Nicotine replacement therapy versus control for smoking cessation.

BACKGROUND: Nicotine replacement therapy (NRT) aims to temporarily replace much of the nicotine from cigarettes to reduce motivation to smoke and nicotine withdrawal symptoms, thus easing the transition from cigarette smoking to complete abstinence. OBJECTIVES: To determine the effectiveness and safety of nicotine replacement therapy (NRT), including gum, transdermal patch, intranasal spray and inhaled and oral preparations, for achieving long-term smoking cessation, compared to placebo or 'no NRT' interventions. SEARCH METHODS: We searched the Cochrane Tobacco Addiction Group trials registe

99mTc-HMPAO brain SPECT imaging in a case of repeated syncopal episodes associated with smoking.

BACKGROUND: We report here a rare case of repeated syncopal episodes associated with smoking and findings of 99mTc-hexamethylpropyleneamine oxime (HMPAO) brain single-photon emission CT (SPECT) imaging. CASE DESCRIPTION: A 77-year-old man had four syncopal episodes during a half-month period. All four occurred when he stood up and walked immediately after smoking a cigarette, and syncope did not occur after cessation of smoking. Although upright testing revealed orthostatic hypotension, the patient did not complain of fainting on standing alone. Compared with brain SPECT in the supine positio

Can we increase smokers' adherence to nicotine replacement therapy and does this help them quit?

OBJECTIVE: To examine the effects of five intervention components on smokers' adherence to combined nicotine patch and nicotine gum during a quit attempt and assess whether adherence is related to cessation. METHOD: Smokers interested in quitting (N = 513; 59% female; 87% White) received nicotine patch plus nicotine gum and participated in a 2x2x2x2x2 randomized factorial experiment (i.e., 32 treatment conditions) evaluating five intervention components: (1) medication adherence counseling versus none; (2) automated medication adherence calls versus none; (3) electronic medication monitoring

Assessing acute nicotine impact on choroidal thickness: a randomized, double-blinded study comparing smoking cessation aids, including nicotine gum and electronic cigarettes.

PURPOSE: To explore whether differences in choroidal thickness arise from nicotine consumption in healthy young individuals, specifically comparing the effects of nicotine gum to electronic cigarette (vaping), while maintaining a consistent 4 mg nicotine dosage. METHODS: In a randomized double-blinded prospective cross-sectional study, 20 healthy participants (mean age ± standard deviation: 23 ± 2.36 years) were randomly assigned to either the nicotine gum or vaping group. Choroidal thickness (ChT) measurements were conducted using optical coherence tomography (OCT) (Topcon 3D OCT-1 Maestro S

Exercise as an adjunct to nicotine gum in treating tobacco dependence among women.

This was the first randomized, controlled smoking cessation trial assessing the efficacy of an exercise intervention as an adjunct to nicotine gum therapy in comparison with both equal contact control and standard care control conditions. Sedentary female smokers aged 18-55 years were provided with nicotine gum treatment along with brief behavioral counseling and were randomized into one of these three behavioral adjunct conditions. In the "intent-to-treat" sample (N = 182), at end of treatment and at 1-year follow-up, there were clear, but nonsignificant, trends in univariate analyses in whic

Coffee and alcohol intake as predictors of smoking cessation and tobacco withdrawal.

In one study of 105 smokers who received physician advice plus placebo gum and in another study of 630 self-quitters, neither the presence or absence, nor amount of precessation alcohol, nor coffee intake, nor changes in alcohol or coffee intake postcessation, predicted relapse or most withdrawal symptoms. The one possible exception was that heavy caffeine and alcohol users reported a greater increase in hunger and craving postcessation; however, these effects were not consistent across measures, follow-ups, and studies. Our results are inconsistent with theories that caffeine intoxication fro

Cigarette abstinence impairs memory and metacognition despite administration of 2 mg nicotine gum.

The authors assessed the effects of cigarette abstinence (nonabstinent vs. minimum 8 hours abstinent) and nicotine gum (0 mg vs. 2 mg nicotine) on sustained attention, free recall, and metacognition using a within-subjects design. Moderate smokers (10 women and 22 men) received one training session followed by four test sessions on consecutive days. Nicotine gum improved sustained attention in both abstinent and nonabstinent states, but had no significant effect on predicted or actual recall levels. Cigarette abstinence significantly impaired free recall and reduced the magnitude of participan

Improving patient education about tobacco withdrawal and nicotine gum use by registered nurses in inpatient psychiatry: A feasibility study.

UNLABELLED: WHAT IS KNOWN ON THE SUBJECT?: Many psychiatric inpatients use tobacco, but most psychiatric hospital units prohibit tobacco use. Psychiatric nurses do not receive adequate education about how to teach patients to best manage tobacco withdrawal symptoms. WHAT DOES THIS PAPER ADD TO EXISTING KNOWLEDGE?: Psychiatric nurses who receive a brief educational intervention about tobacco withdrawal symptoms and best practices for using nicotine gum may be more prepared to teach patients about these topics. In turn, patients may use nicotine gum more often during their hospitalization, leadi

Modeling nicotine arterial-venous differences to predict arterial concentrations and input based on venous measurements: application to smokeless tobacco and nicotine gum.

