Self-Healing Dynamic Hydrogel as Injectable Shock-Absorbing Artificial Nucleus Pulposus.

The intervertebral discs (IVDs) provide unique flexibility to the spine and exceptional shock absorbing properties under impact. The inner core of the IVD, the nucleus pulposus (NP) is responsible for this adaptive behavior. Herein, we evaluate an injectable, self-healing dynamic hydrogel (DH) based on gold(I)-thiolate/disulfide (Au-S/SS) exchange as NP replacement in a spine motion segment model. For the first time, we report the application of dynamic covalent hydrogels inside biological tissues. The dynamic exchange between Au-S species and disulfide bonds (SS) resulted in self-healing abil

Natural history of spinal cord infarction caused by nucleus pulposus embolism.

Nucleus pulposus embolism causing spinal cord infarction is exceptional. A 16-year-old girl was seen with sudden onset of interscapular pain and paraplegia from fatal ischemic transverse myelopathy due to arterial and venous occlusions by fibrocartilaginous embolism. In 32 cases of nucleus pulposus embolism, females predominated (69%) and age distribution was bimodal with peaks at 22 and 60 years (median, 38.5). Embolization was either arterial and venous (50%) or purely arterial (50%). Myelopathy predominated in cervical (69%) and lumbosacral (22%) segments. Schmorl's nodes, larger volume and

Human Wharton's Jelly Cells Activate Degenerative Nucleus Pulposus Cells In Vitro.

To investigate the interaction between human Wharton's jelly cells (WJCs) and degenerative nucleus pulposus cells (NPCs), human WJCs were cocultured with degenerative NPCs with or without direct cell-cell contact. WJCs were isolated from the human umbilical cord and degenerative NPCs were isolated from the surgically obtained degenerative intervertebral disc tissue. The isolated WJCs positively expressed CD73, CD105, CD90, CD29, CD166, and human leukocyte antigen (HLA)-ABC, but negatively expressed CD34, CD45, and HLA-DR. After coculturing with three different WJCs:NPCs ratios for 7 days, the

Reduced nucleus pulposus glycosaminoglycan content alters intervertebral disc dynamic viscoelastic mechanics.

The intervertebral disc functions over a range of dynamic loading regimes including axial loads applied across a spectrum of frequencies at varying compressive loads. Biochemical changes occurring in early degeneration, including reduced nucleus pulposus glycosaminoglycan content, may alter disc mechanical behavior and thus may contribute to the progression of degeneration. The objective of this study was to determine disc dynamic viscoelastic properties under several equilibrium loads and loading frequencies, and further, to determine how reduced nucleus glycosaminoglycan content alters dynam

Nucleus pulposus-induced nerve root injury: effects of diclofenac and ketoprofen.

MAIN PROBLEM: Nucleus pulposus and/or chronic compression can induce spinal nerve root injury. Inflammation has been proposed as having major importance in the pathophysiologic mechanisms involved in the induction of such injuries. Corticosteroids, potent anti-inflammatory drugs, have been demonstrated to reduce nucleus pulposus-induced spinal nerve root injury. The aim of the present study was to assess the effects of two potent non-steroidal anti-inflammatory drugs (NSAIDs), diclofenac and ketoprofen, in experimental nucleus pulposus-induced spinal nerve root injury in a pig model. METHODS:

Tumor necrosis factor alpha and nucleus-pulposus-induced nerve root injury.

STUDY DESIGN: The effects of nucleus pulposus and various treatments to block tumor necrosis factor alpha activity were evaluated in an experimental set-up using immunohistochemistry and nerve conduction velocity recordings. OBJECTIVES: To assess the presence of tumor necrosis factor alpha in pig nucleus pulposus cells, and to see if block of tumor necrosis factor alpha also blocks the nucleus-pulposus-induced reduction of nerve root conduction velocity. SUMMARY AND BACKGROUND DATA: A meta-analysis of observed effects induced by nucleus pulposus revealed that these effects might relate to on

Ultrastructural changes in spinal nerve roots induced by autologous nucleus pulposus.

STUDY DESIGN: Ultrastructural changes were analyzed by transmission electron microscopy in nerve roots exposed to autologous nucleus pulposus experimentally. OBJECTIVES: To assess if ultrastructural changes were present in areas with no light microscopic changes in nerve roots exposed to autologous nucleus pulposus in a pig model. SUMMARY OF BACKGROUND DATA: Previous analyses have shown that there is focal nerve fiber damage in nerve roots exposed to autologous nucleus pulposus in the pig. These changes could not fully explain the reduction in nerve conduction velocity seen in the same nerve

[Acute paraplegia due to embolism from nucleus pulposus tissue (author's transl)].

