Congenital isolated unilateral third nerve palsy in children: the diagnostic contribution of high-resolution MR imaging.

PURPOSE: To describe the neuroanatomical correlates of unilateral congenital isolated oculomotor palsy by means of high-resolution MRI. METHODS: Children with a clinical diagnosis of congenital isolated oculomotr palsy and with a high-resolution MRI acquisition targeted on the orbits and cranial nerves were selected and included in the study. An experienced pediatric neuroradiologist evaluated all the exams, assessing the integrity and morphology of extraocular muscles, oculomotor, trochlear and abducens nerves as well as optic nerves and globes. Clinical data and ophthalmologic evaluations w

Bilateral Extensive Ossification of the Posterior Petroclinoid Ligament: An Anatomical Case Report and Literature Review.

BACKGROUND: The posterior petroclinoid dural fold (commonly referred to as a ligament) forms the roof of the trigeminal porus and the roof of the petroclival venous confluence. It lies in close proximity to the oculomotor nerve that crosses it. Due to the low availability of research material, only a few cadaveric studies have been conducted on the microsurgical anatomy of the petroclinoid ligament in cases of its ossification. Thus our report complements earlier studies and provides detailed data on the spatial relationships between the ossified posterior petroclinoid ligament and the trigemi

Cranial nerve damage after paediatric head trauma: a long-term follow-up study of 741 cases.

A report is given on transient and permanent (6 months) impairment of cranial nerves after paediatric head injuries (N: 741). There is a link between severity of the injury, fractures on the base of the skull, its foramina and channels, and the frequency of cranial nerve involvement. One should try to establish whether a posttraumatic dysfunction of the cranial nerves is primary or secondary in nature, i.e. due to raised intracranial pressure or haemorrhage. In children after head injuries often the cranial nerves of the oculomotor system are affected (20.2%/7.0%)--transient (permanent), follo

[Isolated traumatic oculomotor nerve palsy caused by minor head trauma].

Isolated traumatic oculomotor nerve palsy caused by minor head trauma is extremely rare. We report a case of this condition. A 65-year-old woman was transferred to our hospital after minor head injury. Initial examination of her right eye revealed a dilated pupil. The result of computed tomography, magnetic resonance imaging, and magnetic resonance angiography of the brain were normal. An examination conducted 6 hours after the accident, examination demonstrated right ptosis, and another conducted 20 hours after the accident, she demonstrated partial limitation of adduction, supraduction, and

[Ocular injuries and childbirth].

INTRODUCTION: Severe ocular traumatisms related to childbirth are rare and often subordinate to maneuvers with instruments. The aim of this study was to report on the different severe ocular injuries that might occur during childbirth and their possible consequences. There can be several of these ocular injuries. PATIENTS AND METHODS: Since 1984, we have examined 11 cases of this type of traumatism. For each case, the childbirth circumstances, the affected side, the different injuries observed such as ocular, orbital, cranial and even cerebral lesions, and the results of the ophthalmological

Abducent nerve palsy following transverse fracture of the middle cranial fossa.

Transverse sphenoidal fractures may be associated with a variety of skull base injuries and neural deficits. Among those nerve injuries, oculomotor palsies and particularly sixth cranial nerve palsy, are quite common. Blows on the side of the head in the squamous temporal region may run across the floor of the middle cranial fossa through the greater wing of the sphenoid in the transverse cranial axis. We report three cases of patients who had sustained craniofacial injury which included a transverse fracture of the middle cranial fossa through the sphenoid sinus, extending to the petrous apex

Posttraumatic abducens to oculomotor nerve misdirection.

INTRODUCTION: Paradoxical patterns of extraocular muscle, eyelid, or pupillary movements can occur following injury between divisions of the oculomotor nerve, trigeminal and abducens nerves, and trigeminal and oculomotor nerves. We report three cases of unusual ocular motility and eyelid movements that are a result of aberrant connections between the abducens and oculomotor nerves. METHODS: Three patients with unusual eye movement abnormalities after trauma were studied. A complete ophthalmic examination plus neuroradiologic evaluation were performed. RESULTS: Each patient manifested an aber

Nontraumatic oculomotor nerve palsy following endoscopic sinus surgery.

The proximity of the paranasal sinuses to the orbit and its contents increases the chances of injuries during endoscopic sinus surgery (ESS). We present an extremely rare case of transient oculomotor nerve palsy with no direct injury following ESS, which has never been reported in the literature. The proper clinical approach and medical management are also discussed in this case report.

Oculomotor nerve palsy in the setting of an anterior cerebral A2 segment aneurysm.

