Novel technique of switching TIVA and sevoflurane during epilepsy surgery for combined intraoperative motor evoked potentials monitoring and electrocorticography: an illustrative case report.

BACKGROUND: During epilepsy surgery, it is equally important to record electrocorticography (ECoG) for detecting epileptogenic activity and guiding brain resection, and to evaluate neuromonitoring data, particularly motor evoked potentials (MEP), for avoidance of postoperative neurological complications. However, sevoflurane, which is commonly used during recording of ECoG, may attenuate the MEP response. It enforces anesthesiologists and neurosurgeons to select one anesthetic agent over another, facilitating either ECoG or MEP monitoring. CASE PRESENTATION: In the presented case of a 20-year

Density Spectral Array Patterns During Total Intravenous Anesthesia with Intravenous Lidocaine for Craniotomy: An Illustrative Two-Case Series.

PURPOSE: Intravenous lidocaine is used as an adjuvant to total intravenous anesthesia (TIVA). To our knowledge, density spectral array (DSA) patterns during intravenous lidocaine administration in propofol-remifentanil TIVA for craniotomy have rarely been described. We present two illustrative cases for educational and hypothesis-generating purposes. PATIENTS AND METHODS: Two adults underwent craniotomy for tumor resection (left cerebellopontine angle, Case 1; suprasellar, Case 2) with effect-site target-controlled infusion of propofol (Schnider model) and remifentanil (Minto model) and a bil

Anesthesia Management of Awake Craniotomy for Excision of a Supratentorial Hemorrhagic Brain Tumor Affecting Speech and Memory.

BACKGROUND Awake craniotomy is a neurosurgical approach used for resecting brain tumors in eloquent areas, where real-time neurological monitoring is critical to preserving function. Selecting the appropriate anesthesia approach is essential when preoperative language deficits or patient anxiety may limit intraoperative participation. While motor mapping is an established component of awake craniotomy, the use of a speech-language pathologist for real-time intraoperative monitoring remains underutilized. CASE REPORT A 31-year-old woman presented with a 1-year history of seizures, headaches, an

A case of bronchiectasis needing lung isolation for cerebello pontine angle tumor excision: Anesthetic challenges.

The main goals of neuroanesthesia are the maintenance of adequate cerebral perfusion pressure, avoidance of hypercarbia, hypoxemia, and to provide better brain relaxation. Providing anesthesia for a patient with bronchiectasis needing lung isolation for craniotomy can be challenging. A 56-year-old male patient, case of right lung bronchiectasis with a right cerebello pontine angle tumor underwent excision in the left lateral position. Since he had severe bronchiectasis of the right lung, we had isolated the right lung using right-sided double lumen tube to avoid spillage. Intraoperative split

A Tribute to Dr Robert J. White.

Robert J. White is probably best known as the first neurosurgeon to perform successful "cephalic exchange" on monkeys in 1971. However, he was also a pioneer in the field of neurosurgery and contributed tremendously to the field of neuroanesthesia and bioethics. White received medical training at the University of Minnesota, Harvard University, Peter Bent Brigham Hospital, and Mayo Clinic before becoming the first Chief of Neurosurgery at Metrohealth Hospital in Cleveland, Ohio. He made significant strides in the field of spinal cord cooling and hypothermia. White and his team was also the fir

Surgical experience with frontolateral keyhole craniotomy through a superciliary skin incision.

OBJECTIVE: The purpose of this study was to evaluate the results of 173 frontolateral keyhole minicraniotomies performed on 155 patients with aneurysms of the anterior or posterior cerebral circulation and for supratentorial tumors. METHODS: The frontolateral keyhole craniotomy is a modification of the generally used pterional approach. Of the 155 patients studied, 102 harbored saccular arterial aneurysms in the vessels of the anterior or posterior cerebral circulation, and 53 had various tumors in the frontal base, suprasellar, or parasellar region. The operations were carried out through an

Comparative investigation of the incidence of intraoperative seizures with propofol and remimazolam during anesthetic management for awake craniotomy: Retrospective propensity score matching study.

Seizures are serious complications of awake craniotomy (AC), and generalized clonic seizures are problematic because they require pharmacologic termination and emergency airway management. Although associations between seizure onset and anesthetic agents such as propofol and dexmedetomidine have been reported, data on remimazolam are limited. This study aimed to investigate the characteristics of seizures in patients undergoing AC and to compare the frequency of intraoperative seizures associated with propofol and remimazolam administration for anesthetic management. The data of 328 patients w

Dexmedetomidine versus propofol for sedation in awake neurosurgery: respiratory and hemodynamic safety.

PURPOSE: Monitored anesthesia care (MAC) with continuous sedation for awake craniotomy allows rapid recovery but increases risks of hypoventilation and hypoxemia. In April 2019, our center replaced propofol-based sedation with dexmedetomidine-based protocols to improve respiratory safety. We evaluated the impact of this protocol change on respiratory and hemodynamic outcomes. METHODS: This single-center retrospective before-after observational study analyzed all adult patients undergoing awake craniotomy between April 2017 and July 2021. The propofol group (April 2017-March 2019) received pro

Comparison of sodium thiopental and methohexital for high-dose barbiturate anesthesia.

High-dose sodium thiopental is frequently used in neuroanesthesia. The authors performed a study to compare a shorter-acting barbiturate, methohexital, to sodium thiopental in producing high-dose barbiturate anesthesia. In two groups of five mongrel dogs each, regional cerebral blood flow (CBF) was determined using the radioactive-microsphere technique, and cardiovascular parameters were measured before, during, and 1 hour after a 1 1/2-hour period of deep barbiturate anesthesia with either sodium thiopental or methohexital. Doses of the barbiturates were adjusted to produce electroencephalogr

Anesthesia for minimally invasive neurosurgery.