Typical anterior spinal artery syndrome with initially severe pain and paraplegia with sensory disturbances occurred after a fall in a 36-year-old man with a history of lumbago. Multiple emboli of fibrocartilaginous material with nucleus pulposus cells were found in the foci of myelomalacia. The embolism of nucleus pulposus tissue originating from Schmorl's nodules was probably carried via the blood vessels of the neighbouring vertebral bodies with retrograde flow through the perispinal venous plexus into spinal cord veins and hence into the arteries of the spinal cord via veno-arterial anasto

Charge-engineered microparticles for targeted delivery of RhoA activator to nucleus pulposus.

Intervertebral disc degeneration (IVDD) involves dysregulated mechanobiology of nucleus pulposus (NP) cells, marked by disrupted F-actin organization and inflammation-driven extracellular matrix (ECM) catabolism. Restoring actomyosin contractility and cytoskeletal integrity can blunt ECM degradation. Employing a constitutive Rho activator, CN03, reinstates F-actin organization via the RhoA-ROCK pathway and rescues NP cells from cytokine-driven degeneration. However, its constitutive RhoA activity is short-lived, which demands sustained, spatially targeted NP delivery. In this study, an enginee

The role of nucleus pulposus progenitor cells in intervertebral disc degeneration and regeneration.

Intervertebral disc degeneration (IDD) is a significant public health issue, commonly associated with symptoms such as low back pain (LBP), and poses a substantial burden on both society and affected families. The pathogenesis of IDD primarily involves the depletion and impaired function of nucleus pulposus (NP) cells, as well as alterations in the extracellular matrix (ECM) composition. Current clinical management strategies, including conservative and surgical treatments, aim primarily to alleviate symptoms rather than reverse the pathological process or promote structural regeneration of th

Gait abnormalities and inflammatory cytokines in an autologous nucleus pulposus model of radiculopathy.

STUDY DESIGN: The authors investigated gait abnormalities and mechanical hypersensitivity associated with invertebral disc herniation in a rat model of radiculopathy. Further evaluation involved assessing how nucleus pulposus (NP) injury affected systemic cytokine expression and molecular changes at the dorsal root ganglion (DRG). OBJECTIVE: The objective of this work was to describe the gait and behavioral changes in an animal model of disc-herniation induced radiculopathy. A second objective included examining how these functional changes correlated with neuroinflammation and autoreactive l

The response of the nucleus pulposus of the lumbar intervertebral discs to functionally loaded positions.

STUDY DESIGN: Asymptomatic volunteers underwent magnetic resonance imaging to investigate how different positions affect lumbar intervertebral discs. OBJECTIVE: To quantify sagittal migration of the lumbar nucleus pulposus in 6 functional positions. SUMMARY OF BACKGROUND DATA: Previous studies of the intervertebral disc response in the sagittal plane were limited to imaging of recumbent positions. Developments of upright magnetic resonance imaging permit investigation of functional weight-bearing positions. METHODS: T2-weighted sagittal scans of the L1-L2 to L5-S1 discs were taken of 11 vol

TIE2-positive cells in the nucleus pulposus with a purpose: the who, what and why.

Once thought to be solely involved in vasculogenesis, tyrosine kinase with immunoglobulin-like and EGF-like domains 2 (TIE2) has emerged as a crucial marker of progenitor-like cells in the avascular nucleus pulposus (NP), a tissue with notoriously limited regenerative capacity. Recent evidence suggests that TIE2 + NP cells play a pivotal role in disc tissue homeostasis, influencing extracellular matrix maintenance, cellular renewal, and tissue integrity. However, despite the reported regenerative potential of TIE2 + NP cells, their precise function remains enigmatic. This review consolidates i

Preconditioning Mesenchymal Stem Cells to Enhance Performance in the Degenerate Nucleus Pulposus.

Degeneration of the lumbar intervertebral discs is a leading cause of low back pain, a condition that affects a substantial proportion of the global population. The earliest degenerative changes typically manifest in the central nucleus pulposus (NP) region of the disc and are characterized by elevated local inflammation and a cell-mediated reduction in proteoglycan content and transition to a fibrotic extracellular matrix. The application of adult mesenchymal stem cells (MSCs) to arrest and reverse degenerative changes in the NP has received significant interest due to the availability and su

Getting to the Core: Exploring the Embryonic Development from Notochord to Nucleus Pulposus.

The intervertebral disc (IVD) is the largest avascular organ of the human body and plays a fundamental role in providing the spine with its unique structural and biomechanical functions. The inner part of the IVD contains the nucleus pulposus (NP), a gel-like tissue characterized by a high content of type II collagen and proteoglycans, which is crucial for the disc's load-bearing and shock-absorbing properties. With aging and IVD degeneration (IDD), the NP gradually loses its physiological characteristics, leading to low back pain and additional sequelae. In contrast to surrounding spinal tiss
