After the party's over: evaluation of a drinking and driving prevention program.

This study evaluated a neuroscience nursing community outreach program titled, "After the Party's Over," focused on teenage drinking and driving prevention. The program consisted of a 20-minute slide presentation with contemporary music and narration that depicted graphic pictures of actual patients who sustained severe head and spinal cord injuries (SCIs) as a result of motor vehicle accidents (MVAs). A convenience sample of 274 high school drivers' education students was obtained. A Likert-type scale that measured reported behaviors was administered immediately before and after the program a

New thoughts on reflex sympathetic dystrophy syndrome.

Chronic pain caused by a sympathetic nervous system aberration must be considered separately from chronic pain resulting from carcinoma, arthritis, neuropathies and other delineated pathology. Because the pathophysiology of these conditions and their associated pain can be explained, treatment modalities can be planned specific to the affected individual and targeted toward the altered physiology. If the pathophysiology is not clearly defined or understood, as is the case in reflex sympathetic dystrophy and other idiopathic pain conditions, then prescription and remediation become a challenge

Aging Individuals With Down Syndrome and Dementia as Teachers: Learnings from Staffin a Developmental Disability Program in Long-Term Care.

Older adults with Down syndrome (DS) and dementia are an emerging sub-population. With much longer life spans than decades ago, issues have arisen as to where these aging adults will live and how nurses and other staff in facilities can provide effective care to these individuals. The current article presents a research study that examined the learnings of nurses and staff members working within a western Canadian program for older adults with DS and dementia. These learnings include: the importance of learning from each other; importance of collaboration; how individuals with developmental di

The effect of oral care on intracranial pressure in critically ill adults.

A major goal in the care of patients with neurological problems is to prevent or minimize episodes of increased intracranial pressure (ICP). Elevations in ICP in response to nursing interventions have been acknowledged since the 1960s when ICP monitoring was first introduced in the clinical setting. Until recently, few studies have specifically examined the effect of oral care on ICP, and oral care and other hygiene measures were combined or not specified, prohibiting a direct interpretation of the influence of oral care alone on ICP. The purpose of this study was to describe the relationship

Reassessing Hourly Neurological Assessments: A Quality Improvement Initiative.

BACKGROUND: Frequent neurological assessments (neuro-checks) are critical for early detection of neurological deterioration in patients with acquired brain injuries. However, extended use of hourly neuro-checks may negatively impact patient outcomes by contributing to sleep deprivation, intensive care unit (ICU) delirium, and staff burden. PURPOSE: This quality improvement initiative aimed to evaluate the implementation of a nurse-driven protocol to reduce unnecessary hourly neuro-checks in stable Neurocritical Care Unit (NCCU) patients after 48 hours of admission. METHODS: This project involv

Hyperbaric oxygen therapy for rhinocerebral fungal infection.

Rhinocerebral mucormycosis is an acutely fatal fungal infection that usually arises in the ethmoid sinuses, spreads into the orbits and then into the cranial cavity. It is typically seen in diabetic or immunocompromised patients. Therapy includes aggressive surgical debridement, administration of high-dose amphotericin-B and control of underlying predisposing conditions. Adjunctive hyperbaric oxygen (HBO) is another treatment modality that appears to be promising; oxygen in sufficient concentrations is fungicidal and decreases acidosis thereby increasing tissue survival. It is important for th

Nursing Management of the Patient Undergoing Focused Ultrasound: A New Treatment Option for Essential Tremor.

Essential tremor (ET) is among the most common neurological movement disorders that causes postural or action tremors, with an estimated prevalence nationwide of less than 3% of the population. The incidence of ET increases with age but often affects younger adults and has a familial trait association. Depending on disease progression, ET can cause significant limitations for individuals, in many cases, significantly limiting their ability to perform activities of daily living and occupational responsibilities. Until recently, treatment of ET heavily relied on medication management and invasiv

Hyperdynamic therapy: the nurse's role in the treatment of cerebral vasospasm.

The onset of subtle diffuse ischemic neurological deficits often associated with cerebral vasospasm is a major cause of morbidity and mortality following aneurysmal subarachnoid hemorrhage. The exact etiology of cerebral vasospasm is unclear. Increasing intravascular volume, decreasing blood viscosity and inducing hypertension may help prevent or diminish neurological deficits from cerebral vasospasm by improving cerebral blood flow. An intensive multidisciplinary approach is necessary with the role of the neuroscience nurse being pivotal. An understanding of the subtle neurological changes su

Clinical Localization of Stroke.

Acute stroke assessment is classically supported by clinical localization whereby presenting disabilities are associated with key arterial territories in the brain. Clinical localization skills are rarely taught to nonneurologists; yet, these skills are essential to the provision of evidence-based nursing care of stroke, enabling rapid patient identification, diagnosis, and ultimately, the delivery of acute treatment. This article explores the process of clinical localization in relation to the physiology affected by stroke vascular insufficiency. Elements of the neurologic examination are des

Caring for Traumatic Brain Injury Patients: Australian Nursing Perspectives.

