From Scenario to Reality: How Tabletop Exercises Transformed Emergency Management in Outpatient Dialysis Care.

Emergency preparedness is a critical component of patient safety in outpatient dialysis settings, where patients rely on life-sustaining treatments multiple times each week. Disruptions caused by natural disasters, infrastructure failures, or other emergencies can threaten the continuity of dialysis care and place this vulnerable population at significant risk. Tabletop exercises have emerged as an effective strategy for strengthening emergency readiness by allowing health care teams to rehearse response procedures in a structured, low-risk environment. These scenario-based simulations promote

Effective Strategies for Training Patients to Perform Home Dialysis.

Patient education is critical to the success of the patient on home dialysis. Ideally, pre-admission interventions should include a home environment assessment, assessment of patient and care partner commitment to therapy and level of engagement, assessment of patient support and resources, and assessment of patient and care partner readiness to learn. The training process used by the registered nurse (RN) working in home dialysis training, as well as initial and ongoing assessment of patient and care partner knowledge and skills, ensures the patient is positioned for the best outcomes. Acute Kidney Replacement Therapy (KRT) in Pediatric Patients: A National Survey Assessing Nursing Structure and Standards in KRT Education.

Kidney replacement therapy (KRT) is an essential treatment for children with acute kidney injury, fluid overload, and toxin removal. Successful provision of KRT is important; however, no standard nursing education currently exists. The top 50 pediatric nephrology programs from the 2023 U.S. News & World Report and eight Canadian centers were surveyed regarding their KRT programs. Most programs perform KRT in three or more intensive care units (n = 31, 58.5%), use two or more machines (n = 44, 83%), and use two or more anticoagulation strategies (n = 34, 86.8%). There are multiple programmatic

There Is No Difficult Patient.

Patients receiving chronic hemodialysis frequently experience significant physical, psychological, and emotional stress that often contributes to patients' dysregulation, treatment resistance, and behaviors labeled as "difficult." Traditional responses to these situations can unintentionally escalate the distress the patient is experiencing and negatively affect the staff-patient relationship. This article explores patient behavior through the lens of emotional intelligence, trauma-informed care, and nervous system regulation. Examples from real-world clinical experiences are used to demonstra

Does pausing ultrafiltration as a treatment for intradialytic hypotension cause backfiltration in hemodialysis?

Ultrafiltration is part of the hemodialysis procedure and can also be used exclusively to remove additional fluid in patients with fluid overload. Ultrafiltration that is too aggressive can result in hypotension, cramping, or other untoward effects. Temporarily pausing the ultrafiltration process is a technique used to help manage the adverse effects of rapid fluid removal. Some nurses prefer not to pause ultrafiltration because of the concern for backfiltration, and they elect to manage hypotension and cramping by normal saline infusions or other interventions. This article reviews the eviden

Red blood cell physiology, Epoetin alfa, and iron.

Optimal development of red blood cells (RBCs) in dialysis patients depends on an appropriate mix of Epoetin alfa and iron; the former provides the stimulus for RBC production, while the latter supports the formation of heme. Nephrology nurses are frequently empowered to manage anemia outcomes in the dialysis population, so an understanding of the physiology of RBC development and the roles of Epoetin alfa and iron is therefore essential.

[The role of nurses in the conservative management of advanced chronic kidney disease].

Conservative management is a recognized option for treating advanced chronic kidney disease without the need for dialysis, focusing on quality of life and symptom control. In France, the guidelines published in 2022 by the Francophone Society of Nephrology, Dialysis, and Transplantation serve as a practical reference for this approach. Based on continuity and coordination of care, this management model assigns nurses a central role in clinical assessment, patient support, and care pathway coordination.
