Neuroimaging of Dilated Perivascular Spaces: From Benign and Pathologic Causes to Mimics.

Perivascular spaces (PVSs), also known as Virchow-Robin spaces, are pial-lined, fluid-filled structures found in characteristic locations throughout the brain. They can become abnormally enlarged or dilated and in rare cases can cause hydrocephalus. Dilated PVSs can pose a diagnostic dilemma for radiologists because of their varied appearance, sometimes mimicking more serious entities such as cystic neoplasms, including dysembryoplastic neuroepithelial tumor and multinodular and vacuolating neuronal tumor, or cystic infections including toxoplasmosis and neurocysticercosis. In addition, variou

Intracranial extracerebral neuroglial heterotopia: A case report and review of the literature.

Heterotopic masses of neuroglial tissue are uncommon and most frequently involve extracranial midline structures. We report an unusual case of an intracranial, extracerebral neuroglial heterotopia involving the middle and anterior cranial fossae of a 5-year-old girl who presented with facial asymmetry. The lesion was composed of mature but disorganized gray and white matter admixed with surrounding soft tissues and exhibited histologic features reminiscent of cortical dysplasia. These rare lesions have been postulated to arise from a protrusion of tissue from the neuraxis through a pial defect

DICER1 -Related Primitive Polyphenotypic Neoplasm : A Report of 15 Cases of an Underrecognized Tumor of the Gynecologic Tract and Peritoneum.

Somatic or germline pathogenic/likely pathogenic variants in DICER1 have known associations with certain neoplasms in the gynecologic tract, including Sertoli-Leydig cell tumors, embryonal rhabdomyosarcoma, and adenosarcoma. However, recent studies have highlighted DICER1- related malignant neoplasms with complex admixtures of sarcomatous, primitive glandular, and/or neuroectodermal elements, which are underrecognized and lack consistent nomenclature. We report the largest series of these primitive polyphenotypic DICER1 -related neoplasms arising in the gynecologic tract or peritoneum. The 15

Different immunohistochemical levels of Hsp60 and Hsp70 in a subset of brain tumors and putative role of Hsp60 in neuroepithelial tumorigenesis.

In this work we analysed, by immunohistochemistry, a series of brain tumors to detect the levels and cellular distribution of Hsp60 and Hsp70. We found that Hsp60 levels were significantly higher than those of Hsp70 in neuroepithelial tumors, while levels of both molecules were not significantly different from each other in meningeal neoplasms. In particular, Hsp60 immunopositivity was present mainly at the cytoplasmic level, while Hsp70 immunopositivity was found both in the cytoplasm and in the nucleus of tumor cells. The levels of these molecules in healthy control cells were always very lo

Desmoplastic non-infantile ganglioglioma in late adulthood.

Desmoplastic infantile ganglioglioma (DIG) is an uncommon supratentorial neuroepithelial brain tumor that typically occurs in infants younger than 24 months. Desmoplastic non-infantile ganglioglioma (DNIG) is a rare variant of this intracranial neoplasm. There are only 16 DNIG cases reported in the literature, with all patients under the age of 25 at the time of presentation. These DIG and DNIG cases were radiologically and histologically similar, with good outcome after treatment. Despite the size and high mitotic index for patients with DNIG, the prognosis is generally favorable and gross to

Pre-operative embolization for staged treatment of infantile choroid plexus papilloma.

Choroid plexus papillomas (CPPs) are benign but rare neuroepithelial neoplasms of the choroid plexus that represent the non-malignant form of a spectrum of tumors of the choroid plexus. The vast majority of CPPs present in children under 5 years of age. Some CPPs are diagnosed prenatally, but many of them reach a large size before diagnosis. CPPs typically present with signs and symptoms of hydrocephalus. Treatment of these tumors has traditionally been with surgical resection. Large CPPs in young children present a challenge due to risk of high blood loss during resection. Here, the authors d

Choroid Plexus Papilloma Arising From the Temporal Horn With a Bilateral Hypersecretory Hydrocephalus: A Case Report and Review of Literature.

