Usability and stability of longitudinal at-home sleep evaluation using the Waveband electroencephalogram headband in an insomnia population.

STUDY OBJECTIVES: Sleep monitoring outside of clinics could enhance care for insomnia and other sleep disorders but requires home systems that are easily operable and provide consistent data quality over multiple nights. We assessed the Waveband for usability by participants and feasibility of obtaining multi-night sleep data in the home setting. METHODS: 15 subjects with insomnia wore the Waveband electroencephalogram headband and an FDA-cleared wearable home sleep testing device, WatchPAT ONE ("WP1") for three nights. Usability was assessed via the System Usability Scale (SUS). Feasibility

Differences in day and night shift clinical performance in anesthesiology.

OBJECTIVE: This study examined whether anesthesia residents (physicians in training) performed clinical duties in the operating room differently during the day versus at night. BACKGROUND: Fatigue from sleep deprivation and working through the night is common for physicians, particularly during residency training. METHODS: Using a repeated-measures design, we studied 13 pairs of day-night matched anesthesia cases. Dependent measures included task times, workload ratings, response to an alarm light latency task, and mood. RESULTS: Residents spent significantly less time on manual tasks and m

Increased risk of breast cancer associated with long-term shift work in Canada.

OBJECTIVES: Long-term night work has been suggested as a risk factor for breast cancer; however, additional studies with more comprehensive methods of exposure assessment to capture the diversity of shift patterns are needed. As well, few previous studies have considered the role of hormone receptor subtype. METHODS: Relationships between night shift work and breast cancer were examined among 1134 breast cancer cases and 1179 controls, frequency-matched by age in Vancouver, British Columbia, and Kingston, Ontario. Self-reported lifetime occupational histories were assessed for night shift wor

Radiologists Make More Errors Interpreting Off-Hours Body CT Studies during Overnight Assignments as Compared with Daytime Assignments.

Background There is increasing research attention on the impact of overnight work on radiologist performance. Prior studies on overnight imaging interpretive errors have focused on radiology residents, not on the relative performance of board-eligible or board-certified radiologists at night compared with during the day. Purpose To analyze the rate of clinically important interpretation errors on CT examinations of the abdomen, pelvis, or both ("body CT studies") committed by radiology fellows working off-hours based on day or night assignment. Materials and Methods Between July 2014 and June

Night ventilation by body respirators for patients in chronic respiratory failure due to late stage Duchenne muscular dystrophy.

A program of respiratory muscle sparing in 9 patients with late stage Duchenne muscular dystrophy is presented. Results indicate that regular efficient night-time support by body respirator provides a constant significant improvement in daytime gas exchange for periods averaging up to 2 years following the occurrence of moderately severe respiratory failure. Until now, the longest survival is more than 4 years at the age of 28. the average PaCO2 and PaO2 before night ventilation were 60.8mmHg and 59.3mmHg, respectively. The levels after the program was begun were 45.5 (PaCO2) and 74.6 (PaO2).

Shorter and longer durations of sleep are associated with an increased twelve-month prevalence of psychiatric and substance use disorders: Findings from a nationally representative survey of US adults (NESARC-III).

The lack of comprehensive data on the association between psychiatric and substance use disorders and habitual sleep duration represents a major health information gap. This study examines the 12-month prevalence of mental disorders stratified by duration of sleep. Data were drawn from face-to-face interviews conducted in the National Epidemiologic Survey on Alcohol and Related Conditions III, a nationally representative survey of US adults (N = 36,309). There were 1893 (5.26%) participants who reported <5h of sleep/night; 2434(6.76%) 5 h/night; 7621(21.17%) 6 h/night; 9620(26.72%) 7 h/night;

ACCESS-OSA: a randomized controlled trial for accurate, accessible, and cost-effective screening solutions for obstructive sleep apnoea: study protocol.

