Diagnosis of patent ductus arteriosus by a neonatologist with a compact, portable ultrasound machine.

OBJECTIVES: To conduct a pilot study assessing a neonatologist's accuracy in diagnosing patent ductus arteriosus (PDA) using compact, portable ultrasound after limited training. STUDY DESIGN: Prospective study of premature infants scheduled for echocardiography for suspected PDA. A neonatologist with limited training performed study exams before scheduled exams. Sensitivity and specificity were calculated, compared to the scheduled echocardiogram interpreted by a cardiologist. RESULTS: There were 24 exams. Compared to the scheduled exam, the neonatologist's exam had sensitivity 69% (95% conf

Inherited disorders of mitochondrial fatty acid oxidation: a new responsibility for the neonatologist.

The disorders of mitochondrial fat oxidation present clinically with three major clinical phenotypes: hypoketotic hypoglycemia, cardiomyopathy, and myopathy. Although these features can present together in some of the disorders, one will be the dominant clinical problem. This review will describe these clinical phenotypes while addressing the diagnostic value of various clinical and laboratory studies which are often used for making these diagnoses. With knowledge of the clinical presentation, these diagnoses can often be made very rapidly and at relatively low cost by more specific laboratory

Congenital Conundrum: Unraveling the Puzzle of Double Outlet Right Ventricle for the Neonatologist.

Double outlet right ventricle (DORV) is a complex congenital heart disease with a wide spectrum of anatomical variations and clinical presentations. In this conotruncal anomaly, greater than 50% of the semilunar valve diameter of both the aorta and pulmonary artery arises from the morphologic right ventricle. Additionally, patients with DORV have a ventricular septal defect that allows blood to egress from the left ventricle and may obstruct either the pulmonary or systemic circulations. Physiological characteristics differ among patients based on the orientation of the great arteries, the pre

Evaluation of Neonatologist-Provided Echocardiography in a Tertiary NICU in the United Arab Emirates.

AIM: Our study aimed to review the safety profile of the echocardiography service provided by the neonatologist in our hospital. Until recently, echocardiography was the domain of paediatric cardiologists; today, neonatologists commonly use it to diagnose and manage newborns in many tertiary neonatal units. We achieved this by reviewing the indications and outcomes of neonatal echocardiography. METHOD: We conducted a retrospective analysis of all neonatal echocardiograms performed between April 2013 and December 2016 at our institution. Data were collected on indications, echocardiographic di

Bird's Eye View of a Neonatologist: Clinical Approach to Emergency Neonatal Infection.

Though the incidence of neonatal infection in term and near-term infants is relatively low, incidence of infection in preterm very low birth weight infants is as high as 20-30% and may result in neurodevelopmental impairment or mortality. Pediatricians should be familiar with recognition and emergency management of life-threatening neonatal infections, such as congenital pneumonia, early onset sepsis, late onset sepsis, bacterial and fungal meningitis, disseminated neonatal herpes simplex virus (HSV), and HSV meningoencephalitis. For the pediatrician, it is logical to approach the management o

[Planning delivery in cases of congenital malformations from the neonatologist's point of view].

Nowadays, most major congenital malformations are detected by prenatal ultrasound long before birth. This offers the opportunity to plan and organise the birth of such a baby in the most favourable setting. This paper gives an overview about how to decide on place, timing, and mode of delivery in some typical congenital malformations from the neonatologist's point of view. As, in most cases scientific evidence is lacking, there is an urgent need for prospective clinical trials. The question concerning the best place of birth is answered in a national guideline, which has been consented by seve

Primary ciliary dyskinesia as a cause of neonatal respiratory distress: implications for the neonatologist.

We report two cases of term infants who presented with prolonged respiratory distress, rhinitis, and situs inversus. A high index of suspicion led to the diagnosis of Kartagener Syndrome, which is a subgroup of primary ciliary dyskinesia, in the immediate neonatal period.

Neonatal Acute Kidney Injury: A Survey of Neonatologists' and Nephrologists' Perceptions and Practice Management.