Cerebrospinal fluid (CSF) within the cerebral ventricular system is secreted by a neuroepithelial tissue which is called as the choroid plexus. Tumors arising from these tissues are rare. Choroid plexus papillomas (CPPs) have been denoted as WHO grade I of the choroid plexus tumors. Among the intracranial tumors, neoplasms of the choroid plexus constitute around 0.36-0.6%. CPPs are mostly slow growing and cause symptoms due to mass effect and obstructive hydrocephalus, resulting in increased intracranial pressure. We report a case of CPP arising from the temporal horn in a 7-year-old girl pres

Neuroradiologic characteristics of astroblastoma and systematic review of the literature: 2 new cases and 125 cases reported in 59 publications.

BACKGROUND: Astroblastoma is a rare tumor of uncertain origin most commonly presenting in the cerebrum of children and young adults. The literature contains only case reports and small series regarding its radiologic features. This systematic review is the largest study of imaging findings of astroblastoma to date and serves to identify features that might differentiate it from other neoplasms. OBJECTIVE: This study describes the imaging features of astroblastoma based on a systematic review of the literature and two new cases. MATERIALS AND METHODS: We conducted a PubMed and Google Scholar

Intraventricular Astroblastoma in an Infant: A Case Report and Review of the Literature.

Astroblastomas are rare intracranial tumours of uncertain origin and are commonly present in the supratentorial compartment. Astroblastomas often affect children and young adults. Their occurrence in an intraventricular location, in an infant, is extremely uncommon. We describe a 1-year-old boy who presented with features of raised intracranial pressure. An MRI scan revealed an intralateral ventricular tumour. Complete excision was performed. Its histopathological examination revealed features consistent with astroblastoma. A follow-up MRI scan after 9 months did not show any residual tumour,

Cerebral astroblastoma with oligodendroglial-like cells: A case report.

RATIONALE: Astroblastoma is a rare tumor of the central nervous system with uncertain biological behavior and origin. Its histopathological features have been well established, while, to our knowledge, astroblastoma with oligodendroglial-like cells have not been reported. PATIENT CONCERNS: A 15-year-old girl presented with nausea, vomiting, headache, and visual disturbance. DIAGNOSIS: Magnetic resonance imaging revealed a large neoplasm in the left temporal. Histologically, the tumor showed solid and pseudopapillary structure. Immunohistochemical staining showed that the tumor cells were pos

Gliomatosis cerebri in a patient with Ollier disease.

Gliomatosis cerebri (GC) is an uncommon brain tumour defined as a diffuse neoplastic glial cell infiltration of the brain, involving more than two cerebral lobes and, occasionally, the infratentorial structures or the spinal cord. GC of the oligodendroglial phenotype is extremely rare, especially in the paediatric setting. We describe an unusual case of oligodendroglial GC diagnosed in a 16-year-old boy with Ollier disease. To our knowledge this is the first case of GC reported in a child with Ollier disease.

Desmoplastic infantile astrocytoma with metastases at presentation.

A 4-month-old child presented with nystagmus and macrocephaly. He had a large tumor in the suprasellar and hypothalamic region, as well as two smaller similar masses in the posterior fossa and one in the spinal canal. A biopsy of the suprasellar mass revealed it to be a desmoplastic infantile cerebral astrocytoma. Cerebrospinal fluid obtained at surgery before tumor manipulation showed clusters of malignant cells immunopositive for glial fibrillary acidic protein. In our opinion, the smaller tumors were metastases from the large suprasellar primary astrocytoma. Review of all of the previously

Paediatric-type diffuse low-grade gliomas: A pictorial review.

The fifth edition of the World Health Organization (WHO) Classification of Tumours of the Central Nervous System (WHO CNS5) introduces a revised framework for classifying brain and spinal cord tumours, emphasizing the crucial role of molecular diagnostics in accurately identifying neoplasms. A key advancement in this edition is the clear distinction between paediatric-type diffuse low-grade gliomas (pDLGGs) and paediatric-type diffuse high-grade gliomas (pDHGGs) as separate tumour entities. Although histologically and morphologically similar, paediatric diffuse gliomas differ significantly fro

Nestin expression in cortical dysplasia.

