Relevance of the habenular complex to neuropsychiatry: a review and hypothesis.

Since the initial observation by Brown (1914) that electrical stimulation applied to the habenular efferent bundle in the chimpanzee evoked a pattern of respiration which closely resembled the act of laughter, the habenular complex has remained a mysterious structure. The anatomy of the habenular complex is well delineated (Jones, 1985) forming a major component of the dorsal diencephalic conduction system. Data derived mainly from animal experimentation over the past decade point to the fact that the habenular complex functions as an important link between the limbic forebrain and the midbrai

Brain Stimulation Therapeutic Interventions in Neuropsychiatry: A Cambrian Explosion.

Brain stimulation interventions alter neural activity by applying electrical, magnetic, sonic, or other forms of energy to the nervous system, using invasive or noninvasive methods. In recent years, the basic science and clinical science of neuromodulation have markedly advanced, and the evidence for clinical utility has resulted in new U.S. Food and Drug Administration (FDA) clearance for multiple forms of brain stimulation and multiple indications in neuropsychiatry. This review summarizes the status of the major brain stimulation interventions for neuropsychiatric disorders, emphasizing key

Amalgamating artificial intelligence & neuropsychiatry: Multimodal approach for precision diagnosis.

Human body is an intricate system of complex associations between numerous chemicals, neurotransmitters incessantly striving towards homeostasis. This process is so tightly regulated that these processes quickly reverse the alterations caused due to any insults suffered by the organism; when these repeatedly attack the organism's system, they culminate as diseases. Brain being the hub of such complex interplay, is the most vulnerable site for such imbalances. As per 2021 World Health Organisation Report, over 3 billion of the global population live with a neuropsychiatric ailment. Owing to the

Neuropsychiatry of Huntington's disease and other basal ganglia disorders.

Degenerative diseases of the basal ganglia, such as Huntington's disease (HD), Parkinson's disease, and Wilson's disease, are characterized by motor, cognitive, and psychiatric manifestations. HD, in particular, can be considered a paradigmatic neuropsychiatric disorder that has all three components of the "Triadic Syndromes": dyskinesia, dementia, and depression. The authors examine the phenomenology, prevalence, and management of psychiatric disturbances occurring in diseases of the basal ganglia. They address psychiatric conditions such as depression, mania, psychosis, obsessive-compulsive

Diagnostic challenges revealed from a neuropsychiatry movement disorders clinic.

BACKGROUND: Abnormal movements are frequently associated with psychiatric disorders. Optimized management and diagnosis of these movements depends on correct labeling. However, there is evidence of reduced accuracy in the labeling of these movements, which could result in sub-optimal care. OBJECTIVE: To determine the consensus inter-rater reliability between a movement disorders neurologist and physicians referring from the community for phenomenology and diagnoses of individuals with co-existing psychiatric conditions and movement disorders. METHOD: Charts of all consecutive patients seen i

Analgesic nitrous oxide in neuropsychiatry: past, present and future.

We highlight the distinction between analgesic and anesthetic concentrations of nitrous oxide, with special reference to the safety of analgesic nitrous oxide. We present evidence that the gas at analgesic concentrations is an opioid agonist. Its extremely low abuse potential, despite its opioid properties, is discussed with regard to its evanescent action and possible partial agonistic effects, making it the least addictive of all mind-altering addictive substances. The activities of analgesic nitrous oxide make it an almost ideal agent with which to investigate the functions of the opioid sy

Modulation of glycine transporters as a novel therapeutic strategy in neuropsychiatry.

Glycine plays a dual role in the central nervous system as a fast inhibitory neurotransmitter and a co-agonist at N-methyl-D-aspartate receptors (NMDARs). Its extracellular levels are tightly regulated by two glycine transporters (GlyTs): GlyT1, which modulates glycine near excitatory synapses to influence glutamatergic transmission, and GlyT2, which sustains presynaptic glycine for inhibitory signalling. Dysregulation of GlyT function has been linked to numerous neurological and psychiatric disorders, including schizophrenia, mood and anxiety disorders, neurodegeneration, epilepsy, stroke, ad

Mind and body: how the health of the body impacts on neuropsychiatry.

It has long been established in traditional forms of medicine and in anecdotal knowledge that the health of the body and the mind are inextricably linked. Strong and continually developing evidence now suggests a link between disorders which involve Hypothalamic-Pituitary-Adrenal axis (HPA) dysregulation and the risk of developing psychiatric disease. For instance, adverse or excessive responses to stressful experiences are built into the diagnostic criteria for several psychiatric disorders, including depression and anxiety disorders. Interestingly, peripheral disorders such as metabolic diso

The neuropsychiatry of Niemann-Pick type C disease in adulthood.

