Hyperbaric diuresis and nocturia--a review.

Human divers are now able to engage in a multi-day dive to a considerable depth using the mixed gas saturation diving technique, however their daily activities are somewhat limited in the depth. One of the major problems that has not been generally appreciated is the potential for body fluid disturbances at high pressure due to the development of a sustained diuresis. In addition, a marked nocturia has been observed in recent saturation dives at 26-31 atmospheres absolute (ATA), which disturbs divers' sleep during the night. In this review, we will briefly discuss the basic characteristics and

Nocturia: current status and future perspectives.

PURPOSE OF REVIEW: Nocturia is a bothersome and highly prevalent condition characterized by the need to wake to void at night. Nocturia is equally common in men and women, and although its prevalence increases with age, a significant proportion of younger people are also affected. Nocturia leads to fragmentation of sleep and consequently to a serious decline in daytime functioning and in quality of life and health. Its impact should not be underestimated by clinicians and therefore a review on nocturia is timely and relevant. RECENT FINDINGS: Traditionally, nocturia is regarded as a symptom o

Aging, circadian weight change, and nocturia.

BACKGROUND: Normal young individuals excrete more sodium in their urine during the day than overnight, but the reverse occurs in older individuals with nocturia. The reason is unknown. METHODS: First, a self-study was performed, determining the relation between morning and night weight change as an index of volume expansion. This was followed for 1 year and modified, in separate experiments, by assessing either a diuretic (furosemide 40 mg) or recumbency. Weight and in some instances ankle circumference were measured, and day and night urines collected. Second, a community study of 102 indivi

Managing Nocturia in Frail Older Adults.

This review discusses the available evidence in the current evaluation and treatment of nocturia in frail older adults. No evidence specifically evaluates the use of behavioral interventions in the treatment of the frail older adult with nocturia, but their use is supported in other cohorts. Behavioral modifications and optimal management of comorbidities remain the first-line treatment for all age groups and should be emphasized in the frail due to their favorable safety profile. No studies specific to the frail older adult support the use of pharmacotherapy. Some evidence exists for the effi

Nocturia: Evaluation and Current Management Strategies.

Nocturia is a complex condition that not only significantly affects quality of life, but may be an indicator of systemic disease. Initial assessment includes a detailed history and physical examination and completion of a frequency volume chart to help identify underlying causes, such as sleep disorders, bladder storage disorders, and polyuria. Treatment with alpha-blockers or antimuscarinic medications may help if the underlying cause is determined to be benign prostatic hyperplasia or overactive bladder, respectively. Treatment options for nocturnal polyuria have been limited to behavioral t

Nocturia causes vary with each decade.

PURPOSE OF REVIEW: To review the published literature within the last 18 months that comment on the causes of nocturia, varying with age. The causes are categorized into organized subsets: nocturnal polyuria, storage or reduced bladder capacity, 24-h polyuria, and sleep-associated nocturia. RECENT FINDINGS: Prevalence of nocturia is high, but has been reported at varying rates in the epidemiologic studies. Similarly, reported incidence has a high rate of variability. Nocturia causes were associated with nocturnal polyuria, bladder storage issues, metabolic syndrome, abnormal bowel habits, obe

[Nocturia in obstructive sleep apnoea (OSA)].

INTRODUCTION: Nocturia (two or more urinations per night) is a common symptom in OSA subjects. Higher secretion of atrial natriuretic peptide, increased intra-abdominal pressure, diuretics, diabetes, excessive fluid intake, awakenings are responsible for nocturnal urination. The aim of this study was to evaluate incidence of nocturia in moderate and severe OSA. MATERIAL AND METHODS: We studied 171 consecutive OSA patients (135 males and 36 females) - means: age - 53.6 +/- +/- 10.8 years, AHI/RDI - 43.6 +/- 23.2, BMI - 35.8 +/- 6.3 kg/m(2). RESULTS: To assess relations between nocturia and AH

[25 years old woman with nocturia].

A 25 years old lady presented to our outpatient clinic complaining nicturia, polyuria and polydipsia. On the basis of clinical assessment and the results of a simple test, a diagnosis of Nephrogenic Diabetes Insipidus (NDI) was made. The diagnosis of NDI is often missed, and appropriate treatment therefore delayed, with severe consequences especially in elderly and bed restricted people.

Nocturia in Men: Is the Prostate Really to Blame?

Nocturia is an extremely common accepted consequence of aging and generally viewed as a quality-of-life issue. However, nocturia can be an indicator of disease with significant health-related consequences including daytime functional disturbances, heart disease, diabetes, obesity, mood disturbances, reduced work productivity, increased falls, increased fractures, and increased mortality. In men, nocturia is frequently attributed to prostatic hyperplasia, but this is only one of many causes, requiring clinicians to have widespread knowledge to treat patients appropriately to improve their overa

Effects of fatty acid metabolites on nocturia.

