Neoadjuvant Systemic Therapy (NAST) in Patients with Melanoma: Surgical Considerations by the International Neoadjuvant Melanoma Consortium (INMC).

Exciting advances in melanoma systemic therapies have presented the opportunity for surgical oncologists and their multidisciplinary colleagues to test the neoadjuvant systemic treatment approach in high-risk, resectable metastatic melanomas. Here we describe the state of the science of neoadjuvant systemic therapy (NAST) for melanoma, focusing on the surgical aspects and the key role of the surgical oncologist in this treatment paradigm. This paper summarizes the past decade of developments in melanoma treatment and the current evidence for NAST in stage III melanoma specifically. Issues of s

The role of adjuvant therapy in non-pCR patients after neoadjuvant chemoimmunotherapy: Comparison with neoadjuvant chemoradiotherapy for ESCC.

BACKGROUND: Neoadjuvant chemoimmunotherapy (NCIT) is increasingly used for locally advanced esophageal squamous cell carcinoma (ESCC), but the role of adjuvant therapy-particularly immunotherapy-after NCIT remains undefined. METHODS: This single-institution retrospective study included 458 ESCC patients treated with neoadjuvant chemoradiotherapy (NCRT, n = 187) or NCIT (n = 271) followed by esophagectomy (2021-2024). A 1:1 propensity score matching (PSM) based exclusively on pretreatment characteristics was used to compare the neoadjuvant strategies. To assess postoperative adjuvant therapy w

Neoadjuvant and Adjuvant Therapies for High-Risk Prostate Cancer.

High-risk localized prostate cancer (HR-PCa) presents a substantial risk of recurrence and progression despite definitive local treatment. This article evaluates the evolving role of neoadjuvant and adjuvant therapies, including androgen deprivation therapy, second-generation androgen receptor pathway inhibitors, and chemotherapy. While neoadjuvant strategies improve pathologic responses, survival benefits remain unproven. Adjuvant therapy is increasingly individualized, with early salvage radiotherapy favored over routine adjuvant radiotherapy based on recent trial data. Ongoing studies, mole

Updates in adjuvant and neoadjuvant therapies for resectable cutaneous melanoma.

Over the last 15 years, treatment of melanoma has been revolutionized using immune checkpoint inhibitors and mitogen-activated protein kinase-targeted therapies. However, some patients are at high risk of disease recurrence. Therefore, strategies to reduce the risk of recurrence are needed. Recent trials have evaluated the use of immune checkpoint inhibitors prior to stage III/IV melanoma resection (neoadjuvant treatment) in comparison to conventional use of these drugs after surgery (adjuvant treatment). In addition, checkpoint inhibitors are now being studied for stage IIb/IIc melanoma. Here

Updates and Review of Neoadjuvant Therapy for Resectable Pancreatic Cancer.

BACKGROUND: Pancreatic adenocarcinoma (PDAC) is a disease process that carries high morbidity and mortality and is suspected to supplant colorectal cancer as the second leading cause of cancer-related death in the USA. Resectable disease is considered no tumor contact with the celiac axis, common hepatic artery, or superior mesenteric artery and less than 180-degrees contact with portal vein or superior mesenteric vein. The current standard of care for resectable disease is surgery. SUMMARY: There is an ongoing paradigm shift to consider neoadjuvant therapy (NAT) for initially resectable dise

Total neoadjuvant therapy for rectal cancer: a guide for surgeons.

The modern management of rectal cancers continues to evolve. With the release of data from new landmark randomized controlled trials (RAPIDO, PRODIGE-23), total neoadjuvant therapy (TNT) has moved to the forefront of locally advanced rectal cancer treatment and is considered a standard option in selected patients. Total neoadjuvant therapy promises enhanced systemic disease control, better treatment adherence and less time with an ostomy. However, TNT as currently described encompasses a number of different potential treatment options that differ significantly in terms of their radiation dosag

Surgical resection criteria and neoadjuvant therapies for intrahepatic cholangiocarcinoma.

The staging of intrahepatic cholangiocarcinoma (ICC) is complex, and there is no consensus among international cancer groups on how to most appropriately select candidates with nonmetastatic disease for surgical resection. Factors contributing to a higher stage of disease include larger tumor size, multiple tumors, vascular invasion (either portal venous or arterial), biliary invasion, involvement of local hepatic structures, serosal invasion, and regional lymph node metastases. For patients selected to undergo surgery, it is well-documented that R0 resection translates to a survival benefit.

Pathology of neoadjuvant therapy and immunotherapy testing for breast cancer.

Neoadjuvant chemotherapy (NACT) has become the standard of care for high-risk breast cancer, including triple-negative (TNBC) and HER2-positive disease. As a result, handling and reporting of breast specimens post-NACT is part of routine practice, and it is important for pathologists to recognise the changes in tumour cells, tumour-associated stroma and background breast tissue induced by NACT. Familiarity with characteristic stromal features enables identification of the pre-treatment tumour site and allows confident diagnosis of pathological complete response (pCR) which is important for dec

Caffeine as a Neoadjuvant Therapy in Parathyroid Adenomas: A Narrative Review.

