Noroviruses are a frequent cause of both acute gastroenteritis and outbreaks of gastroenteritis. Infection is usually self-limiting although it has been associated with mortality in children in the developing world and in vulnerable groups such as immunodeficient or immunosuppressed and elderly patients elsewhere. Diagnostic tests may be useful in preventing or limiting the spread and duration of outbreaks, and are needed to define norovirus-associated morbidity and mortality. However, the interpretation of test results should take account of the limitations of the different tests currently av

Infectious diarrhea is a common occurrence in the immunosuppressed population. We present a 43-year-old individual with large-volume stool output Norovirus acute gastroenteritis in the setting of relapsed refractory acute myelogenous leukemia, hematopoietic stem cell transplantation, and biopsy-proven cutaneous and pulmonary graft-versus-host disease. Therapeutic options such as intravenous immunoglobulin or reduction of immunosuppressants were not a feasible choice. A prompt clinical cure was achieved with nitazoxanide, a broad-spectrum antimicrobial agent. Nitazoxanide may be a safe therapeu

Noroviruses are the principal cause of epidemic gastroenteritis worldwide. Multiple reports have concluded that the major capsid proteins of GII.4 strains, which cause 80% of norovirus infections worldwide, are evolving rapidly, resulting in new epidemic strains. Surrogate neutralization assays using sera from outbreaks and from immunized mice suggest that, as with influenza virus, antigenic variation maintains GII.4 persistence in the face of human population herd immunity. To test this hypothesis, mice were hyperimmunized with virus-like particles (VLPs) representing an early (GII.4-1987) an

BACKGROUND: Traveler's diarrhea is the most common medical complaint of international visitors to developing regions. Previous findings suggested that noroviruses (NoVs) are an underappreciated cause of traveler's diarrhea. METHODS. In the present study, we sought to define the presence of NoVs in 320 acute diarrheic stool samples collected from 299 US students who traveled to Guadalajara, Cuernavaca, or Puerto Vallarta, Mexico, during the period from 2007 through 2008. Conventional and quantitative real-time polymerase chain reaction assays were used to detect and determine NoV loads in stool

In late 2006, CDC began receiving requests from numerous state public health departments for information about a perceived increase in the number of outbreaks of acute gastroenteritis (AGE), especially those involving person-to-person transmission in long-term--care facilities. No national surveillance system exists for AGE outbreaks, including those caused by norovirus, unless foodborne transmission is suspected. In the absence of national surveillance data, CDC attempted to better characterize the outbreaks of AGE by analyzing information from the following sources: 1) detailed data on recen

Noroviruses (Norwalk-like viruses) are recognized as major causes of acute gastroenteritis worldwide. Outbreaks of norovirus gastroenteritis are often associated with consumption of oysters. In this study, oysters imported into Hong Kong from 11 countries over a 3-year period were screened by RT-PCR. Overall, 53 out of 507 (10.5%) samples were positive for norovirus-RNA, and a wide variety of strains were found. Two novel genetic clusters were detected, which could indicate novel human or animal norovirus strains. However, whether these two new clusters are of human or animal origin is not kno

Human noroviruses (HuNoVs) are a leading cause of acute gastroenteritis worldwide. HuNoV infections lead to substantial societal and economic burdens. There are currently no licensed vaccines or therapeutics for the prevention or treatment of HuNoVs. A lack of well-characterized in vitro and in vivo infection models has limited the development of HuNoV countermeasures. Experimental infection of human volunteers and the use of related viruses such as murine NoV have provided helpful insights into HuNoV biology and vaccine and therapeutic development. There remains a need for robust animal model

BACKGROUND: Despite accounting for approximately one fifth of all acute gastroenteritis illnesses, norovirus has received comparatively less attention than other infectious pathogens. With several candidate vaccines under development, characterizing the global economic burden of norovirus could help funders, policy makers, public health officials, and product developers determine how much attention and resources to allocate to advancing these technologies to prevent and control norovirus.
METHODS: We developed a computational simulation model to estimate the economic burden of norovirus in ev

Noroviruses cause epidemic and sporadic acute gastroenteritis in both children and adults. We report a rare case of intestinal necrosis due to norovirus gastroenteritis in a healthy adult. A 47-year-old man presented with worsening abdominal pain, diarrhea, vomiting, and abdominal fullness. Physical examination revealed abdominal distension and diffuse tenderness. Contrast-enhanced computed tomography revealed intestinal distention, pneumatosis, and portal venous gas, findings suggestive of intestinal necrosis. Norovirus genome was detected in his stools using the RT-PCR method. Upon laparotom

BACKGROUND: The US Centers for Disease Control and Prevention estimates 20.9 million norovirus infections annually in the United States. Although the acute disease burden is sizeable, emerging data suggest norovirus may be associated with chronic gastrointestinal problems. We identified known outbreaks in US military recruits and used the Defense Medical Encounter Database (DMED) to identify the risk of new onset functional gastrointestinal disorders (FGD) and gastroesophageal reflux disease (GERD).
METHODS: Subjects reporting for care of acute gastroenteritis (AGE) at a military treatment cl

