led to the development of some of the most important methods for treating persons for drug use disorders, such as methadone and buprenorphine for opiate use disorder. However, there has been fierce political resistance to implementation and scale-up of harm reduction in the USA. This resistance is rooted in historical demonization of particular psychoactive drugs that were associated with stigmatized racial/ethnic groups. With the discovery of acquired immunodeficiency syndrome (AIDS) in 1981, harm reduction became important not only for treating sub

Developing Safe Syringe Exchange Programs: Role of the North Carolina Division of Public Health.

Syringe exchange programs became legal in North Carolina on July 11, 2016. A combination of forces led to this progressive public health measure, including advocacy of the State Health Official, in a state characterized by a conservative political climate. Data collected by the division of public health were a key contributor to the initiative. Nearly 5 North Carolinians died each day from unintentional medication or drug overdose. High rates of coinfection including hepatitis B and C, human immunodeficiency virus, and endocarditis were shown to have substantial economic consequences. The Nort

Needle syringe programmes and opioid substitution therapy for preventing hepatitis C transmission in people who inject drugs.

BACKGROUND: Needle syringe programmes and opioid substitution therapy for preventing hepatitis C transmission in people who inject drugsNeedle syringe programmes (NSP) and opioid substitution therapy (OST) are the primary interventions to reduce hepatitis C (HCV) transmission in people who inject drugs. There is good evidence for the effectiveness of NSP and OST in reducing injecting risk behaviour and increasing evidence for the effectiveness of OST and NSP in reducing HIV acquisition risk, but the evidence on the effectiveness of NSP and OST for preventing HCV acquisition is weak. The impact of the COVID-19 pandemic on people who inject drugs accessing harm reduction services in an rural American state.

BACKGROUND: The impact of public health policies during the COVID-19 pandemic on people who inject drugs (PWID) has varied across regions. In other countries, recent research has shown that PWID access to harm reduction services, despite rapid adaptations, has been negatively impacted. Our study describes these impacts in a rural state. METHODS: We conducted semi-structured interviews with PWID, community partners, and healthcare providers in the rural state of Maine (USA). We explored how changes made during the pandemic impacted access to harm reduction services, including basic services (i

Syringe services programs: An examination of legal, policy, and funding barriers in the midst of the evolving opioid crisis in the U.S.

BACKGROUND: Syringe Services Programs (SSPs) have been proposed as a key intervention to address increasing rates of opioid injection, overdose, and infectious disease transmission in the U.S. In recent years, multiple states and jurisdictions have enacted laws and policies to enable implementation of SSPs. These statutory and regulatory changes have resulted in the expansion of SSPs in a short period of time under a patchwork of different regulations and policies. Understanding how SSPs are responding to this evolving policy environment in the midst of a worsening opioid crisis can inform the

Harm reduction policy in Taiwan: toward a comprehensive understanding of its making and effects.

BACKGROUND: In response to the spread of HIV caused by needle sharing among injection drug users (IDUs), the Taiwan Centers for Disease Control implemented a pilot harm reduction program in 2005 that expanded nationwide in 2006. The policy led to a significant reduction in the number of HIV-positive cases among IDUs in 4 years. METHODS: This article aims to provide a critical evaluation of this harm reduction policy in Taiwan. The research leading to this article included a thorough literature review and in-depth interviews with 31 active policy participants, including people working in hospi

HIV risk practices among needle exchange users and nonusers in Chicago.

OBJECTIVES: To assess associations between needle exchange program (NEP) use and drug injection risk practices. METHODS: Between 1997-2000, injecting drug users (IDUs) in Chicago were recruited from NEPs and an area with no NEP, interviewed about risk practices, and counseled and tested for HIV. The risk practices of "regular NEP users"--those who obtained at least half of their needles from an NEP (n = 558)--were compared with those of IDUs who did not use an NEP (n = 175). RESULTS: In multivariate analysis, regular NEP users, compared with NEP nonusers, were less likely to receptively shar

Injection Behaviors and Use of Syringe Service Programs over Time among People Who Inject Drugs in Baltimore, Maryland.

Introduction: People who inject drugs (PWID) are at increased risk for infectious disease transmission, including hepatitis C and HIV. Understanding trends in injection risk behaviors and syringe service program (SSP) use over time can help improve infectious disease prevention and other harm reduction services. Methods: Using National HIV Behavioral Surveillance System data from Baltimore, Maryland, we examined changes in receptive sharing of (1) syringes, (2) injection equipment, (3) syringes to divide drugs; and (4) receipt of syringes from SSPs among PWID from 2009 to 2018 (n = 518 in 2009