BACKGROUND: Neonatal acute kidney injury (AKI) occurs in 40 to 70% of critically ill neonatal intensive care admissions. This study explored the differences in perceptions and practice variations among neonatologists and pediatric nephrologists in diagnostic criteria, management, and follow-up of neonatal AKI. METHODS: A survey weblink was emailed to nephrologists and neonatologists in Australia, Canada, New Zealand, India, and the United States. Questions consisted of demographic and unit practices, three clinical scenarios assessing awareness of definitions of neonatal AKI, knowledge, manag

[Are in-utero interventions justified?--perspective of neonatologists. Part I. Congenital diaphragmatic hernia (CDH)].

INTRODUCTION: In-utero interventions are often perceived by parents as the only hope for their unborn child. Because it is neonatologists who have to deal with a sick newborn and sometimes unrealistic optimism of the parents after delivery we have taken on the task of reviewing the current knowledge concerning fetal surgeries from the neonatologist's perspective. In the first of three parts we have analyzed the data for in-utero interventions for CDH. OBJECTIVE: Our main objective was to evaluate available data and to ascertain whether performing fetal surgeries for CDH is justified. METHODS

Universal antenatal group B streptococcus screening? The opinions of obstetricians and neonatologists within Australia.

Group B streptococcus (GBS) is the leading infectious cause of morbidity and mortality in Australian newborns. Although intrapartum chemoprophylaxis is recommended to reduce the risk of neonatal GBS transmission and disease, controversy exists as to the best method to select women 'at risk' for this treatment. Our study aimed to survey the opinions of obstetricians and neonatologists currently in practice in Australia on GBS screening and treatment. Of the 488 obstetricians and 68 neonatologists currently in practice who responded to the survey, 271 obstetricians (56%) and 40 neonatologists (6

Parental influence on clinical management during neonatal intensive care: a survey of US neonatologists.

OBJECTIVE: Family-centered care (FCC), which includes involving parents in conversations about medical management, is increasingly employed in the neonatal intensive care unit (NICU). Our aim was to determine which care decisions are discussed by neonatologists with families most frequently and the percentage of clinicians influenced by such conversations. METHODS: Anonymous web-based survey provided to 2137 neonatologists assessing information sharing and parental involvement. RESULTS: Thousand and two neonatologists responded in which 893 fully completed the surveys. 88% practice FCC. Topi

Understanding perspectives of safety-net NICU neonatologists disclosing genomic sequencing results: a mixed methods approach.

OBJECTIVE: Examine neonatologists' perspectives on disclosing rapid genomic sequencing (rGS) results, including the barriers and facilitators they perceive. STUDY DESIGN: Neonatologists from 10 NICUs participating in the Virtual Genome Center (VIGOR) study were surveyed after each rGS disclosure and interviewed regarding their experience after 2 or more disclosures. Quantitative results were compared by result type, and qualitative results were iteratively coded for themes. RESULTS: 93% of neonatologists felt comfortable disclosing rGS results regardless of result type. Neonatologists utiliz

Anatomic Concordance of Neonatologist-Performed Echocardiography as Part of Hemodynamics Consultation and Pediatric Cardiology.

BACKGROUND: Targeted neonatal echocardiography (TnECHO) performed by neonatologists as part of a hemodynamics consultation is increasingly being used in neonatal intensive care units. To minimize delays in obtaining physiologic data, first echocardiograms may be obtained by the neonatal hemodynamics team and reviewed afterward by a pediatric cardiologist. This practice has not been systematically evaluated. The aim of this study was to compare concordance between anatomic findings on TnECHO and pediatric cardiology reports. METHODS: This was a retrospective evaluation of 339 infants at low ri

[Unexpected respiratory distress in the delivery room--a rare differential diagnosis for the obstetrician and neonatologist].

We present the case of a female infant born prematurely at 34 weeks of gestation. Prenatally a midsized ventricular septal defect was diagnosed. Due to marked respiratory distress intubation was attempted but failed, since the tube could not be placed beyond the glottis. Oxygenation could be improved by nasopharyngeal bag ventilation. The clinical course as well as radiographic imaging was suggestive for a complete tracheal agenesis with broncho-oesophageal fistula which was confirmed at autopsy. Tracheal agenesis (TA) is a rare differential diagnosis of postnatal respiratory distress and the
