EEG-based diagnostics of the auditory system using cochlear implant electrodes as sensors.

The cochlear implant is one of the most successful medical prostheses, allowing deaf and severely hearing-impaired persons to hear again by electrically stimulating the auditory nerve. A trained audiologist adjusts the stimulation settings for good speech understanding, known as "fitting" the implant. This process is based on subjective feedback from the user, making it time-consuming and challenging, especially in paediatric or communication-impaired populations. Furthermore, fittings only happen during infrequent sessions at a clinic, and therefore cannot take into account variable factors t

Implantable microcoils for intracortical magnetic stimulation.

Neural prostheses that stimulate the neocortex have the potential to treat a wide range of neurological disorders. However, the efficacy of electrode-based implants remains limited, with persistent challenges that include an inability to create precise patterns of neural activity as well as difficulties in maintaining response consistency over time. These problems arise from fundamental limitations of electrodes as well as their susceptibility to implantation and have proven difficult to overcome. Magnetic stimulation can address many of these limitations, but coils small enough to be implante

Combining ENG and EEG integrated analysis for better sensitivity and specificity of neuroprosthesis operations.

Combining non-invasive monitoring of action-related brain signals with the invasive recordings of the nerve motor output could provide robust natural and bidirectional multimodal Brain-Machine interfaces. One 26 years old, right-handed male who had suffered traumatic trans-radial amputation of the left arm was connected in a bidirectional way with a robotic hand prostheses. Cortical signals related with movement programming, execution, and feed-back were recorded by non-invasive scalp electrodes to detect high-level information (i.e. onset of movement intention), while the efferent neural acti

Safety measures implemented for modular functioning electrical stimulators.

The modular architecture allows for greater flexibility in the building of neural prostheses with a variety of channels but may result in unpredictable accidents under circumstances such as sensor displacements, improper coordination of the connected modules and malfunction of any individual module. A novel fail-safe interface is offered as a solution that puts in place the necessary safety measures when building a module based functional electrical stimulator. By using a single reference line in the interconnecting bus of the modules, various commands would immediately be directed to each mod

Clinical applications of penetrating neural interfaces and Utah Electrode Array technologies.

This paper briefly describes some of the recent progress in the development of penetrating microelectrode arrays and highlights the use of two of these devices, Utah electrode arrays and Utah slanted electrode arrays, in two therapeutic interventions: recording volitional skeletal motor commands from the central nervous system, and recording motor commands and evoking somatosensory percepts in the peripheral nervous system (PNS). The paper also briefly explores other potential sites for microelectrode array interventions that could be profitably pursued and that could have important consequenc

Light-Based Neuronal Activation: The Future of Cranial Nerve Stimulation.

Despite advances in implant hardware, neuroprosthetic devices in otolaryngology have sustained evolutionary rather than revolutionary changes over the past half century. Although electrical stimulation has the capacity for facile activation of neurons and high temporal resolution, it has limited spatial selectivity. Alternative strategies for neuronal stimulation are being investigated to improve spatial resolution. In particular, light-based neuronal stimulation is a viable alternative and complement to electrical stimulation. This article provides a broad overview of light-based neuronal sti

Graphene-Integrated Ultrathin Neural Probe for Multiregional Cortical Recordings.

Electrophysiological measurement techniques are essential for understanding the functions of the central and peripheral nervous systems. Specifically, noninvasive neural probes, such as surface electrode arrays, provide stable electrophysiological recordings without eliciting an immunological response. However, the ability to capture complex interactions across multiple brain regions is limited by their localized recording site. Here, we present the "large-area NeuroWeb (LNW)", an ultrathin, minimally invasive neural probe designed for extensive cortical recording and stimulation. LNW consists

Grasp Posture Variability Leads to Greater Ipsilateral Sensorimotor Beta Activation During Simulated Prosthesis Use.

Motor behaviour using upper-extremity prostheses of different levels is greatly variable, leading to challenges interpreting ideal rehabilitation strategies. Elucidating the underlying neural control mechanisms driving variability benefits our understanding of adaptation after limb loss. In this follow-up study, non-amputated participants completed simple and complex reach-to-grasp motor tasks using a body-powered transradial or partial-hand prosthesis simulator. We hypothesised that under complex task constraints, individuals employing variable grasp postures will show greater sensorimotor be

Ankle Torque Estimation With Motor Unit Discharges in Residual Muscles Following Lower-Limb Amputation.

There has been increased interest in using residual muscle activity for neural control of powered lower-limb prostheses. However, only surface electromyography (EMG)-based decoders have been investigated. This study aims to investigate the potential of using motor unit (MU)-based decoding methods as an alternative to EMG-based intent recognition for ankle torque estimation. Eight people without amputation (NON) and seven people with amputation (AMP) participated in the experiments. Subjects conducted isometric dorsi- and plantarflexion with their intact limb by tracing desired muscle activity

Prosthetic interfaces with the visual system: biological issues.

