[Psychological factors and sleep in the lifeworld nursing home from the nursing home residents' perspective. A qualitative study].

BACKGROUND: Sleep disorders are common among nursing home residents. However, the possible involvement of psychological factors has been given little attention up until now. We investigated nursing home residents' perspective on psychological factors and sleep disorders within the nursing home environment. MATERIALS AND METHODS: We conducted a qualitative interview study comprising episodic interviews with 30 nursing home residents in five different nursing homes. The data was analysed by thematic coding. RESULTS: Nursing home residents require peace of mind in order to sleep well. Ruminatio

Nurses' and family members'. Perceptions of nursing home residents' needs.

The purpose of this study was to determine the degree of agreement between nursing home staff and families in assessing the needs of cognitively impaired nursing home residents. A convenience sample of 36 family members of nursing home residents with cognitive impairment and the nurses assigned to those patients was recruited from three nursing homes. The Nurse and Resident Perceptions Questionnaire, adapted from Lindgren and Linton (1991), was used to assess functioning in activities of daily living, physiologic, and psychosocial areas. Significant differences were found for eight items of ph

Nurses' Pain Management Competency in Nursing Home Residents With Dementia.

BACKGROUND: Nursing home (NH) residents with dementia experience undertreated pain. Nurses have inconsistent pain management practices in dementia, and limited studies have examined nurses' pain management competencies using observations. PURPOSE: Thus, this study examined NH nurses' pain management competency in dementia using the Pain Competency Evaluation in Dementia (PACED) scale. METHODS AND DESIGN: This descriptive study recruited NH Licensed practical nurses (LPN) and registered nurses (RN) in Pennsylvania. Each nurse performed pain management on two randomly assigned virtual simulati

Screening for atrial fibrillation in high-risk nursing home residents.

BACKGROUND: Various methods have been implemented for screening of patients for atrial fibrillation (AF), but the yield has generally been low. Targeting high-risk patients may improve detection of asymptomatic AF, which could be of value if appropriate treatment could be initiated before a potential thromboembolic event. OBJECTIVE: The purpose of this study was to test screening of high-risk nursing home residents having ≥2 risk factors for AF and no previous history of AF using a smartphone-based electrocardiographic (ECG) monitoring device to determine whether it is an accurate, easy-to-us

Advance Directives and Care Received by Older Nursing Home Residents.

BACKGROUND: Research shows variable success as to whether care provided aligns with individual patient preferences as reflected in their advance directives (AD). OBJECTIVE: We aimed to study AD status and subsequent care received in older nursing home (NH) residents deemed at risk for infections and care transitions: those with a urinary catheter (UC), feeding tube (FT), or both. Design/participants/measurements: A subgroup analysis of a prospective cohort of 90 residents with a UC and/or FT from 15 NHs in southeast Michigan. Outcomes assessed at enrollment and at 30-day intervals were hospit

Prosthetic rehabilitation of edentulism prevents malnutrition in nursing home residents.

To investigate the association between prosthetic rehabilitation and malnutrition in institutionalized elders, 255 nursing home residents were recruited for this study and underwent a comprehensive dental examination. The body mass index (BMI) was administered to estimate the nutritional condition. Participants with BMI < 20 kg/mc were categorized as malnourished (n = 33), whereas all others were categorized as adequately nourished (n = 222). The number of teeth present and the prevalence of prosthetic rehabilitation were significantly lower in malnourished participants (P < .05). Malnutrition

Prevalence of vertebral fracture in oldest old nursing home residents.

UNLABELLED: We evaluated vertebral fracture prevalence using DXA-based vertebral fracture assessment and its influence on the Fracture Risk Assessment (FRAX) tool-determined 10-year fracture probability in a cohort of oldest old nursing home residents. More than one third of the subjects had prevalent vertebral fracture and 50% osteoporosis. Probably in relation with the prevailing influence of age and medical history of fracture, adding these information into FRAX did not markedly modify fracture probability. INTRODUCTION: Oldest old nursing home residents are at very high risk of fracture.

Major abdominal surgery in nursing home residents: a national study.

OBJECTIVE: To determine surgical risk in nursing home residents undergoing major abdominal surgery. BACKGROUND: Recent studies suggest that surgery can be performed safely in the very old. Surgical risk in nursing home residents is poorly understood. METHODS: We used national Medicare claims and the nursing home Minimum Data Set (1999-2006) to identify nursing home residents undergoing surgery (surgery for bleeding duodenal ulcer, cholecystectomy, appendectomy, and colectomy, n = 70,719). We compared operative mortality and use of invasive interventions (mechanical ventilation, intravascular

Detection and correction of underweight problems in nursing home residents.

Subnormal weight in the elderly is a well-known phenomenon; the ramifications may result in illness and death. There are many reasons for weight loss, usually related to changes that accompany aging or against a background of acute or chronic illness. Our goal in this study was to identify the underweight people living in our retirement facility at different functional levels and to plan and carry out nursing interventions in cooperation with the rest of the multidisciplinary team to increase their weight to within normal limits. Of the 205 residents who live in the home, 48 (23.4%) were found

Family perceptions of palliative care consultations for nursing home residents.

OBJECTIVES: Palliative care (PC) consultations, often facilitated by providers external to nursing homes (NHs), can reduce unwanted hospitalizations and improve decision-making and communication for nursing home (NH) residents. However, limited literature has documented the specific elements within PC consultations that drive positive outcomes. Given the critical role of family members, especially for residents with cognitive impairment, this study aimed to: (1) identify actions and interactions during recent PC consultation visits, and (2) summarize family perceptions of these visits. METHODS

Asymptomatic Acute Hypertension in Nursing Home Residents: Rarely an Emergency.

Hypertension is extremely common in the geriatric population, with estimates as high as 75%. Among nursing home residents, the prevalence is even higher. Asymptomatic hypertension, an acute increase in blood pressure without end-organ involvement, is common in this patient population and results in many cases being transported and evaluated in an emergency department (ED). Many of these cases do not need ED evaluation. The Geriatric Emergency Department Accreditation and Improved Care of Nursing Home Residents and ED Task Force, with representatives from the American College of Emergency Physi

Sexual abuse of older nursing home residents: a literature review.

Despite an increasing literature related to elder abuse, sexual abuse of older persons in general and of vulnerable adults living in nursing homes in particular is still sparsely described. The purpose of this study was to assess the state of knowledge on the subject of sexual abuse against older nursing home residents through a literature review. Systematic searches in reference databases including Cinahl, Medline, OVID Nursing Database, ISI Web of Science, PsycINFO, Cochrane Library, and SveMed + were conducted. Through several phases of selection of the articles, using strict inclusion and

Prevalence and associated factors of dysphagia in nursing home residents.

Dysphagia is a major health care problem in nursing homes. It can lead to fatal complications including aspiration pneumonia and even death. The aims of this study were to evaluate the prevalence of dysphagia in nursing home residents in South Korea and to identify factors associated with dysphagia. The study was conducted in two urban nursing homes and a total of 395 older adults (aged 65-103 years old, 76.5% female) were enrolled. The presence of dysphagia was evaluated using the Gugging Swallowing Screen (GUSS) test. Out of 395 older adults, the prevalence of dysphagia was 52.7%. The follow
