Endoscopic endonasal removal of the invaginated odontoid process of the C2 vertebra.

UNLABELLED: Pathological processes in the craniovertebral region (clivus, C1 anterior arch, odontoid process and body of the C2 vertebra, i.e. C0-C1-C2 segments) are very difficult to diagnose and treat. The craniovertebral junction instability may develop in the case of a significant lesion of C1-C2 segments. Among diseases causing destruction of the clivus structures and C1-C2 vertebrae and compression of the spinal cord, the following ones are most common: chordoma, giant cell tumor, osteoblastoma, rheumatoid lesion, metastases, platybasia, and basilar impression. These diseases can cause t

Fractures of the odontoid process: analysis of the functional results after surgery.

Eighteen patients who sustained type II/III fractures of the odontoid process, as classified by Anderson and D'Alonzo [2], underwent anterior screw fixation, as described in detail by Böhler [4] as well as Grob and Magerl [16]. Follow-up investigations 3, 12 and in some cases up to 60 months later confirmed sufficient functional results regarding the mobility of the upper cervical spine. The passive mobility of the cervical spine was analyzed by the radiographic evaluation technique of Dvorak and co-workers [12] and Penning [26], which revealed a hypomobility of the C2-3 segment 1 year after s

The odontoid process in Morquio-Brailsford's disease. The effects of occipitocervical fusion.

High definition computed cervical myelograms have been made in flexion and extension in 13 patients with Morquio-Brailsford's disease. We observed that: 1) odontoid dysplasia was present in every case, with a hypoplastic dens and a detached distal portion which was not always ossified; 2) atlanto-axial instability was mild, and anterior atlanto-axial subluxation was absent in most cases; 3) severe spinal cord compression, when present, was due to anterior extradural soft-tissue thickening; 4) this compression was not relieved by flexing or extending the neck and was manifested early in life; 5

Osteomyelitis of the Odontoid Process in Children: Two Cases and Review of the Literature.

OBJECTIVE: To report clinical, laboratory and radiologic manifestations in 2 infants with osteomyelitis of the odontoid process (dens). BACKGROUND: Vertebral osteomyelitis is uncommon, and osteomyelitis of the dens has rarely been reported in the pediatric population. METHODS: The medical records of 2 infants diagnosed with dens osteomyelitis were reviewed. RESULTS: Both infants had fever, which resolved spontaneously before admission. Both were nontoxic appearing with persistent neck stiffness and torticollis. White blood count and C-reactive protein were only mildly elevated in both cases

Persistence of the midline septum of the odontoid process: anatomical and radiological study.

BACKGROUND: During our screening of the computed tomography (CT) images of the craniocervical junction, we noticed that the odontoid process occasionally contains a highly ossified region in the midline consistent with a bony septum. Given its potential significance, this study aimed to investigate the highly ossified region in the midline of the odontoid process and to discuss its clinical relevance. METHODS: Eleven C2 vertebrae from formalin-embalmed cadaveric heads were assigned to micro-CT examination. When the midline septum was confirmed, gross anatomical and histological observations w

Indications and technique for the operative treatment of hypoplastic deformities of the odontoid process.

On the basis of four typical cases of congenital hypoplastic deformities of the odontoid process the clinical significance, symptoms, diagnosis and the indications for operative treatment of the malformations are described. It is further shown that these malformations lead to clinical manifestations only when associated with a primarily existing, or a later acquired inadequacy of the ligaments with movement-induced changes in the position of the atlas. The operative treatment recommended involves a special technique for a bilateral dorsal fixation with tibial grafts. By this operative techniqu

Tophaceous gout of the odontoid process causing glossopharyngeal, vagus, and hypoglossal nerve palsies.

Gout affecting the axial joints is uncommon; however, its involvement may be complicated by neurological symptoms associated with spinal compression at the affected level. Specific involvement of the odontoid process is even rarer. We report the first case of gout involving the odontoid process with resultant glossopharyngeal (CN IX), vagus (CN X) and hypoglossal (CN XII) nerve palsies.

Combined fractures of the odontoid process and upper thoracic spine: two case reports.

We describe two cases of combined fractures of the odontoid process and upper thoracic spine that have not been previously reported. The first patient, a 21-year-old man, sustained an odontoid process fracture with posterior displacement and a fracture-dislocation injury at T4/5 in a motorcycle accident. The second patient, a 66-year-old woman, fell from a cliff and sustained an odontoid process fracture with posterior displacement and a bursting fracture at T3 and T4 with rotation. The first patient exhibited complete paraplegia below the T5 level of the spinal cord. The second patient escape

[Direct osteosynthesis of fractures of the odontoid process using a biodegradable implant (B.O.P.)].

The authors describe a new technique for treatment fractures of the dens. An anterior approach (technique of Boehler) is used. It provides for one-stage fusion and no additional disturbance of C1-C2 rotation. Under AP and lateral guidance the body of C2 is drilled medially, and so are the site of fracture. Then comes the gradual impaction of a conical implant of biocompatible resorbable implant [*], tip cephalad. The biopolymer is used for grafting purposes in all type of fractures of the odontoid process, which permits the latter to be fused without using screws or any other metal implant.

Anderson type II fracture of the odontoid process: results of anterior screw fixation.

Controversy exists in the literature regarding the adequacy of one or two screws for direct fixation of the odontoid process. Proponents of the two-screw technique believe that a single screw is not adequate to stabilize the fracture. Conversely, the insertion of two 3.5-mm screws in the medullary cavity of the odontoid process is technically difficult and can jeopardize the surface area left for fracture healing. The authors conducted a prospective study of 30 cases with Anderson type II fracture of the odontoid process treated by direct anterior fixation using the two-screw technique. The sc

Safe Margin beyond Dens Tips to Ventral Dura in Anterior Odontoid Screw Fixation : Analysis of Three-Dimensional Computed Tomography Scan of Odontoid Process.

OBJECTIVE: Anterior odontoid screw fixation is a safe and effective method for the treatment of odontoid fractures. The surgical technique is recommended for perforation of the apical cortex of the dens by the lag screw. However, overpenetration of the apical cortex may lead to potentially serious complications such as damages of adjacent vascular and neural structures. The purpose of this study was to assess the role of three-dimensional computed tomography (CT) scan to evaluate the safe margin beyond dens tip to ventral dura for anterior odontoid screw fixation. METHODS: We retrospectively

Prevertebral soft-tissue swelling as a sign of undisplaced fracture of the odontoid process.

Marked evanescent swelling of the prevertebral soft tissues has been found in 2 patients with undisplaced fractures of the odontoid process. In one patient the significance of the swelling was appreciated only in retrospect and after shift had occurred at the fracture site. The experience gained from this patient alerted the radiologist and the clinician so that any further damage could be averted in the second patient. All patients with head injuries require radiography of the cervical spine. Soft-tissue swelling anterior to the atlas should be viewed in a serious light.

Fractures of the atlas associated with fractures of the odontoid process and transverse ligament ruptures.

One case of fracture of the atlas associated with rupture of the transverse ligament and three cases of atlantal fracture with associated fractures of the odontoid process are reported. In such cases, halo immobilization is recommended until the atlantal fracture has healed, followed by atlanto-axial fusion for instability of the cervical spine.
