Nutritional support for critically ill children.

BACKGROUND: Nutritional support in the critically ill child has not been well investigated and is a controversial topic within paediatric intensive care. There are no clear guidelines as to the best form or timing of nutrition in critically ill infants and children. This is an update of a review that was originally published in 2009. . OBJECTIVES: The objective of this review was to assess the impact of enteral and parenteral nutrition given in the first week of illness on clinically important outcomes in critically ill children. There were two primary hypotheses:1. the mortality rate of crit

[Nutritional support in chronic neurological diseases].

Malnutrition is common in neurodegenerative disorders and is associated with a worse prognosis and an increased risk of complications. Factors leading to malnutrition in these patients are: diseased nutrient intake, due to anorexia, dysphagia and other factors, gastrointestinal symptoms, and energy expenditure alterations. Nutritional evaluation and monitoring is mandatory and should be part of regular clinical evaluation. It will help to identify those patients that need specialized nutritional support. In this paper, relevant aspects regarding nutritional evaluation and support in patients s

[Nutritional support in acute kidney injury].

Optimal nutritional requirements and nutrient intake composition for patients with acute kidney injury is still a partially unresolved issue. Targeting nutritional support to the actual protein and energy needs improves the clinical outcome of critically ill patients. So far, very few data are currently available on this topic in acute kidney injury. In this specific clinical condition, the risk for under- and overfeeding may be increased by factors interfering on nutrient need estimation, such as rapidly changing body weight due to fluid balance variations, nutrient losses and hidden calorie

Nutritional support of the oncology patient.

This review focuses on the nutritional support of the non-surgical cancer patient. The following topics are reviewed: cancer cachexia (definition and staging, prevalence and impact on clinical outcome); nutritional screening to identify potential candidates for nutritional support; nutritional requirements in terms of macro-and micro-nutrients of the advanced cancer patient. Finally, the indications and results of nutritional support are presented with a special focus on the following issues: routes of delivering nutritional support, the use of standard or n-3 fatty acids-enriched oral nutriti

Nutritional support in acute kidney injury.

Acute kidney injury (AKI), at least in critically ill patients, seldom occurs as isolated organ failure. Much more often it emerges as a component of the multiple organ failure syndrome, within the framework of the severe and prolonged catabolic phase determined by critical illness, and intensified by specific derangements in substrate utilization due to the acute loss of kidney function. On these bases, patients with AKI often have protein-energy wasting (preexisting and/or hospital acquired), which represents a major negative prognostic factor. Thus, nutritional support is frequently require

Ornithine alpha-ketoglutarate in nutritional support.

Ornithine alpha-ketoglutarate (OKG) is a salt formed of two molecules of ornithine and one molecule of alpha-ketoglutarate. OKG has been successfully used by the enteral and parenteral route in burn, traumatized, and surgical patients and in chronically malnourished subjects. According to the metabolic situation, OKG treatment decreases muscle protein catabolism and/or increases synthesis. In addition, OKG promotes wound healing. The mechanism of action of OKG is not fully understood, but the secretion of anabolic hormones (insulin, human growth hormone) and the synthesis of metabolites (gluta

Nutritional support for the shock patient.

In the past, patients were indescriminately fed until they began to gain weight; however, this should not be the philosophy today. With the specialized solutions and laboratory tests available today, precise tailoring of a patient's nutritional needs can be achieved. Nutritional support has been devised to maintain the energy, vitamins, and minerals necessary to supply amino acids for protein synthesis and to reduce the loss of amino acids from the periphery. Providing patients with early nutritional supplementation either by the parenteral or enteral route decreases the incidence of septic co

Nutritional support of the cancer patient.

Cachexia is common in patients with advanced neoplastic disease, and beyond certain levels, is associated with decreased survival. Depletion of vitamins and minerals may co-exist with protein-calorie undernutrition. The causes of the cancer cachexia syndrome may include decreased food intake, increased fecal and urinary nutrient losses, and abnormal metabolic pathways in both the host and the tumor. The complex nature of weight loss in cancer makes restoration of lean body mass more difficult in cancer than in non-cancer undernutrition. Non-volitional feeding (hyperalimentation) frequently cau

Nutritional support during pregnancy and lactation.

Needs for virtually all nutrients increase during pregnancy and lactation but those for iron are the most difficult to meet from a normal diet. During pregnancy, nutritional objectives include helping the woman to increase the quality of her diet in protective nutrients as well as energy to meet the elevated needs of gestation, ensuring satisfactory weight gain patterns, prescribing iron and possible folic acid supplements, ensuring that intakes of alcohol, drugs, and other potentially harmful substances are moderate, preparing her for feeding the infant, and coping with any special diet-relat

Nutritional assessment and support during infection.

Protein malnutrition is one of the principal factors relating to morbidity and mortality during infection. Nutritional assessment is required to determine the severity of depletion and degree of hypermetabolism affecting this patient population. Simple anthropometric and 24-hr urine collections together with routine biochemical analyses can readily allow clinical assessment to occur. Optimal utilization of dietary intake is dependent on the degree of protein catabolism and energy expenditure in excess of the basal energy requirement. Urinary nitrogen excretion in 24-hr on a protein-free diet i

Do data support nutrition support? Part I: intravenous nutrition.

Intravenous (parenteral) nutrition has been advocated widely as adjunctive care in patients with a variety of underlying diseases. However, the enthusiasm for this therapeutic intervention was based largely on expert opinion. Because the best way to assess the efficacy of any treatment is to test it in a randomized controlled trial, this review will focus on data that was derived from such studies. Using established search strategies, randomized controlled trials were sought that compared one of two forms of intravenous nutrition: parenteral nutrition (nitrogen and >or=10 kcal/kg/day of non-pr

Nutrition Support Therapy.

Nutrition support therapy is the delivery of formulated enteral or parenteral nutrients to restore nutritional status. Family physicians can provide nutrition support therapy to patients at risk of malnutrition when it would improve quality of life. The evidence for when to use nutrition support therapy is inconsistent and based mostly on low-quality studies. Family physicians should work with registered dietitian nutritionists to complete a comprehensive nutritional assessment for patients with acute or chronic conditions that put them at risk of malnutrition. When nutrition support therapy i

Pediatric nutrition support.

Pediatric patients now survive and thrive because of advancements made in nutrition support. Growth and development is of paramount importance in this population and adequate nutrition is necessary. Growth of premature infants receiving nutrition support mirrors intrauterine rates. Survival without nutrition support is not always assured because of inadequate nutrient stores. Institution of nutrition support in children with chronic illnesses also promotes growth and development. Enteral nutrition is preferred because of immunologic benefits, lower cost, and less frequent infectious complicati

Do data support nutrition support? Part II. enteral artificial nutrition.

Artificial nutrition is widely advocated as adjunctive care in patients with a variety of underlying diseases. In recent years more emphasis has been placed on delivering it directly into the gastrointestinal tract through tubes in the stomach or proximal small intestine (enteral nutrition). Because the efficacy of any therapeutic intervention is best established by demonstrating it in one or more randomized controlled trials, this review focuses on data from such studies. The specific issue to be assessed is the ability of enteral nutrition to influence the mortality and morbidity of various