Caffeine is the most used central nervous system stimulant drug to date. Many studies have shown the association of caffeine with bone remodeling, urinary calcium excretion, kidney stones, acid peptic disease, and the development of cancer. However, there has been very little research exploring the association between caffeine use and parathyroid gland disorders. We shed light on the possible connection between caffeine and parathyroid adenomas, as suggested in the literature.

Role of chemoradiotherapy in oesophageal cancer -- adjuvant and neoadjuvant therapy.

Despite low postoperative mortality rates, the long-term outcomes from surgical-based treatment for oesophageal cancer remain poor. Chemoradiotherapy (CRT), either given before surgical resection as neoadjuvant therapy or after resection as adjuvant therapy, has been postulated to improve these outcomes. This systematic review examines the evidence for these approaches. The evidence for postoperative radiotherapy is limited and conclusions are difficult, but it may have a role in patients at high risk of local relapse (positive margins). The addition of chemotherapy is recommended when possibl

[Staging and neoadjuvant therapy of squamous cell carcinoma of esophagus].

Once the diagnosis of esophageal cancer is established, the decision on treatment will depend on the stage of the disease. Since improvement of prognosis can only be expected in patients with complete removal of their tumor, preoperative staging plays a pivotal role in the decision-making process. Preoperative diagnostic procedures should define the tumor in its relation to the tracheal bifurcation (site), determine the depth of tumor invasion (T status), evaluate regional lymph node metastases (N1 disease) and exclude distant metastases (M1 disease). Endosonography represents currently the mo

Precision before incision: neoadjuvant tumor-agnostic therapies.

The emergence of tumor-agnostic therapies has fundamentally transformed precision oncology. Traditionally developed and approved for metastatic disease, nine therapies spanning immunotherapy, targeted agents, and antibody-drug conjugates now demonstrate a compelling rationale for earlier deployment in the neoadjuvant setting. This review examines the paradigm shift from last-resort to first-consideration approaches. Key advantages of neoadjuvant, biomarker-guided therapy include intervention prior to the development of therapeutic resistance, optimal immune system engagement, and opportunities

Neoadjuvant Reirradiation for Radiation Therapy-Associated Angiosarcoma of the Breast.

PURPOSE: Radiation-associated angiosarcoma of the breast (RAASB) is a rare side effect after breast radiation and has been associated with poor outcomes. At this time, there is no consensus regarding management of RAASB, and the role of reirradiation remains controversial. We present our modern institutional outcomes in managing RAASB with incorporation of neoadjuvant hyperfractionated reirradiation. METHODS AND MATERIALS: Patients identified were treated between 2016 and 2020 with inclusion of any histologically proven RAASB without metastatic disease at diagnosis, while excluding those with

Neoadjuvant Therapy for Duodenal and Ampullary Adenocarcinoma: A Systematic Review.

BACKGROUND: The role of systemic therapy in the management of ampullary (AA) and duodenal adenocarcinoma (DA) remains poorly understood. This study sought to synthesize current evidence supporting the use of neoadjuvant therapy (NAT) in AA and DA. METHODS: The study searched PubMed, Cochrane Library (Wiley), Embase (Elsevier), CINAHL (EBSCO), and ClinicalTrials.gov databases for observational or randomized studies published between 2002 and 2022 evaluating survival outcomes for patients with non-metastatic AA or DA who received systemic therapy and surgical resection. Current and future role of neoadjuvant therapy for breast cancer.

Neoadjuvant systemic chemotherapy is a possible therapeutic approach for the treatment of locally advanced operable, primarily non-operable or inflammatory breast cancer. Neoadjuvant systemic chemotherapy is an option for breast cancer patients who would require adjuvant chemotherapy otherwise based on clinical and histological examination and imaging. The use of neoadjuvant systemic therapy in operable breast cancer is currently increasing because of its advantages that include higher rates of breast conserving surgery and the possibility of measuring early in-vivo response to systemic treatm

[What to expect from the neoadjuvant therapy of the oesophageal carcinoma?].

AIM: The aim of this study was to assess the neoadjuvant therapy of the oesophageal carcinoma firstly from the perspective of the immediate effect on the resection procedure itself and, secondly, from the perspective of the long-term results. METHODOLOGY: Prior to the resection of the oesophagus, the patients were administered the neoadjuvant therapy. The patients were allocated to three groups with the following branches of the cytostatic treatment: CDDP+FU, TAX+FU a CDDP. A smaller group with a less advanced disorder was treated only surgically, with the oesophageal resection. Furthermore,

Current status of neoadjuvant therapy for adenocarcinoma of the distal esophagus.

Prospective studies dealing with preoperative therapy in adenocarcinoma of the esophagus alone are rare. The interpretation of the preferential phase II trials and a few phase III trials is complicated, as most studies include adenocarcinoma of the esophagus (i.e., Barrett's carcinoma), adenocarcinoma of the esophagogastric junction (including cardia carcinoma and subcardia carcinoma), or squamous cell carcinoma. Preoperative chemotherapy, generally well tolerated, cannot decrease the incidence of local failure beyond the level achieved with surgery alone, but it might delay systemic relapse.
