The objectives of this study were to assess the effect that acoustic neuroma surgery has on tinnitus and to investigate possible predictors (tumour size and patients' ages at operation) as well as to ascertain if the overall quality of life in patients with acoustic neuromas is affected by their tinnitus. A questionnaire was sent to randomly selected patients post acoustic-neuroma surgery. This was based on the Glasgow Benefit Inventory and contained a standardised series of four functional gradings for tinnitus. In this study, 51 patients from a total of 68 returned the questionnaire, and the

This article describes surgical methods developed during my resection of 600 acoustic neuromas, 599 of which were total removals. This series began in 1965, with the introduction of the surgical microscope and the development of microsurgical technique. In the 1960s, there was no established technology for the microsurgery of acoustic neuromas. Much of the work of the great early neurosurgeons--Cushing, Dandy, Olivecrona, and other remarkable pioneers--did not really apply. New approaches, solutions, and tools were introduced at that time, and there was a long, steep learning curve. I hope tha

This paper discusses surgery and radiosurgery from the author's personal experience with both techniques. Multi-discipline assessment will allow the patient to make an informed decision regarding preferred management.

Three cases of unilateral acoustic neuroma in childhood that are associated with neither neurofibromatosis type 1 nor type 2 were reported. All three cases had a hearing disorder as an initial symptom. Two of them had a large neuroma and had considerable abnormal findings in neurootological examinations, and one case with an intracanalicular tumor showed a unilateral progressive sensorineural hearing loss that had no response to steroid administration. Surgical removal of the tumor was carried out for these cases. Different approaches were used in each case; suboccipital approach, one-stage su

This article focuses on the incidence, pathophysiology, and management of pain removal of acoustic neuroma. The debate regarding the choice of surgical approach and frequency of headaches is reviewed. The proposed causes of postoperative headaches are discussed. Specific surgical strategies to minimize the risk for developing prolonged postoperative headaches are presented, and medical management options are summarized.

Recently, it has become obvious that disabling postoperative headache is a major problem with acoustic neuroma surgery. A questionnaire was used to retrospectively evaluate the incidence, clinical features, prognosis and possible therapeutic measures of this particular form of headache. Forty-two percent (42%) of patients had some headache prior to surgery but this was not a major complaint. After surgery, 75% of patients experienced headache. Only 24% had complete relief of headache. A very gradual improvement of the pain occurred in 32%. Pathogenesis remains unclear, but clinical characteris

72 cases (74 tumors) of acoustic neuroma pathologically confirmed after operation were analysed with special regard to the misdiagnosis made in their initial seeking for medical advice from ENT doctors. The average period of misdiagnosis was 5.48 years. The disease entities that the acoustic neuroma had been mistaken for were sensorineural hearing loss of unknown origin, idiopathic sudden deafness, tinnitus of unknown origin, cervical osteophytosis, nasopharyngeal carcinoma, etc. Emphasis was placed on the discussion of various causes of misdiagnosis.

Because acoustic neuromas most frequently arise in the posteriorly placed vestibular nerves, they usually displace the facial and cochlear nerves anteriorly (Figs. 11, 12, and 13). The facial nerve is stretched around the anterior half of the tumor capsule. Variability in the direction of growth of the tumor arising from the vestibular nerves may result in the facial nerve being displaced, not only directly anteriorly, but also anterior-superiorly or anterior-inferiorly. The nerve is infrequently found on the posterior surface of the tumor. Because the facial nerve always enters the facial can

There has been a continuous lowering of mortality and morbidity rates in the 20th century, thanks to increasing experience and newer technology. Despite this, the readership should be aware of the hazards involved in this challenging field. Complications occur, and their incidence, mechanism, management, and prevention have been outlined here so physicians performing these procedures will be better prepared to manage them. We have found that the occurrence of complications can be greatly reduced through a team approach using the combined talents of a neurotologist, neurosurgeon, and frequently

von Recklinghausen's disease, neurofibromatosis, is a neurocutaneous, systemic disease characterized by central nervous system tumors and disorders, café-au-lait spots, generalized cutaneous neurofibromas, skeletal deformities, and somatic and endocrine disorders. It is an autosomal dominant, hereditary disorder occurring in approximately 1 in 2500 births with a 50% affliction rate among family members. This patient had both systemic and ocular manifestations of the disease. She had neurofibromas of the face, neck, and iris, café-au-lait spots on neck and arms, short stature, hypothyroidism, a

The findings in a series of 80 cerebellopontine angle tumors are presented. Complications and results are covered in detail. A new method of controlling postoperative cerebrospinal fluid (CSF) leak is presented. Care of the eye in postoperative facial paralysis is emphasized, and a new type of moisture chamber is introduced.

Acoustic neuroma is one of the most common tumors of the posterior cranial fossa. Its removal is always a challenge for the neurosurgeon and the patient. The history of surgery for acoustic neuromas is inextricably linked with the history of neurosurgery in general. The modern surgical community must know history and be able to use it. Only then will the development of surgery lead to the preservation of the quality of life of patients. In the history of surgery for acoustic neuromas, the stages of its development are clearly visible from the description of the clinical picture through the stu