INTRODUCTION: A case is presented which highlights a rare cause of oculomotor nerve palsy (third nerve palsy) in the setting of subarachnoid hemorrhage secondary to an A2 segment anterior cerebral aneurysm. A third nerve palsy is most often associated with posterior communicating artery aneurysms which are explained by the anatomic proximity of nerve and artery. Third nerve palsies from remote aneurysms however are more difficult to understand. PRESENTATION: A patient presented to the emergency department with severe headache, dizziness, nausea and vomiting. Her examination was remarkable onl

Oculomotor palsy in diabetics.

INTRODUCTION: Oculomotor palsy is one of the most frequent neuro-ophthalmologic complications of diabetic patients. It generates less interest in the literature than the other ocular manifestations. Our goal was to study the clinical, epidemiological, therapeutic and prognostic characteristics of oculomotor palsy in the diabetic. METHODS: This is a retrospective study of 24 diabetic patients with oculomotor palsy. The ophthalmological examination emphasized ocular motility. We performed an orthoptic assessment and a Hess-Lancaster test. Neuro-imaging was ordered in case of IIIrd and IVth nerv

Aberrant regeneration after traumatic oculomotor nerve palsy causing paradoxical infraduction on attempted supraduction.

A 52-year-old man was referred to our strabismus clinic for evaluation of abnormal ocular motility of the right eye of 40 years' duration that resulted from childhood trauma, when he had suffered a head contusion in an automobile accident. Immediately after the injury, he had ophthalmoplegia and ptosis in the right eye and was diagnosed with oculomotor nerve palsy. The present examination revealed a right exotropia of 35Δ and a hypotropia of 25Δ in the primary position at both near and far fixation. However, the right eye did not move upward on attempted supraduction; furthermore, infraduction

Occurrence of oculomotor dysfunctions in acquired brain injury: a retrospective analysis.

BACKGROUND: The purpose of this retrospective study was to determine the frequency of occurrence of oculomotor dysfunctions in a sample of ambulatory outpatients who have acquired brain injury (ABI), either traumatic brain injury (TBI) or cerebrovascular accident (CVA), with associated vision symptoms. METHODS: Medical records of 220 individuals with either TBI (n = 160) or CVA (n = 60) were reviewed retrospectively. This was determined by a computer-based query spanning the years 2000 through 2003, for the frequency of occurrence of oculomotor dysfunctions including accommodation, version, v

Rare Case of Post-Traumatic Abducens-Oculomotor Nerve Synkinesis Exhibiting Unusual Ocular Motility.

BACKGROUND Acquired synkinesis is a well-known phenomenon following oculomotor nerve injury. The abnormal movement appears within the distribution of 1 nerve, or other cranial nerves may be involved. The common misdirection of aberrant regeneration of oculomotor nerve involves the lid, extraocular muscles, or pupil. This report presents a case of aberrant connection between abducens and oculomotor nerve, which is quite rare. CASE REPORT A 21-year-old man with history of left-sided frontal, lateral orbital wall, and zygomatic fracture from head trauma in a motor vehicle accident presented for e

Tolosa-Hunt syndrome with reversible dissection aneurysm.

A 49-year-old female presented with recurrent intolerable right retro-orbital pain, ptosis and diplopia for 4 months. Neurological evaluation revealed right oculomotor nerve and abducent nerve injuries. Neuroimaging found enlarged right cavernous sinus, right intracavernous carotid dissection aneurysm and a stenosis proximal to it. Tolosa-Hunt syndrome (THS) was suggested and treated with steroid. The clinical symptoms were alleviated after the treatment. After 3 months of follow-up cerebral angiography showed the lesions of the right intracavernous carotid stenosis and the dissection almost d

Sub total unilateral oculomotor nerve palsy in a Le Fort I osteotomy.

A 35-year-old man developed a pupil sparing palsy of the oculomotor nerve after a standard Le Fort I osteotomy during which there had been more bleeding than usual. No definite cause was found but we suggest that it was caused by ischaemia of the nerve secondary to local injury by haematoma or instrumentation. The anatomical basis for this is discussed.

Surgical resection of A giant ventral pontine cavernous malformation: Two-dimensional video.

Cavernous malformations (CM) in the ventral pons pose significant surgical challenges due to their deep anatomical location and complex neurovascular structures [1]. This report details the successful surgical management of a giant ventral pontine CM in a 38-year-old female exhibiting left-sided limb weakness (muscle strength Grade IV), and was approved by the ethics committee. Magnetic resonance imaging (MRI) findings indicated the CM extended from the pial surface predominantly towards the right side. Utilizing diffusion tensor imaging (DTI), we determined that the corticospinal tract was la