Obstructive sleep apnoea (OSA) is common and burdensome, yet current diagnostic pathways remain costly and often misclassify patients. Single-night polysomnography (PSG), the diagnostic gold standard, fails to capture night-to-night variability in severity, is difficult to access and costly. Emerging technologies enable multi-night in-home evaluation, but robust comparative evidence regarding accuracy and cost-effectiveness is lacking. This study will evaluate (a) the diagnostic accuracy of three multi-night in-home devices versus conventional single night PSG; (b) the patient and signal chara

Acute effects of night work and meals on blood glucose levels.

Employees working at night are at increased risk of diabetes. A possible mechanism is related to differences in glucose regulation at night. Laboratory simulated night work studies show regulation of blood glucose is impaired at night. Regular exposure to high glucose levels at night may explain the observed relationship between night work and diabetes. We performed a field study of 19 nonsmoking women from the health-care sector to investigate how night work and the composition of meals affect post-prandial blood glucose levels. Blood glucose levels were self-assessed by finger-prick blood sa

Long-term effectiveness of cognitive-behavioural self-help intervention for nightmares.

Nightmares are a prevalent disorder leading to daily impairments. Two cognitive-behavioural self-help interventions--imagery rehearsal and exposure--recently showed short-term efficacy compared to a waiting-list and a group that recorded their nightmares. This paper reports the long-term results of the imagery rehearsal (n=103) and exposure (n=95) interventions. Participants were assigned randomly to a condition after completing baseline measurements; they received a 6-week self-help intervention and completed questionnaires 4, 16 and 42 weeks after end of treatment. Initial effects on nightma

Factors contributing to missed injuries in patients with trauma: Polytrauma and night-time presentation at the trauma resuscitation unit.

INTRODUCTION: In trauma care, missed injuries (MIs), defined as injuries identified during clinical reassessment involving physical examination and additional diagnostic tests, such as radiological imaging, initial and secondary surveys (these surveys are conducted according to the Advanced Trauma Life Support [ATLS] guidelines), significantly impact patient outcomes, including morbidity, mortality, and length of hospital stay. This study aimed to identify factors contributing to MIs to establish strategies for improving MI detection and care outcomes in patients from a trauma centre. MATERIA

Trial Protocol: A Randomised, Controlled Cross-Over Trial Evaluating the Digital Sleep Revolution Diagnostic-Therapeutic Pathway for Obstructive Sleep Apnoea.

Obstructive sleep apnoea (OSA) is common, but current diagnostic pathways rely on single-night assessment and the apnoea-hypopnoea index. Both are limited for assessment of variability and linking only weakly to symptoms, comorbidity and treatment response. The current protocol addresses this gap by aiming to validate a diagnostic pathway integrating a digital platform, mobile application, multiple-night sleep testing, semiautomated analysis and a novel sleep health questionnaire, labelled as 'Sleep Revolution'. Study objectives are to compare staff time required for diagnosis between standard

Housing First: Unsuppressed Viral Load Among Women Living with HIV in San Francisco.

While poverty is an established barrier to achieving success at each step of the HIV care continuum, less is known about specific aspects of poverty and how they overlap with behavior in exceptionally low-income individuals who live in well-resourced areas. We considered unsuppressed viral load over 3 years among women living with HIV in San Francisco who used homeless shelters, low-income hotels and free meal programs. One-hundred twenty study participants were followed; 60% had > 1 unsuppressed viral load and 19% were unsuppressed at every visit. Across six-month intervals, the odds of unsup

[Systems analysis as a necessary preliminary to improving psychiatric services].

The systems analysis approach to the work of a regional psychiatric service is described. Standard rehabilitation programs for principal groups of patients have been developed. These programs allow one to choose an optimal structure of the rehabilitation complex based on real requirements of the population. The complex includes a dispensary division, a hospital, a home treatment service, daytime and night wards, and workshops for occupational therapy. The disjunction of purposes and functions has provided complete succession in the work of all those services. The purposes and functions of each
