Advancing Nurses' Role to Address Issues Facing Japanese Anesthesia Practice.

Japan is challenged with unique social problems because of its declining birthrate and rapidly aging population. By the year 2025, all of Japan's baby boomers will be 75 years or older, making Japan a "superaging" society. Japanese healthcare expenditures are rapidly climbing because of the need for increasingly complex medical-surgical treatments for this aging population. In addition, a major shortage of anesthesiologists has produced serious threats to patient safety, as well as to quality and timeliness of surgical care. In an attempt to meet the demand for anesthesia services and to ensur

The Metamorphosis of NAR to CRNA: The Caterpillar to Butterfly Analogy.

The journey from nurse anesthesiology resident (NAR) to certified registered nurse anesthetist (CRNA) is a profound transformation, one that extends beyond skill acquisition to the evolution of professional identity. Using the metaphor of a caterpillar's metamorphosis into a butterfly, this article explores how academic rigor, clinical immersion, and mentorship shape resilience, adaptability, and confidence in future CRNAs. Grounded in growth mindset theory, it highlights how embracing challenges, seeking feedback, and reframing setbacks as opportunities for learning build both competence and

Beyond the monitors: Anaesthesiologists' experiences of the process of extubation.

BACKGROUND: Although extubation is a high-risk phase associated with risk of severe complications for patients undergoing general anaesthesia, there is a lack of research about this phenomenon from the perspective of anaesthesiologists' experiences of the process of extubation in the anaesthesia setting. AIM: To describe Swedish anaesthesiologists' experiences of the extubation process in the anaesthesia setting. METHODS: A qualitative descriptive design study with individual semi-structured interviews was conducted in three hospitals in Sweden with a total of 17 anaesthesiologists. A qualit

The Other Side of the Difficult Airway: A Disciplined, Evidence-based Approach to Emergence and Extubation.

Successful intubation of the patient with a difficult airway marks the beginning of the challenges facing the nurse anesthetist charged with delivery of that patient's anesthesia care. Too often, on successful intubation of this patient, the anesthetist and other members of the perioperative team may relax too much. Substantial planning and consultations have been employed to achieve the successful intubation of the patient with the difficult airway. Yet frequently, the final aspect of any general anesthetic-extubation-does not receive sufficient planning even though there continues to be a la

States Should Remove Barriers to Advanced Practice Registered Nurse Prescriptive Authority to Increase Access to Treatment for Opioid Use Disorder.

In October 2018, President Trump signed into law H.R. 6, the Substance Use-Disorder Prevention that Promotes Opioid Recovery and Treatment for Patients and Communities Act. This piece of legislation addresses treatment, prevention, recovery, and enforcement with particular attention to access in rural areas. It contains numerous provisions to improve needed access to treat substance use disorders and especially opioid use disorder (OUD), including mandatory coverage of medications for OUD, partial elimination of Medicaid payment for inpatient mental health treatment, and state planning grants

Comparison of methohexital and propofol use in ambulatory procedures in oral and maxillofacial surgery.

PURPOSE: Short-acting anesthetic agents, such as propofol and methohexital, are commonly used for ambulatory procedures in the practices of oral and maxillofacial surgeons (OMS). This study compares the safety and anesthetic outcomes of propofol and methohexital. In addition, the study compares the safety and outcomes of these agents when administered either by an OMS who simultaneously provides anesthesia and performs the procedure (anesthetist/surgeon), or by a non-OMS provider of anesthesia (anesthesiologist or certified registered nurse anesthetist; CRNA) whose sole obligation is to provid

Establish a perioperative check forum for peripheral intravenous access to prevent the occurrence of phlebitis.

The prevalence of intravenous (IV) catheter-related infections is 0.5 per 1000 device days, and these infections cause tenderness, erythema, swelling and phlebitis. Catheter-related bloodstream infections (CRBSI) may independently increase hospital costs and length of stay; the aim of the study was to set up a standard operating procedure (SOP) for the maintenance of peripheral vein catheter patency and the prevention of IV catheter-related complications. This is a retrospective study, enrolling patients who received anesthesia between April 2010 and January 2011. The study included 1 month of

Application of Predictive Analytics in Pregnancy, Birth, and Postpartum Nursing Care.

Predictive analytics has emerged as a promising approach for improving reproductive health care and patient outcomes. During pregnancy and birth, the ability to accurately predict risks and complications could enable earlier interventions and reduce adverse events. However, there are challenges and ethical considerations for implementing predictive models in perinatal care settings. We introduce major concepts in predictive analytics and describe application of predictive modeling to perinatal care topics such as fertility, preeclampsia, labor onset, vaginal birth after cesarean, uterine ruptu

Untoward pathophysiological events: simulation as an experiential learning option to prepare anesthesia providers.

Untoward pathophysiological conditions coupled with anesthesia delivery can lead to critical incidents. Anesthetists must be armed with a set of complex skills to respond effectively in acute situations. A survey of nurse anesthetists was conducted in which respondents were asked to report the frequency that they had experienced one of 24 select events in their practice in the last 60 days. They were asked to rank how meaningful it would be for graduate students to experience the same or a similar event in their training in order to be prepared for practice. Also, they were asked to rate, amon

A Rare Event With High Acuity: A Case of Autonomic Dysreflexia.