Each year, millions of people suffer traumatic brain injury (TBI). It is not inherent to any country or group of people. It occurs as a result of falls, combat situations, sports injury, schoolyard playgrounds, and car accidents. It does not discriminate with age or status. Cost implications for health care settings and individuals are substantial. Management requires prompt neurologic assessment by a highly specialized multidisciplinary team of neuroscience practitioners. It is important to understand normal brain anatomy and physiology to identify what is unusual or abnormal. Keen observatio

Nursing Management of Lumbar Drainage in Cryptococcal Meningitis: A Case Report.

Raised intracranial pressure is a hallmark of cryptococcal meningitis and is associated with increased mortality. Continuous drainage of lumbar cerebrospinal fluid is suggested to control intracranial pressure. The complications induced by this treatment have been described. However, nursing care associated with identification and management of complications is less well known. We encountered a patient with human immunodeficiency virus-negative cryptococcal meningitis who developed increasing cerebrospinal fluid pressure, hearing impairment, and limb weakness. The patient's symptoms improved s

The biobehavioral correlates of post-traumatic brain injury depression.

Traumatic brain injury (TBI), typically resulting from vehicle accidents or acts of violence, is a major public health concern. Survivors often face lifelong impairments affecting functional status, cognition, and mood. Depression, the most common mood disorder affecting TBI survivors, most likely is underdiagnosed and presents differently than primary depression or depression after other brain insults. Unfortunately, few studies have been reported that advance theoretical perspectives on post-TBI depression. Currently, there is some evidence that neurochemical and anatomical changes contribut

CNS tuberculosis: a review and illustration from an autopsy case.

An estimated one-third of the world's population (2 billion people) is infected with the tubercle bacilli (TB), which is estimated to cause 6% of all deaths worldwide. Despite there being a decline in the incidence of tuberculosis seen in Europe, there are still some countries in the rest of the world where the estimated number of new cases is very high. When a person presents with persistent fever with or without neurological symptoms, the diagnosis of TB cannot be excluded. We present a case report of a 26-year-old male patient, who died of CNS tuberculosis. Such case studies will help keep

The primary dystonias of childhood.

Dystonia refers to a specific clinical entity as well as movements occurring as a result of other syndromes. Dystonic movements are the most commonly misdiagnosed abnormal involuntary movements. Dystonia worsens under emotional or physical stress and with fatigue, lessens with rest, relaxation, hypnosis and sensory tricks, can have diurnal fluctuations and may disappear in sleep. Dystonia is often confused with chorea and myoclonus and considered to be voluntary, that is, psychogenic in origin, in part because of its unusual and varied clinical expression. The neuroscience nurse, cognizant of

Anatomic correlates of psychological events related to temporolimbic epilepsy.

A number of behavioral disturbances have been associated with epilepsy. This is particularly true of temporal lobe seizures. While many ictal behaviors such as automatisms and rhythmic movement of an extremity are recognized as seizure activity, many psychological and cognitive alterations are not appreciated as being related to epilepsy. An increased awareness of the range of possible behaviors associated with epilepsy will enhance the observations made by the neuroscience nurse and augment the information available for formulating a differential diagnosis and localizing the lesion. Upper cervical spine chordoma: a case study.

Cervical chordomas are rare, slow-growing, but locally aggressive tumors. Predominantly found in people 50-69 years old, a chordoma arises from remnants of the primitive notochord. It is most often found in the sacrococcygeal or skull base areas. However, it can be found throughout the spine. Because chordomas are slow growing, they may reach considerable size before the patient becomes symptomatic. Surgical resection with a wide margin is the only curative procedure. Usually, because of tumor location and infiltration, this is not possible. Although this type of tumor is generally considered

Coping Strategies of Neurology Nurses Experiencing Abuse From Patients and Families.

OBJECTIVE: The objectives of this study were to assess incidence of verbal and physical abuse of neurology nurses, identify their coping strategies, and explore relationships between demographic characteristics, incidence, and coping strategies. BACKGROUND: A 2011 U.S. Department of Justice survey found that average annual rates of nonfatal violent crime against nurses were greater than all occupations and identified nurses as having the highest percentage of workplace violence. METHODS: A descriptive, exploratory research design utilizing an online survey of 112 neurology nurses in the Unit

A pediatric experience with endoscopic third ventriculostomy for hydrocephalus.

Pediatric neuroscience nurses deal with many children with hydrocephalus. This paper will provide a review of hydrocephalus with a specific focus on third ventriculostomy. Endoscopic third ventriculostomy (ETV) is an accepted treatment option for patients with obstructive hydrocephalus. At the Alberta Children's Hospital in Calgary, Alberta, we have been performing ETV for 15 years. This experience has helped us better understand the complications and benefits of ETV. The author will provide data on the complications specific to ETV in pediatric patients at the Alberta Children's Hospital. A c
