BACKGROUND: Nipple-sparing mastectomies (NSMs) preserve the intact nipple, including nipple duct orifices. Retained orifices might remain patent and communicate with the underlying reconstruction. We report the incidence and outcomes of nipple discharge after NSM in pregnant and nonpregnant women.

Hyperkeratosis of the nipple and areola is a rare condition first described by Tauber in 1923. Less than 100 cases have been reported in the literature. BACKGROUND: Nipple inversion is a relatively common problem in adolescent and adult women; however, most present surgical treatments are prone to injure the lactiferous ducts and impair the breast feeding function. We report four cases of adenoma of the nipple, a rare benign epithelial benign tumor occurring under the nipple and areola. Nipple-sparing mastectomy is an option for patients fitting oncologic criteria and may improve cosmetic outcomes of breast reconstruction. When anatomical limitations exist, we propose the use of free nipple grafting, akin to reduction mammaplasty. This study is a retrospective review of patients having a nipple-sparing mastectomy and immediate reconstruction using free nipple grafting (N = 36 breasts). Average graft take was 93.6% with no nipples having complete graft loss. Four nipples (11%) lost all projection and 4 nipples experienced significant hypopigmentation requiring tattooing (11%).

Severe gynecomastia with excessive skin is difficult to treat by only periareolar excision or suction-assisted lipectomy or both. The nipple and surrounding area, like other areas of skin, are subject to irritation, inflammation, and infection. Twenty women with persistent sore nipples were first seen by a lactation consultant and later referred to a dermatologist. The dermatologist successfully treated 18 of the 20 mothers. A lactation consultant should be able to identify those women who will benefit from evaluation and treatment by a dermatologist.

Bloody nipple discharge is a complaint that worries both the patient and the physician. Unlike other types of nipple discharge, it signals intrinsic breast disease. A benign etiology is the rule rather than the exception, but this symptom warrants careful evaluation. If there is no palpable mass or mammographic abnormality, bloody discharge from a single duct in a nonpregnant young woman is usually due to a benign intraductal papilloma.

Hyperkeratosis of the nipple and areola is a rare condition. We report two cases of hyperkeratosis of the nipple and areola occurring in men with no underlying endocrinopathy or synthetic estrogenic drug therapy. Both patients demonstrated prompt resolution of the hyperkeratosis of the nipples with a keratolytic gel. Because our cases were not associated with ichthyosis or epidermal nevus, they best fit into the category of nevoid hyperkeratosis of the nipples.

Usually hyperkeratosis of the nipple and areola is associated with ichthyosis and epidermal nevus. The patient's lesions will resemble acanthosis nigricans but histologically suggest a completely different entity. Presented is a case of hyperkeratosis of the nipples and areolae with no association with ichthyosis or epidermal nevus.
