Influence of SPRINT Study Type Automated Office Blood Pressure Measurements on Hypertension Diagnosis in Kidney Transplant Patients.

BACKGROUND/AIMS: We compare conventional office blood pressure measurements with automated SPRINT-study type readings in kidney transplant recipients in order to determine the impact of the white coat effect in a prospective observational study. METHODS: Adult patients with a functional renal transplant not dependent on dialysis were eligible. Readings were taken in the office in presence of the physician with an oscillometric method. Afterwards, readings were repeated with the patients resting alone in a quiet examination room with an automated blood pressure monitor. After 5 minutes of rest

Evaluation of the Omron HEM-907 automated blood pressure device: comparison with office and ambulatory blood pressure readings.

Blood pressure (BP) measured in the clinic is subject to the white coat effect and does not always indicate the 'usual' BP. Ambulatory BP is the current gold standard, but remains inconvenient for routine use. Interest in automated BP, where the healthcare professional is absent from the examination room during BP measurement, is growing, as this reduces the white coat effect and yields BP values that are close to ambulatory readings. The aim of this study was to investigate how well automated office BP (AOBP), measured using the Omron HEM-907 device, compares with observed office BP (OOBP, he

Diagnostic Performance of Unattended Automated Office Blood Pressure Measurement for Hypertension Screening Among People With and Without HIV.

BACKGROUND: The diagnostic performance of automated office blood pressure (AOBP) in screening for hypertension in people with HIV (PWH) is not known. METHODS: We conducted a cross-sectional analysis of baseline data from PWH and people without HIV (PWoH) from the Mwanza HIV&CVD cohort study. We conducted unattended AOBP and 24-hour ambulatory BP monitoring as recommended by international guidelines. Using average 24-hour BP as the reference standard, we estimated the prevalence of hypertensive diagnostic phenotypes and calculated measures of diagnostic performance at different diagnostic cuto

Automated office blood pressure and 24-h ambulatory measurements are equally associated with left ventricular mass index.

BACKGROUND: To determine whether automated office blood pressure (AOBP) readings are associated with left ventricular mass (LVM) index as closely as those of 24-h ambulatory blood pressure (ABP) and also to confirm that the values of the two methods are comparable in the appraisal of blood pressure in a European population referred for suspected hypertension. METHODS: In a sample of 90 individuals with office hypertension, we compared AOBP to awake systolic ABP measurements (ABPM) values and their associations with LVM indices, expressed as LVM divided by body surface area (LVMI(BSA)) and by

In the aftermath of SPRINT: further comparison of unattended automated office blood pressure measurement and 24-hour blood pressure monitoring.

AIMS: Several papers reported that unattended automated office blood pressure (uAutoOBP) is closely related to daytime ambulatory blood pressure monitoring (ABPM). In the present study, we aim to study uAutoOBP and its relation to 24-hour ABPM and ABPM variability. MATERIAL AND METHODS: Stable treated hypertensive subjects were examined in two Czech academic hypertension centres. uAutoOBP was measured with the BP Tru device; attended BP three times with auscultatory method (AuscOBP) by the physician. ABPM was performed within one week from the clinical visit. RESULTS: Data on 98 subjects age

Ambulatory and Unattended, Automated Office Blood Pressure in People With and Without HIV in Tanzania: A Cohort Study of Repeatability and Regression to the Mean.

Sub-Saharan Africa (sSA) bears a high burden of hypertension and HIV, yet blood pressure measurement often relies on single-day, conventional office readings, prone to measurement error. We assessed the short-term repeatability of unattended, automated office BP (uAOBP) and ambulatory blood pressure monitoring (ABPM), and whether reproducibility differs by HIV status in Tanzania. We analyzed data from the Mwanza HIV&CVD cohort study, using measurements from enrollment and 1-month later. Bland-Altman plots visualized mean differences and limits of agreement. Reproducibility of hypertension clas

Automated office blood pressure is in agreement with awake and mean 24-hour ambulatory blood pressure at the lower blood pressure range.