Autonomic dysreflexia (AD) presents a unique circumstance that many learn about in school but seldomly see in clinical practice. In this case study, AD is identified in a higher-level spinal cord injury (SCI) than what is typically imagined. In this case, physical and pharmacologic techniques are used to mitigate AD. Additionally, the procedure is still performed using various pharmacologic agents and collaborative efforts from the surgical and anesthesia team. A unique discussion on differentials and treatment options for AD is then presented. In conclusion, anesthesia providers must recogniz

Prevention of Nausea and Vomiting After Laparoscopic Sleeve Gastrectomy: Are We Doing Enough?

The worldwide obesity epidemic has led to an increase in the number of bariatric procedures for treatment. Currently, the most commonly performed bariatric surgical operation in the United States is the laparoscopic sleeve gastrectomy (LSG). The risk of postoperative nausea and vomiting (PONV) is exceptionally high following the LSG: as much as 65%. Anesthesia providers should be mindful of this elevated risk and stay up to date on evidence-based techniques that help to mitigate the incidence of PONV related to this specific procedure. The purposes of this clinical review are to increase provi

Prevention and Treatment of Laryngospasm in the Pediatric Patient: A Literature Review.

Laryngospasm is an exaggeration of a protective reflex that prevents aspiration of foreign objects into the lower airway (eg, during swallowing). This results in complete or partial closure of the glottis, and impedance or total obstruction of airflow to the trachea and lungs. Often, the resulting hypoxia will by itself break a laryngospasm; however, if the spasm continues without relief, it can lead to pulmonary edema, cardiac dysrhythmias, cardiac arrest, and ultimately death. This evidence-based literature review explores the pathophysiology of laryngospasm and covers mechanical and pharmac

Anesthetic implications of necrotizing fasciitis.

Necrotizing fasciitis, also known as necrotizing acute soft tissue injury (NASTI), commonly occurs as a consequence of group A streptococcal disease. An estimated 9,400 cases of invasive group A streptococcal disease occur annually in the United States, with 600 cases classified as NASTI. Mortality associated with NASTI is estimated at 20% to 50%. Research indicates that early diagnosis and surgical excision of necrotic tissue is the key to minimalization of morbidity and mortality associated with NASTI. Repeated surgeries typically are required in patients with NASTI, posing unusual anestheti

Setting the Foundation for an In Situ Simulation Program Through the Development of a Malignant Hyperthermia Simulation in the Operating Room.

This article reviews the VA Manchester Healthcare System's journey to develop a robust simulation program and how an in situ simulation for malignant hyperthermia (MH) laid the foundation. Nurse educators collaborated with a certified registered nurse anesthetist to develop a simulation in the operating room using our MH protocol, which included the support of the Malignant Hyperthermia Association of the United States. The positive reaction from this in situ training led to additional simulation requests for the education department. [J Contin Educ Nurs. 2020;51(11):523-527.].

Anesthetic management of children with craniofacial anomalies.

Children with craniofacial anomalies, in particular those with facial clefts and the associated syndromes of Treacher Collins, Pierre Robin, and Klippel-Fiel and craniosynostosis and its related syndromes, present some of the most hazardous and difficult challenges that anesthetists may encounter within the entire practice of pediatric anesthesia. Through this review of the embryological development of the above-listed anomalies, the specific concerns of the airway and coexisting congenital defects, the reader will develop a better understanding of each anomaly in order to create a safe anesth

Anesthetic management of the cocaine abuse patient.

Cocaine is an extremely addictive local anesthetic which can produce stimulation of the sympathetic nervous system due to the inhibition of catecholamine reuptake at the synaptic junction. Because of the rapid metabolism of cocaine, the probability of a patient presenting to the operating room with acute intoxication is unlikely. However, the physiological effects of chronic cocaine abuse on various organ systems have an impact on anesthesia management. A preoperative review of major organ systems is essential. Selective beta 1 antagonists (i.e., esmolol) may need to be titrated with a direct

Anaphylactoid syndrome of pregnancy: a review of the literature with latest management and outcome data.

This article is a review of information on the subject of anaphylactoid syndrome of pregnancy or amniotic fluid embolism, as it is more commonly known. Information was found through a computer search of English articles published from 1958 to 2001. Anaphylactoid syndrome of pregnancy has an 80% to 90% mortality rate. It is difficult to diagnose, and once symptoms occur, there is a rapid deterioration in the patient's condition. It has severe manifestations such as shock and coagulopathy. Identification of the exact mechanism and pathophysiology that trigger the syndrome continue to be investig

Xenon: anesthesia for the 21st century.

Xenon is a naturally occurring, gaseous element that comprises 0.000008% of air, or 0.05 parts per million. It was discovered by Ramsey and Travers in 1898. Xenon is found on the Periodic Table in group 0, which is the group commonly referred to as the noble or inert gases. It is obtained by fractionally distilling liquefied air. Xenon has been studied sporadically within the discipline of anesthesia as a replacement for nitrous oxide. Because it is a naturally occurring element, xenon is not a pollutant. It is not an occupationally hazardous gas. It is neither teratogenic nor fetotoxic, as is

Intraoperative ventilatory settings during abdominal surgery among 1,091 anesthesia providers.

BACKGROUND: Data are lacking regarding current ventilatory settings during abdominal surgery. Expert recommendations emphasize the need for protective ventilation, and several tools are now available to individualize positive end-expiratory pressure (PEEP). METHODS: We conducted a nationwide cross-sectional survey among French anesthesiologists and nurse anesthetists in November 2025 to assess ventilatory settings during abdominal surgery, both for laparotomy and laparoscopy. Protective ventilation was defined as the combination of low tidal volume (6-8 mL/kg predicted body weight), low-to-mo
