Precise grading of non-muscle invasive bladder cancer with multi-scale pyramidal CNN.

The grading of non-muscle invasive bladder cancer (NMIBC) continues to face challenges due to subjective interpretations, which affect the assessment of its severity. To address this challenge, we are developing an innovative artificial intelligence (AI) system aimed at objectively grading NMIBC. This system uses a novel convolutional neural network (CNN) architecture called the multi-scale pyramidal pretrained CNN to analyze both local and global pathology markers extracted from digital pathology images. The proposed CNN structure takes as input three levels of patches, ranging from small pat

Current State of Bladder Preservation in High Grade Non-Muscle Invasive Bladder Cancer and Localized Muscle Invasive Bladder Cancer.

PURPOSE OF REVIEW: In recent years, new, effective bladder sparing techniques have emerged as favorable options for patients with BCG-unresponsive high-grade non-muscle-invasive bladder cancer (NMIBC) and localized muscle-invasive bladder cancer (MIBC), leading to a paradigm shift from the traditional radical cystectomy in clinical practice. Our aim is to examine the evolution of these techniques, summarize the current evidence, and shed light on the future of these treatment options. RECENT FINDINGS: Bladder preservation techniques offer a patient-centered approach while also demonstrating n

Long Noncoding RNAs Identify a Subgroup of Patients with Low-grade Non-muscle-invasive Bladder Cancer with Less Favorable Outcomes.

Low-grade (LG) stage Ta non-muscle-invasive bladder cancer (NMIBC) is typically nonaggressive with low progression rates and favorable cancer-specific survival. However, frequent monitoring is required because of the risk of recurrence. In this study, we analyzed RNA sequencing data for 286 LG Ta tumors from the UROMOL consortium. Unsupervised consensus clustering of long noncoding RNA profiles identified a subgroup of patients with significantly worse recurrence-free and progression-free survival. These patients may benefit from intensified intravesical therapy or closer surveillance. Further

Novel immunotherapeutic options for BCG-unresponsive high-risk non-muscle-invasive bladder cancer.

BACKGROUND: High-risk non-muscle-invasive bladder cancer (HR-NMIBC) presents a challenge to many physicians due to its ability to resist Bacillus Calmette-Guérin (BCG) intravesical therapy and the substantial rate of progression into muscle-invasive bladder cancer (MIBC). Patients who are BCG-unresponsive have worse prognosis and thus require further management including radical cystectomy (RC), which significantly impacts quality of life. Moreover, the ongoing worldwide shortage of BCG warrants the need for policies that prioritize drug use and utilize alternative treatment strategies. Hence,

Tissue polypeptide antigen as a non-invasive bladder Cancer biomarker.

Tissue Polypeptide Antigen (TPA), a non-invasive biomarker comprising fragments of cytokeratins 8, 18, and 19, has demonstrated potential for non-invasive surveillance of bladder cancer. However, challenges related to standardization impede its clinical implementation. This narrative review provides a focused overview of the relationship between TPA molecular biology and clinical laboratory practice, with special emphasis on analytical standardization, preanalytical harmonization, and evidence-based diagnostic algorithms. Urinary TPA has a sensitivity of 80-85% and specificity of 97% in optimi

Molecular and Genetic Insights for Personalized Treatment of Non-Muscle-Invasive Bladder Cancer.

Nonmuscle-invasive bladder cancer (NMIBC) is a clinically heterogeneous disease, characterized by a high recurrence rate and a progression risk that are challenging to predict. Although traditional risk models based on histopathology and clinical staging are widely used, their ability to inform personalized treatment remains limited. Recent advances in molecular profiling have provided a more comprehensive understanding of NMIBC biology. This article synthesizes current evidence regarding the molecular determinants, highlighting their roles in enhancing prognostication and informing personaliz

Network meta-analysis of immune checkpoint inhibitors in BCG-naïve, high-risk non-muscle-invasive bladder cancer.

PURPOSE: Bacillus Calmette-Guérin (BCG) remains the standard first-line therapy for BCG-naïve high-risk non-muscle-invasive bladder cancer (NMIBC), yet recurrence and progression remain clinically significant. Multiple phase III trials have evaluated immune checkpoint inhibitors (ICIs) combined with BCG, but the relative efficacy of individual agents and the influence of carcinoma in situ (CIS) remain unclear. We performed a network meta-analysis (NMA) to compare ICI-based regimens and assess treatment ranking, with CIS stratification. METHODS: A frequentist NMA was performed using phase III

Deep learning-based automated identification of papillary bladder cancer from endoscopic still images using SE-ResNeXt-50.

Bladder cancer remains a major global health challenge, necessitating rigorous endoscopic surveillance and the precise identification of neoplastic lesions. While the detection of overt tumors is routine, achieving high specificity and distinguishing borderline tumor structures are critical for optimal clinical decision-making. This is especially vital given the rapid evolution of intravesical therapies for BCG-unresponsive non-muscle invasive bladder cancer (NMIBC). Recent clinical advancements with novel agents-such as gemcitabine-releasing intravesical systems (TAR-200), nadofaragene firade

Event-free survival and complete response rate as surrogate endpoints for overall survival in high-risk non-muscle-invasive bladder cancer: A meta-analysis of bacillus Calmette-Guérin-naive or -remote cases.

BACKGROUND: Event-free survival (EFS) and complete response rate (CRR) are recommended as primary endpoints in bladder cancer trials to enable timely assessment of therapeutic benefit. However, their surrogacy relationship with overall survival (OS) in high-risk non-muscle-invasive bladder cancer (HR NMIBC) remains unclear. This meta-analysis evaluated EFS and CRR as surrogate endpoints for OS in bacillus Calmette-Guérin (BCG)-naive or -remote HR NMIBC. METHODS: A systematic literature review identified trials enrolling BCG-naive or -remote HR NMIBC patients and reporting EFS, CRR, and OS. We

BCG and Alternative Therapies to BCG Therapy for Non-Muscle-Invasive Bladder Cancer.

Bladder cancer is a heterogeneous disease. Treatment decisions are mostly decided based on disease stage (non-muscle invasive or muscle invasive). Patients with muscle-invasive disease will be offered a radical treatment combined with systemic therapy, while in those with non-muscle-invasive disease, an attempt to resect the tumor endoscopically will usually be followed by different intravesical instillations. The goal of intravesical therapy is to decrease the recurrence and/or progression of the tumor. In the current landscape of bladder cancer treatment, BCG is given intravesically to induc