Dysregulation of circadian rhythm can cause nocturia. Levels of fatty acid metabolites, such as palmitoylethanolamide (PEA), 9-hydroxy-10E,12Z-octadecadienoic acid (9-HODE), and 4-hydroxy-5E,7Z,10Z,13Z,16Z,19Z-docosahexaenoic acid (4-HDoHE), are higher in the serum of patients with nocturia; however, the reason remains unknown. Here, we investigated the circadian rhythm of fatty acid metabolites and their effect on voiding in mice. WT and Clock mutant (ClockΔ19/Δ19) mice, a model for nocturia with circadian rhythm disorder, were used. Levels of serum PEA, 9-HODE, and 4-HDoHEl were measured eve

The Importance of Terminology for Quantifying Nocturia in Practice.

BACKGROUND: Although nocturia, affecting mainly the elderly population, is a urinary tract symptom, it may also be a part of conditions not related to the urinary tract. In this scenario, nocturia terminology is crucial to avoid confusion and misunderstanding across different specialities. The International Continence Society (ICS) provides the updated nocturia terminology. OBJECTIVE: To review, describe, and explain the applicability of nocturia terminology based on the ICS standardisation documents. DESIGN, SETTING, AND PARTICIPANTS: The ICS report provides definitions on the terminology f

Fluid control in elderly patients with nocturia.

OBJECTIVES: To investigate the pathophysiology of nocturnal polyuria associated with aging. METHODS: Fifty patients (mean age 67.7 years, range 50-87) with nocturia were recruited for this prospective study. Patients were classified into nocturnal polyuria (NP) and non-nocturnal polyuria (non-NP) groups based on records of their frequency-volume charts. A hypertonic saline infusion test was carried out to evaluate individual osmotic and volume control. RESULTS: In the NP group, there was a significantly increased nocturnal diuretic rate compared with the daytime diuretic rate. In the non-NP

Nocturia: a new perspective on an old symptom.

The purpose of this review is to make urologists aware of the fact that nocturia among elderly men has a multifactorial aetiology and does not always depends on bladder outlet obstruction. After diagnosis of the underlying cause, specific treatments can be offered to the patient, one of which is transurethral resection of the prostate. Nocturia, defined as waking up at night to void, is a very common and bothersome symptom, affecting >50% of both men and women aged >60 years. Nocturnal polyuria is one reason for nocturia. Recent studies have shown that this condition can now be treated success

Are Nocturia and Falls Associated? A Secondary Analysis.

IMPORTANCE: Nocturia is believed to increase falls in older patients, a leading cause of injury-related death; however, the impact of other risk factors in patients with nocturia is limited. OBJECTIVES: To evaluate the association between nocturia and falls and injury. STUDY DESIGN: This was a secondary analysis of male and female participants from one site of the Lower Urinary Tract Dysfunction Network study. Participants with prior falls were excluded. Bayesian logistic and ordinal regression models were used to estimate associations between nocturia episodes and fall outcomes, adjusting f

Future Considerations in Nocturia and Nocturnal Polyuria.

Nocturnal polyuria (NP), the most common etiology of nocturia, can be caused by various medical conditions, including cardiovascular disease, obstructive sleep apnea, renal tubular dysfunction, as well as medications (eg, diuretics) and/or behavioral patterns. NP in the absence of underlying medical conditions has been described as NP syndrome and is thought be the result of impaired circadian release of endogenous arginine vasopressin. Desmopressin, a synthetic arginine vasopressin analog, has been shown to be an effective replacement therapy in adults with nocturia due to NP. Further studies

[Nocturia in men with benign prostatic hyperplasia].

Nocturia, defined as nocturnal micturition with a frequency of at least once per night, is one of the most frequent lower urinary tract symptoms in men with benign prostatic hyperplasia (BPH) and often causes them to consult a physician. Nocturia is often bothersome and responsible for increased morbidity and mortality. Nocturia can be caused by increased fluid intake, increased diuresis or decreased bladder capacity, either alone or in combination. The underlying pathophysiology of nocturia can only be detected by methodical evaluation of the patient. Bladder diaries for 3 days are an essenti

Nocturia: Current Evaluation and Treatment for Urology.

Nocturia is a very common yet a highly underreported entity. Recently, there has been an increasing interest in nocturia because of its association with serious adverse health conditions such as diabetes, heart disease, elevated BMI, and mortality. Frequency volume charts are the cornerstone of initial assessment and are pivotal in determining the type of nocturia and associated causes. The diagnostic evaluation is aimed at clarifying the underlying cause: sleep disturbances, reduced bladder capacity (functional or extrinsic), and increased urine production (nocturnal or daily). Management of nocturia: the role of antidiuretic pharmacotherapy.

Strategies to manage nocturia include lifestyle modifications and treatment with alpha-blockers, antimuscarinic therapies, and antidiuretics. The concept of achieving success should not be limited to reduction of nighttime voids; it should ideally include proof of improvement of conditions generally associated with nocturia, such as falls, quality of life, and overall health. Few studies have looked specifically at parameters other than nocturnal voids, such as sleep latency, first undisturbed sleep period (FUSP), and total sleep time, including their clinical relevance to patient well-being.