The design of effective visual prostheses for the blind represents a challenge for biomedical engineers and neuroscientists. Significant progress has been made in the miniaturization and processing power of prosthesis electronics; however development lags in the design and construction of effective machine-brain interfaces with visual system neurons. This review summarizes what has been learned about stimulating neurons in the human and primate retina, lateral geniculate nucleus and visual cortex. Each level of the visual system presents unique challenges for neural interface design. Blind pat

A physiologically inspired meta-pattern generator bridging intention and motor primitives for human voluntary locomotion.

Understanding the hierarchical organization and transmission characteristics of neural signals-from motor intention, through spinal integration, to lower limb joint motions-is critical for developing physiologically plausible, intention-driven motion generation models. Such models not only advance our comprehension of the neurophysiological mechanisms underlying lower limb locomotion but also provide a biological foundation for the direct neural control of lower limb prostheses and rehabilitation robots, offering both theoretical significance and practical utility. This challenge has persisted

Hearing impairment is associated with enhanced neural tracking of the speech envelope.

Elevated hearing thresholds in hearing impaired adults are usually compensated by providing amplification through a hearing aid. In spite of restoring hearing sensitivity, difficulties with understanding speech in noisy environments often remain. One main reason is that sensorineural hearing loss not only causes loss of audibility but also other deficits, including peripheral distortion but also central temporal processing deficits. To investigate the neural consequences of hearing impairment in the brain underlying speech-in-noise difficulties, we compared EEG responses to natural speech of 1

Neural signal recording and processing in somatic neuroprosthetic applications. A review.

Neurointerfaces have acquired major relevance as both rehabilitative and therapeutic tools for patients with spinal cord injury, limb amputations and other neural disorders. Bidirectional neural interfaces are a key component for the functional control of neuroprosthetic devices. The two main neuroprosthetic applications of interfaces with the peripheral nervous system (PNS) are: the refined control of artificial prostheses with sensory neural feedback, and functional electrical stimulation (FES) systems attempting to generate motor or visceral responses in paralyzed organs. The results obtain

Integrating Motor Unit Activity With Deep Learning for Real-Time, Simultaneous and Proportional Wrist Angle and Grasp Force Estimation.

OBJECTIVE: Myoelectric prostheses offer great promise in enabling amputees to perform daily activities independently. However, existing neural interfaces generally cannot simultaneously and proportionally decode kinematics and kinetics in real time, nor can they directly interpret neural commands. We thus propose a novel framework that integrates motor unit activity with deep learning and demonstrate its efficiency in the real-time, simultaneous, and proportional estimation of wrist angles and grasp forces. METHODS: This framework utilizes real-time high-density surface electromyography decom

Neural implant for the treatment of multiple sclerosis.

The methods used to treat various neurological diseases are evolving. The facilities provided by the technology have led to creation of new treatment opportunities. Neuromodulation is one of these important methods. By definition, the neuromodulation is a change in neural activity which occurs by stimulating a specific area of nervous system. The mentioned stimulation can be electrical, magnetic, or chemical. This method is used in various diseases, such as stroke, Parkinson's, Alzheimer's, and amyotrophic lateral sclerosis (ALS). Multiple sclerosis (MS) is no exception in this regard and meth

Neural Vision Restoration in Ophthalmology.

Neural vision restoration is a rapidly advancing discipline at the intersection of neuroscience, bioengineering, and ophthalmology. This review synthesizes emerging strategies to restore visual perception through retinal prostheses, optic nerve and thalamic implants, cortical brain-computer interfaces (BCIs), optogenetics, and non-invasive stimulation. Although initial experiments have demonstrated primitive visual abilities such as light perception and motion detection, artificial vision remains cognitively demanding and fundamentally different  from natural vision. Separation of fascicles for motor unit separability in reinnervated muscles for neuroprosthesis application.

After amputation, muscle signals can be used as a signal source for advanced prosthetic control. Innovative surgical techniques like muscle reinnervation surgeries have proven to be an advanced option. This study introduces a novel variation of the traditional muscle reinnervation approaches to provide a more robust and flexible signal source in prosthetic limbs. By implanting two separated fascicles at distant locations within a single muscle, our approach facilitates more effective innervation of the muscle to increase the spatial separability of the signal. The efficacy of hybrid neuroprostheses in the rehabilitation of upper limb impairment after stroke, a narrative and systematic review with a meta-analysis.

BACKGROUND: Paresis of the upper limb (UL) is the most frequent impairment after a stroke. Hybrid neuroprostheses, i.e., the combination of robots and electrical stimulation, have emerged as an option to treat these impairments. METHODS: To give an overview of existing devices, their features, and how they are linked to clinical metrics, four different databases were systematically searched for studies on hybrid neuroprostheses for UL rehabilitation after stroke. The evidence on the efficacy of hybrid therapies was synthesized. RESULTS: Seventy-three studies were identified, introducing 32 h
