The current, globalised food system supplies 'cheap' food to a large proportion of the world's population, but with significant social, environmental and health costs that are poorly understood. The present paper examines the nature and extent of these costs for both rural and urban communities, by illustrating the financial pressures on food producers and manufacturers to produce cheap food, the disconnection people experience with how and where their food is produced, and the rise in obesity levels that plague the globe. The paper then proposes that community food systems may play an importa

It is now well-established that exercise can disturb various aspects of gastrointestinal integrity and function. The pathophysiology of these perturbations, termed "exercise-induced gastrointestinal syndrome (EIGS)," can lead to exercise-associated gastrointestinal symptom (Ex-GIS) inconveniences. EIGS outcomes can impact physical performance and may lead to clinical manifestation warranting medical intervention, as well as systemic responses leading to fatality. Athlete support practitioners seek prevention and management strategies for EIGS and Ex-GIS. This current position statement aimed t

PURPOSE: Registered dietitians (RDs) have the potential in cancer cachexia management. We investigated RDs' knowledge on cancer cachexia and assessed the effects of factors on RDs' perception of playing an important role in cancer cachexia management.
METHODS: This is a secondary analysis of a survey examining the perspectives of health care professionals on cancer cachexia management. We sent the questionnaire to 451 RDs. RDs were divided into two groups: RDs with and without the perception. Comparisons were made using the Mann-Whitney U test or chi-square test. To examine the effects of fac

Young children dependent on gastrostomy feeding are examples of special-needs children now eligible for expanded nutrition services in schools through Public Law 99-457. With the implementation of this law, nutritionists have an opportunity to expand their roles as consultants to special education teachers. A team approach involving nutrition, nursing, and special education was used to prepare a gastrostomy-fed preschool child and his family for school. The family of the multihandicapped child had been isolated by the child's feeding problems and viewed the problems as barriers to his entering

Large language models (LLMs) can answer dietary questions, but their use as substitutes for registered dietitians remains insufficiently quantified. Evaluations have identified inaccurate nutrient targets, incomplete clinical advice, and insufficient personalization, alongside useful performance on some general nutrition tasks. Social media and food-delivery interfaces may also influence dietary choices through food cues, social endorsement, and choice architecture. This conceptual perspective presents ADRIFT (Algorithmic Dietary Reconfiguration through Informational, Framing, and Technologica

OBJECTIVES: The evidence supporting nutritional counseling (NC) as a care strategy for inflammatory bowel disease (IBD) was evaluated in this review. Using a "WH" (what, why, which/where, who, when, and who/for whom) question approach, the review is structured around key questions that prompt detailed answers, providing insights into how NC can optimize IBD management.
BACKGROUND: Nutritional counseling has emerged as an essential component of effective management for IBD. However, a lack of nutritional expertise can limit its successful implementation, hindering optimal patient care.

Many patients undergo small bowel and colon surgery for reasons related to malignancy, inflammatory bowel disease (IBD), mesenteric ischemia, and other benign conditions, including post-operative adhesions, hernias, trauma, volvulus, or diverticula. Some patients arrive in the operating theatre severely malnourished due to an underlying disease, while others develop complications (e.g., anastomotic leaks, abscesses, or strictures) that induce a systemic inflammatory response that can increase their energy and protein requirements. Finally, anatomical and functional changes resulting from surge

BACKGROUND AND AIMS: Patients with neuroendocrine tumours (NET) are at nutritional risk due to symptoms and treatment side-effects. Current evidence-based guidelines lack information regarding optimal nutritional management. This study aimed to describe health professional knowledge and management of nutrition complications in GEP NET patients and summarise current international practice.
METHODS: Multidisciplinary health professionals who regularly provide care for NET patients, were invited to participate in a 21-item online survey. Survey questions asked about symptom prevalence, nutrition

Continuing care (CC) facilities have been impacted by a growing demand for services, insufficient resources for the provision of quality food and nutrition care, and, most recently, the COVID-19 pandemic. This study explored the roles and responsibilities of dietitians working in CC facilities in Nova Scotia (NS) before and after the COVID-19 first wave. Using ethics-approved questionnaires, the estimated 75 dietitians working in CC facilities in NS were surveyed in Fall 2019 and Fall 2020 about their roles and responsibilities. Twenty responded to the first questionnaire and 15 to the second.

Weight loss is a physical representation of the impact of cancer on the body and loss of control. For patients with esophagogastric cancer (EGC), loss of appetite and weight loss lead to reduced quality of life and poor clinical outcomes. In this issue of JNCCN, Dijksterhuis et al present data evaluating the prevalence of pretreatment cachexia, association of pretreatment cachexia with survival, and early integration of dietitians in the care of patients with EGC. This 3-year Dutch study included >400 patients with EGC across all stages. Half of the patients had cachexia at presentation, which

BACKGROUND: Malnutrition, sarcopenia and cachexia are clinical wasting syndromes characterised by muscle loss. Systematic monitoring by body composition assessment (BCA) is recommended for the diagnosis, treatment and monitoring of the syndrome(s). This study investigated practices, competency, and attitudes of Australian dietitians regarding BCA, to inform a local implementation process.
METHODS: Applying the Action cycle in the Knowledge to Action framework, surveys were distributed to the 26 dietitians of an 800-bed tertiary hospital. The survey assessed barriers and enablers to performing

AIM: While a patient's nutritional status is known to generally have a role in postoperative wound healing, there is little information on its role as therapy in the multifaceted problem of diabetic foot infections (DFIs).
METHODS: We assessed this issue by conducting a retrospective case-control cohort study using a multivariate Cox regression model. The nutrition status of the DFI patients was assessed by professional nutritionists, who also orchestrated the nutritional intervention (counselling, composition of the intrahospital food) during hospitalization.
RESULTS: Among 1,013 DFI episod

BACKGROUND & AIMS: To identify manufactured soy-based products more recommended by pediatricians and nutritionists; to determine fluoride concentrations in these products; to evaluate children concerning fluorosis in primary teeth and its association with the consumption of soy-based products.
METHODS: Pediatricians and Nutritionists answered a questionnaire about soy-based products they most recommended to children. Fluoride concentrations of the 10 products more cited were analyzed with the ion-specific electrode. Dental fluorosis exams were performed in 315 4-6-year-old children. Dean's In

BACKGROUND: Economically disadvantaged families receive care in both clinical and community settings, but this care is rarely coordinated and can result in conflicting educational messaging. WEE Baby Care is a pragmatic randomized clinical trial evaluating a patient-centered responsive parenting (RP) intervention that uses health information technology (HIT) strategies to coordinate care between pediatric primary care providers (PCPs) and the Special Supplemental Nutrition Program for Women, Infant and Children (WIC) community nutritionists to prevent rapid weight gain from birth to 6 months.

Information on methods of initiating the phenylalanine-restricted diet was obtained from the medical personnel of sixteen clinics in the Collaborative Study of Children Treated for Phenylketonuria. The four dietary methods used initially to lower serum phenylalanine were: Normal dilution Lofenalac, Lofenalac, with phenylalanine added as milk, Lofenalac with milk added to provide 200 mg. phenylalanine per day, and alternating bottles of normal dilution Lofenalac and cow's milk during the first four days of therapy. During the first week of treatment, eight clinics obtained serum phenylalanine d