Significant arterio-venous differences in nicotine concentrations have been observed during and after cigarette smoking, nicotine nasal spray, and intravenous nicotine administration. In this paper we describe a novel mathematical method for estimating arterial blood levels from venous blood level data. The model allows to quantify: (i) the influence of the microcirculation in the hands and forearm on the distribution of nicotine, and (ii) the influence of disregarding the venous to arterial circulation in the estimate of systemic inputs. We also (iii) propose a general method to predict arter

Beyond Tobacco: Bridging Gaps in Social History Records for Tobacco-Free Nicotine Pouch Consumers.

OBJECTIVE: Nicotine-containing products have historically been tobacco derivatives like cigarettes, cigars, and dip. Recently, tobacco-free nicotine (TFN) products have been marketed as a healthy alternative. TFN pouches are small, discreet, flavored pouches containing nicotine designed to be placed between the gum and lip. This product does not fit a conventional tobacco category, leading to inaccurate reporting. This study aims to investigate discrepancies in physician documentation of TFN pouches. METHODS: A retrospective chart review was conducted on TFN users. SETTING: Single Health Car

Signs and symptoms of tobacco withdrawal.

To test the validity, magnitude, and clinical significance of the signs and symptoms of tobacco withdrawal defined by DSM-III, both observed and reported signs and symptoms were measured in 50 smokers during two days of ad lib smoking and then during the first four days of abstinence. Observer and subject ratings of the DSM-III symptoms of craving for tobacco, irritability, anxiety, difficulty concentrating, and restlessness increased after cessation. In addition, bradycardia, impatience, somatic complaints, insomnia, increased hunger, and increased eating occurred after cessation. The frequen

Nicotine replacement therapy for long-term smoking cessation: a meta-analysis.

OBJECTIVE: To assess if the effect of a single treatment episode with nicotine replacement therapy (NRT) enhances smoking cessation over many years. DATA SOURCES: Meta-analysis of all randomised controlled trials of NRT with final follow-up more than one year after the start of treatment. Twelve eligible trials were identified, all placebo-controlled, having final follow-ups ranging from 2-8 years. All had earlier follow-ups at 12 months. They comprised 2408 active and 2384 placebo treatment participants. DATA SYNTHESIS: The odds ratio (OR) in favour of NRT at final follow-up was 1.99 (95% c

A program to help major league baseball players quit using spit tobacco.

There are few reports in the scientific literature that describe tested methods for helping people quit using spit (smokeless) tobacco. This paper reports data from a pilot study to determine the effectiveness of two dental-oriented interventions to promote cessation of ST use among major league baseball players. These preliminary findings suggest that interventions involving an oral examination and advice to quit, combined with behavioral counseling, may effectively decrease ST use among professional baseball players.

Nicotine potentiates the effects of haloperidol in animals and in patients with Tourette syndrome.

Nicotine was found to markedly potentiate haloperidol-induced hypokinesia in rats. Nicotine alone was without effect. Subsequently, concurrent administration of 2 mg nicotine gum to 10 Tourette syndrome patients being treated with haloperidol produced a substantial decrease in tics and improvement of concentration and attention span. Nicotine gum alone was without effect. While 80% of children showed improvement with nicotine gum, 70% completely discontinued the gum because of side-effects, primarily involving nausea and bitter taste. Nicotine may prove useful for treating other neuroleptic re

Integration of smoking cessation into standard treatment for patients receiving opioid agonist therapy who are smoking tobacco: protocol for a randomised controlled trial (ATLAS4LAR).

BACKGROUND: About 85% of patients receiving opioid agonist therapy (OAT) for opioid dependence are smoking tobacco. Although smoke-related pulmonary diseases are significant contributors to morbidity and mortality, few smoking cessation interventions are evaluated within this group, and few OAT patients are offered smoking cessation as an integrated part of their addiction treatment. This study protocol describes an integrated smoking cessation intervention aimed at patients receiving OAT and smoking tobacco. METHODS: This is a multicentre, randomised controlled clinical trial that will recru

[Tobacco abuse: background and countermeasures].

The extreme extent of world-wide tobacco consumption, mainly in the form of cigarette smoking, is a phenomenon of the 20th century and can be termed a pandemic. Partly due to the massive and uninhibited advertising of the tobacco companies smoking prevalence has dramatically increased until the mid-sixties. Since then thousands of reports on the damaging-effects of smoking have been published which has led to a change in attitude and to a decrease in smoking prevalence at least in industrialised countries. In Switzerland about 25% of the adult population smoke, but the percentage in the age gr