PURPOSE OF REVIEW: With an ultimate aim of improving patients overall outcome and satisfaction, minimally invasive surgical approach is becoming more of a norm. The related anesthetic evidence has not expanded at the same rate as surgical and technological advancement. This article reviews the recent evidence on anesthesia and perioperative concerns for patients undergoing minimally invasive neurosurgery. RECENT FINDINGS: Minimally invasive cranial and spinal surgeries have been made possible only by vast technological development. Points of surgical interest can be precisely located with the

[Intracranial carotid artery bifurcation aneurysms].

INTRODUCTION: Intracranial carotid artery bifurcation aneurysms are infrequent but its clinical behavior, high risk of bleeding and complex anatomic relationships of the sac permit to consider these lesions as a challenge cases. PATIENTS AND METHODS: 497 patients harboring intracranial aneurysms were operated on at Manuel Ascunce Domenech Hospital, Camagüey, Cuba between January 1982 to august 2001. We utilized microsurgical procedures, optical magnification, specialized neuroanesthesia and Intensive Care Unit postoperatory following. All patients were evaluated clinically with World Federati

Anesthetic management for parturients with neurological disorders.

Anesthesia care for the pregnant and the parturient presenting with a neurological disease requires (1) expertise with neuroanesthesia and obstetric anesthesia care, (2) accurate physical examination of the neurological system preoperatively, (3) safe choice and conductance of the anesthesia technique (mostly regional anesthesia), (4) avoidance of unfavorable drug effects for the fetus and the nervous system of the mother, and (5) intraoperative neuromonitoring together with the control of the fetal heart rate. The most important message is that in the ideal case, any woman with a known, preex

A Journey Through 100 Years of Vestibular Schwannoma Surgery at Mayo Clinic: A Historical Illustrative Case Series.

: The present state of vestibular schwannoma (VS) management is the product of over a century of technical progress by revolutionary surgeons who transformed a once perilous operation. At the beginning of the 1900s, patients who did not succumb to their disease were treated exclusively with surgery, which itself was almost assuredly devastating. Through the pioneering work of surgeons such as Harvey Cushing, Walter Dandy, William House, and others, safer surgical approaches were established with concurrent advances in neuromonitoring, neuroanesthesia, radiology, and adoption of the operating m

Implications of cranial pinning during awake craniotomy on anesthetic requirements: A retrospective cohort study.

BACKGROUND: Awake craniotomy (AC) is the gold standard for tumor resections in eloquent brain regions requiring surgical precision. Traditional AC uses pins to immobilize the head, which may contribute to scalp injury, discomfort, and hemodynamic fluctuations. We evaluated perioperative outcomes of AC performed with and without pin fixation at a single tertiary center. METHODS: We conducted a retrospective cohort study of adults undergoing AC between October 2018 and June 2023. Outcomes included head movement and movement-related workflow disruptions, anesthetic dosing, hemodynamics, operativ

Recent experience in the management of meningeal hemangiopericytomas.

Although the histogenesis of meningeal hemangiopericytomas (HMP) remains controversial, both biological and clinical evidence seems to identify these neoplasms as a separate entity with respect to meningiomas. In order to assess the current prognosis of HMP we reviewed our personal experience limited to the last decade (1986-1995): during this period 7 patients (4M, 3F) were treated by surgery alone or surgery combined with postoperative radiotherapy. In spite of meticulous attempts at radical resection, the tumors recurred in all but two cases with a mean interval of 85 months, and a total of

Neurosurgery in the sitting position: retrospective analysis of 692 adult and pediatric cases.

AIM: The sitting position is routinely used in many centers, although its use remains controversial and appears to be diminishing because of the risk of venous air embolism (VAE). MATERIAL AND METHODS: This is a retrospective analysis of 601 adult and 91 pediatric cases underwent neurosurgery from January 1995 through December 2010 in the sitting position. The incidence of VAE and other complications related to the sitting position has been determined. VAE was defined as a sudden and sustained decrease of end-tidal carbon dioxide (ETCO2) ≥0.7 kPa. RESULTS: The incidence of VAE in children an

[Neurological diseases in pregnancy: implications for anesthesia care--Part 1].

In the parturient as well as in the pregnant patient with neurological disease, surgery is necessary more frequently than in healthy pregnants. Most pregnancies of these patients will result in a slightly increased rate for cesarean section. The focus of anesthesia care is mostly to avoid damage to the fetus, in some pathologies to protect the mother. Pregnancy itself may change the course of pre-existent chronic neurological diseases such as epileptic seizure, multiple sclerosis, or myasthenia gravis. Other diseases will have their onset predominantly in pregnancy such as back pain, nerve com

Comparison of Subjective Patient Experiences Between Asleep-Awake-Asleep and Monitored Anesthesia Care Techniques During Awake Craniotomy.

BACKGROUND: Awake craniotomy (AC) with functional brain mapping is the standard approach for the resection of brain tumors in eloquent areas. Two main anesthetic techniques are commonly used: "Asleep-Awake-Asleep" (SAS) and "Monitored Anesthesia Care" (MAC). SAS involves general anesthesia during nonawake phases, while MAC utilizes conscious sedation, allowing for spontaneous ventilation without invasive airway devices. Both are effective and safe, but information on patient-reported intraoperative experiences is lacking. Here, we compare the subjective experience, postoperative quality of lif