OBJECT: It is recognized that cortical dysplasia (CD) is associated with an increased incidence of glioneuronal neoplasms. Among hypothetical considerations, there is the possibility that CD and other neuronal migration abnormalities harbor dysmature cells with the potential to give rise to glioneuronal neoplasms. Such cells, if present, would be reasonably expected to display immature features. The goal of the present study was to characterize the expression of nestin, a neuroepithelial precursor/stem cell antigen, in CD, along with other pathological and clinical features of this entity. A Case of Dysembryoplastic Neuroepithelial Tumor in an HIV-Positive Adult: Diagnostic Lessons for Clinicians.

Dysembryoplastic neuroepithelial tumor (DNET) is a benign central nervous system neoplasm, usually diagnosed in pediatric patients with epilepsy. We describe a case of a 57-year-old man presenting with a first episode of generalized tonic-clonic seizure and subacute cognitive decline, in whom a multicystic right temporal lobe lesion was detected. Further workup led to a new diagnosis of human immunodeficiency virus (HIV) infection. Comprehensive investigations excluded infectious and malignant etiologies. Review of prior imaging, combined with multidisciplinary input, led to a diagnosis of DNE

Palisading pattern in cerebral neuroblastoma mimicking the primitive polar spongioblastoma. An ultrastructural study.

The present case study portrays a neoplasm composed predominantly of palisading uni- and bipolar cells with histological features mimicking the polar spongioblastoma. Ultrastructural examination of this single case reveals cells that range from the embryonal neuroepithelial cell to neurons with synapse formation and the presence of dense-cored vesicles in neurites. The morphology of these neoplastic cells is similar to the neuronal line of differentiation described in both normal developing mammalian central nervous system and in the transplantable mouse teratoma line (OTT-6050). Polymorphous low-grade neuroepithelial tumour of young (PLNTY): the new kid on the block.

INTRODUCTION: Polymorphous low grade neuroepithelial tumor of the young (PLNTY) is a newly described epileptogenic tumor first reported by Jason. T. Huse et al. in 2016. Only a very few cases have been reported so far and has been recently incorporated in the World Health Organization (WHO) Central Nervous System Classification of tumours, 5th edition, 2021. Here we report a rare case of PLNTY which closely resembles DNET (Dysembryoplastic neuroepithelial tumor) with plenty of interesting findings which would otherwise go unnoticed resulting in a nonspecific or misclassified diagnosis. Mice Expressing Myc in Neural Precursors Develop Choroid Plexus and Ciliary Body Tumors.

Choroid plexus tumors and ciliary body medulloepithelioma are predominantly pediatric neoplasms. Progress in understanding the pathogenesis of these tumors has been hindered by their rarity and lack of models that faithfully recapitulate the disease. Here, we find that endogenous Myc proto-oncogene protein is down-regulated in the forebrain neuroepithelium, whose neural plate border domains give rise to the anterior choroid plexus and ciliary body. To uncover the consequences of persistent Myc expression, MYC expression was forced in multipotent neural precursors (nestin-Cre:Myc), which produc

Renal neoplasms mimicking rhabdoid tumor of kidney. A report from the National Wilms' Tumor Study Pathology Center.

Fifty-six renal neoplasms reviewed by the National Wilms' Tumor Study Pathology Center presented with histologic features that resulted in confusion with rhabdoid tumor of kidney, a usually lethal childhood renal tumor; all were eventually diagnosed as other entities. Conspicuous filamentous cytoplasmic inclusions or large nucleoli, typical findings in rhabdoid renal tumors, were the usual source of diagnostic difficulty. Most, but not all, tumors occurred in pediatric patients. Sixteen were examples of Favorable Histology Wilms' tumor, which invited confusion with rhabdoid tumors either on th