Automated office blood pressure measurement eliminates the white coat effect and is associated with awake ambulatory blood pressure. This study examined whether automated office blood pressure values at lower limits were comparable to those of awake and mean 24-hour ambulatory blood pressure. A total of 552 patients were included in the study, involving 293 (53.1%) men and 259 (46.9%) women, with a mean age 55.0 ± 12.5, of whom 36% were treated for hypertension. Both systolic and diastolic automated office blood pressures exhibited lower values compared to awake ambulatory blood pressure among

Comparison of blood pressure measurements using an automated blood pressure device in community pharmacies and family physicians' offices: a randomized controlled trial.

BACKGROUND: Accurate measurement of blood pressure is the foundation of appropriate diagnosis, treatment and ongoing management of hypertension. The use of automated blood pressure devices in community settings such as pharmacies provide opportunities for additional blood pressure measurement; however, it is important to ensure that these measurements are comparable to those taken in physicians' offices using the same devices. We conducted a randomized controlled trial to assess whether blood pressure readings assessed by use of an automated device differed according to the setting, specifical

Clinical Impact of 3- Vs. 5-Minute Delay and 30- Vs. 60-Second Intervals on Unattended Automated Office Blood Pressure Measurements.

BACKGROUND: Guidelines advise automated office blood pressure (AOBP) with an initial 5-minute delay and multiple measurements at least 60 seconds apart. Recent studies suggest that AOBP may be accurate with shorter delays or intervals, but evidence in clinical settings is limited. METHODS: Patients referred to 1 hypertension (HTN) center underwent 24-hour ambulatory blood pressure monitoring (ABPM) and 1 of 4 nonrandomized, unattended AOBP protocols: a 3- or 5-minute delay with a 30- or 60-second interval, i.e., 3 min/30 s/30 s, 3/60/60, 5/30/30 and 5/60/60 protocols. Comparison of awake ambulatory blood pressure and automated office blood pressure using linear regression analysis in untreated patients in routine clinical practice.

The recent American hypertension guidelines recommended a threshold of 130/80 mmHg to define hypertension on the basis of office, home or ambulatory blood pressure (BP). Despite recognizing the potential advantages of automated office (AO)BP, the recommendations only considered conventional office BP, without providing supporting evidence and without taking into account the well documented difference between office BP recorded in research studies versus routine clinical practice, the latter being about 10/7 mmHg higher. Accordingly, we examined the relationship between AOBP and awake ambulator

Atrial Fibrillation Screening During Routine Automated Office, Home, and Ambulatory Blood Pressure Measurement: A Diagnostic Test Accuracy Systematic Review and Meta-Analysis.

BACKGROUND: Atrial fibrillation (AF) is often asymptomatic and undiagnosed. As AF and hypertension often coexist, opportunistic AF detection during routine automated blood pressure (BP) measurement appears to be an attractive screening method. METHODS: A systematic literature search was conducted to identify studies assessing the diagnostic test accuracy of office, home, or 24-hour ambulatory BP measuring devices with AF detection algorithms versus reference electrocardiography. Analyses were performed per participant (AF status based on several BP readings; most office/home devices) or per r

Comparing Automated Office Blood Pressure Readings With Other Methods of Blood Pressure Measurement for Identifying Patients With Possible Hypertension: A Systematic Review and Meta-analysis.

IMPORTANCE: Automated office blood pressure (AOBP) measurement involves recording several blood pressure (BP) readings using a fully automated oscillometric sphygmomanometer with the patient resting alone in a quiet place. Although several studies have shown AOBP measurement to be more accurate than routine office BP measurement and not subject to a "white coat effect," the cumulative evidence has not yet been systematically reviewed. OBJECTIVE: To perform a systematic review and meta-analysis to examine the association between AOBP and office BP readings measured in routine clinical practice

Automated blood pressure measurement for diagnosing hypertension.

Guidelines for the management of hypertension have started to include home blood pressure (BP) and 24-h ambulatory BP monitoring as preferred methods for diagnosing hypertension. The next step will be to incorporate automated office BP measurement into the algorithm for diagnosing hypertension. Recent studies support this approach with automated office BP readings being closely correlated with the ambulatory BP.