Psychotic symptoms occur in a variety of medical and neurological conditions. The authors describe three young men with a variant form of Niemann-Pick type C disease, a neurodegenerative disorder related to abnormal intracellular cholesterol metabolism, who presented with psychosis in early adulthood. Two patients were treated for schizophrenia for many years prior to a diagnosis of Niemann-Pick type C. The cases presented in this article illustrate the role of changes in both white and gray matter structures in psychosis, and, like the assessments of other neurodevelopmental disorders that pr

Outcomes of CPAP treatment in a sleep laboratory specialized in neuropsychiatry.

The rapidly increasing number of sleep laboratories implicates their specialization into various fields of sleep medicine. In our sleep laboratory that specializes in neuropsychiatry, patients with the symptoms typical for the obstructive sleep apnea/hypopnea syndrome (OSAHS) were routinely redirected to a local respiratory clinic. Some patients, however, admitted to our center for other reasons revealed OSAHS in nocturnal polysomnography. The purpose of this retrospective study was to assess the outcome of CPAP in treating the sleepiness in this group of patients. Our material consisted of 36

The Bionics Bus for Neurology and Neuropsychiatry: Concept Development and Validation.

Healthcare delivery in the United Kingdom is increasingly becoming a challenging issue where demand is regularly outstripping availability. This is particularly a challenge in neurology and neuropsychiatry, where delays in diagnosis and treatment are leading to worse health and social outcomes. The Darzi report, which focused on three key tenants, has been hailed as the future blueprint for National Health Service (NHS) sustainability and high-quality care delivery. These three tenants are moving from analogue to digital approaches, focusing on prevention and wellbeing, and supporting diagnosi

Aging and the neuropsychiatry of multiple sclerosis: a cross-sectional study.

Aging in multiple sclerosis (MS) affects clinical and radiological disease activity. Yet, evidence is equivocal about the effects of aging on the neuropsychiatric sequelae of MS, including anxiety, depression, fatigue, and cognitive dysfunction. This study aimed to clarify how the neuropsychiatric symptoms of MS vary across ages. A consecutive cohort of 1194 people with MS (pwMS) underwent neuropsychological testing using the Minimal Assessment of Cognitive Function in MS, the Hospital Anxiety and Depression Scale sub-scales for anxiety (HADS-A) and depression (HADS-D), Modified Fatigue Impact

Neuropsychiatry and Neuropsychopharmacology of Dementia: a Guide to Evaluation, Diagnosis, and Management.

In the present study, I provide an examination of the neuropsychiatric approach to patients with various types of dementia, including Alzheimer's disease, Parkinson's disease dementia, Lewy body dementia, vascular dementia, frontotemporal dementia, and more. With a focus on the intersection of psychiatry and neurology, this paper underscores the importance of comprehensive neuropsychiatric evaluation, rigorous diagnosis, and evidence-based management. The paper delineates the neuropsychiatric manifestations specific to each type of dementia and explores both non-pharmacological and pharmacolog

Psilocybin in neuropsychiatry: a review of its pharmacology, safety, and efficacy.

Psilocybin is a tryptamine alkaloid found in some mushrooms, especially those of the genus Psilocybe. Psilocybin has four metabolites including the pharmacologically active primary metabolite psilocin, which readily enters the systemic circulation. The psychoactive effects of psilocin are believed to arise due to the partial agonist effects at the 5HT2A receptor. Psilocin also binds to various other receptor subtypes although the actions of psilocin at other receptors are not fully explored. Psilocybin administered at doses sufficient to cause hallucinogenic experiences has been trialed for ad

Clinical characteristics of patients with epilepsy in a specialist neuropsychiatry service.

Neuropsychiatry services provide specialist input into the assessment and management of behavioral symptoms associated with a range of neurological conditions, including epilepsy. Despite the centrality of epilepsy to neuropsychiatry and the recent expansion of neuropsychiatry service provision, little is known about the clinical characteristics of patients with epilepsy who are routinely seen by a specialist neuropsychiatry service. This retrospective study filled this gap by retrospectively evaluating a naturalistic series of 60 consecutive patients with epilepsy referred to and assessed wit

Folate and homocysteine metabolisms and their roles in the biochemical basis of neuropsychiatry.

The term 'one-carbon metabolism' is commonly used to describe 3 separate metabolic processes: folate metabolism, the homocysteine remethylation cycle, and the transsulfuration pathway. Folate metabolism concerns the biochemical reactions in which endogenous and exogenous one-carbon units are transferred to tetrahydrofolates. The remethylation cycle is used for the synthesis of methionine from homocysteine with one-carbon units that come from folate. This methionine is thenfused for the synthesis of S-adenosyl methionine, which is a general donor of methyl groups for many biochemical reactions