Norovirus (NoV) is recognised as one of the emerging viruses causing infection in humans. It is the leading cause of outbreaks of viral gastro-enteritis worldwide. In children, NoV plays an increasing and important role in enteric infection, apart from rotavirus, especially in the post-rotavirus vaccine era. NoV-infected children usually present with typical clinical manifestations of acute viral gastro-enteritis, including vomiting and watery diarrhoea, and paediatric patients are more liable to have dehydration requiring hospitalisation. Other than these symptoms, severe or atypical complica

Norovirus (NoV), a member of the family Caliciviridae, is a major cause of acute water- and food-borne nonbacterial gastroenteritis and forms antigenically diverse groups of viruses. Human NoVs are divided into at least three genogroups, genogroups I (GI), GII, and GIV, which contain at least 15, 18, and 1 genotypes, respectively. Except for a few genotypes, all NoVs bind to histo-blood group antigens (HBGAs), namely ABH antigens and Lewis antigens, in which carbohydrate core structures, (types 1 and 2) constitute antigenically distinct phenotypes. Volunteer challenge studies have indicated th

Noroviruses have emerged as a major cause of acute gastroenteritis in humans of all ages. Despite high infectivity of the virus and lack of long-term immunity, volunteer and authentic studies has suggested the existence of inherited protective factors. Recent studies have shown that histo-blood group antigens (HBGAs) and in particular secretor status controlled by the alpha1,2fucosyltransferase FUT2 gene determine susceptibility to norovirus infections, with nonsecretors (FUT2-/-), representing 20% of Europeans, being highly resistant to symptomatic infections with major strains of norovirus.

Norovirus (NoV) genogroups I and II (GI and GII) are now recognized as the predominant worldwide cause of outbreaks of acute gastroenteritis in humans. Three recombinant NoV GII isolates were identified and characterized, 2 of which are unrelated to any previously published recombinant NoV. Using data from the current study, published sequences, database searches, and molecular techniques, we identified 23 recombinant NoV GII and 1 recombinant NoV GI isolates. Analysis of the genetic relationships among the recombinant NoV GII isolates identified 9 independent recombinant sequences; the other

Given limited data on norovirus vaccine acceptance, we performed an exploratory survey in a rural Guatemalan community on knowledge, interest, and willingness to pay (WTP) for a norovirus vaccine. Cluster-randomized households with children aged 6 weeks to 17 years were enrolled into one of two norovirus surveillance studies: 1) a prospective cohort (N = 207 households) and 2) two separate, community-based, cross-sectional surveys (N = 420 households). After completion of the surveillance study, vaccine surveys were completed by 564 (90%) of 627 households. Most households correctly answered q

Human norovirus (HuNoV) is the leading cause of acute nonbacterial gastroenteritis globally. Histo-blood group antigens (HBGAs) have been recognized as attachment factors for HuNoV. However, the receptors or other potential attachment factors for HuNoV have not been fully elucidated. A bacterial cell surface-displayed system was used to express GII.4 HuNoV protruding domain (GII.4 P) to capture attachment factors from a diarrheal sample of a patient infected with GII.4 HuNoV. A library of candidate HuNoV proteinaceous attachment factors was constructed. Through a database comparison, a literat

This study examined the association of norovirus with foodborne disease outbreaks linked to food handlers and foodborne outbreaks of acute gastroenteritis associated with norovirus, in comparison to Salmonella foodborne gastroenteritis. Comparative analysis using chi(2)-tests showed no similarity between the outbreaks from norovirus and those from Salmonella. Odds ratios also showed a stronger similarity between the proportions of food handler-implicated norovirus outbreaks than from Salmonella-implicated outbreaks. An analysis of data found norovirus transmission to occur more frequently spre

Norovirus (NoV) is an enteric non-enveloped virus which is the leading cause of gastroenteritis across all age groups. It is responsible for around 200,000 deaths annually and outbreaks are common in small communities such as educational and care facilities. 40% of all NoV outbreaks occur in long-term and acute-care facilities, forming the majority of outbreaks. Nosocomial settings set ideal environments for ease of transmission, especially due to the presence of immunocompromised groups. It is estimated to cost global economies around £48 billion a year, making it a global issue. NoV is trans

Noroviruses are genetically diverse RNA viruses associated with acute gastroenteritis in mammalian hosts. Phylogenetically, they can be segregated into different genogroups as well as P (polymerase)-groups and further into genotypes and P-types based on amino acid diversity of the complete VP1 gene and nucleotide diversity of the RNA-dependent RNA polymerase (RdRp) region of ORF1, respectively. In recent years, several new noroviruses have been reported that warrant an update of the existing classification scheme. Using previously described 2× standard deviation (sd) criteria to group sequence
