Pro: The Use of Sugammadex Does Not Preclude the Need for Objective Neuromuscular Monitoring.

Based on strong evidence, optimal management of neuromuscular block should incorporate objective (or quantitative) neuromuscular monitoring with appropriately dosed antagonists such as sugammadex. Sugammadex has the unique ability to antagonize aminosteroidal-induced neuromuscular block at any level; however, neostigmine is a reasonable alternative at minimal levels. Blind or excessive administration of sugammadex does not eliminate the risk of patients having postoperative residual neuromuscular block. Significant variability exists in how patients respond both to neuromuscular blocking agent

A Survey of the Society for Pediatric Anesthesia on the Use, Monitoring, and Antagonism of Neuromuscular Blockade.

BACKGROUND: Although the package insert clearly states that "the safety and efficacy of sugammadex in pediatric patients have not been established," we hypothesized that sugammadex is used widely in pediatric anesthetic practice supplanting neostigmine as the primary drug for antagonizing neuromuscular blockade (NMB). Additionally, we sought to identify the determinants by which pediatric anesthesiologists choose reversal agents and if and how they assess NMB in their practice. Finally, because of sugammadex's effects on hormonal contraception, we sought to determine whether pediatric anesthes

Mammalian neuromuscular junctions: modern tools to monitor synaptic form and function.

Studies of neuromuscular synaptic structure and function have, historically, given general insights into synaptic mechanisms, whose principles were then extended to most other chemical synapses in diverse nervous systems. Often, these insights have been acquired through rigorous application of novel tools. Here, two genetic approaches to the visualisation of neuromuscular form and physiology are highlighted: first, mice expressing spectral variants of green fluorescent protein in neurones, axons and motor nerve terminals, including the 'brainbow' mouse transgenic lines. Second, the expression

Function and fatigue monitoring in neuromuscular electrically‑evoked exercise after spinal cord injury.

INTRODUCTION: Functional electrical stimulation (FES) can restore or augment movement in individuals with spinal cord injury (SCI), yet rapid neuromuscular fatigue limits training duration and the feasibility of closed‑loop control. AREAS COVERED: We review FES control‑system components and sensory feedback modalities used to monitor function and fatigue in SCI, synthesizing evidence from kinetic and kinematic sensors, namely joint angle, torque, center of pressure, and muscle‑level sensors such as surface/evoked electromyography, mechanomyography, ultrasound, and electroencephalography. Quantitative Neuromuscular Monitoring in General Anesthesia: A Systematic Review of Recovery and Safety Outcomes.

Quantitative neuromuscular monitoring is increasingly used during general anesthesia to confirm recovery after neuromuscular blocking agents (NMBAs) and reduce postoperative respiratory risk. Residual neuromuscular blockade remains a preventable concern, yet monitoring practices, device performance, reversal use, and recovery thresholds remain variable across perioperative settings. This systematic review evaluated evidence on quantitative neuromuscular monitoring in general anesthesia, focusing on recovery assessment, residual blockade detection, device agreement, reversal practice, and posto

The Impact of Quantitative Neuromuscular Monitoring and Sugammadex Reversal on Postoperative Hypoxemia in Obese Patients.

BACKGROUND: Residual neuromuscular blockade is a potential contributor to postoperative pulmonary complications, particularly in obese patients undergoing abdominal surgery. Continued reliance on qualitative neuromuscular monitoring and underutilization of sugammadex highlight a critical gap between evidence and practice. The low incidence of severe complications necessitates large studies to assess the impact of practice changes on clinical outcomes. The incidence and magnitude of postoperative hypoxemia may serve as a marker for other, more severe adverse outcomes. This before/after study ev

Ipsilateral and Simultaneous Comparison of Responses from Acceleromyography- and Electromyography-based Neuromuscular Monitors.

BACKGROUND: The paucity of easy-to-use, reliable objective neuromuscular monitors is an obstacle to universal adoption of routine neuromuscular monitoring. Electromyography (EMG) has been proposed as the optimal neuromuscular monitoring technology since it addresses several acceleromyography limitations. This clinical study compared simultaneous neuromuscular responses recorded from induction of neuromuscular block until recovery using the acceleromyography-based TOF-Watch SX and EMG-based TetraGraph. METHODS: Fifty consenting patients participated. The acceleromyography and EMG devices analy

Pain, agitation, delirium, and neuromuscular blockade: a review of basic pharmacology, assessment, and monitoring.

Agitation, pain, and delirium are focal points for targeted pharmacologic therapy in the intensive care unit. Understanding how to treat these essential entities necessitates fundamental understanding of the pharmacology of sedation, analgesia, and antipsychotics. Monitoring the effectiveness of these medications is crucial to optimize therapeutic outcomes and minimize untoward effects. Agents used in the management of agitation include drugs that target γ-aminobutyric acid A such as benzodiazepines and propofol and those with other targets such as dexmedetomidine and ketamine.
