Parotidectomy: 10-Year Experience in the Hands of Surgical Oncologists.

BACKGROUND AND OBJECTIVES: Parotidectomy is the most common salivary gland surgery, but there remains a paucity of literature regarding its outcomes in the hands of surgical oncologists. METHODS: A retrospective review was conducted of all patients (n = 74) who underwent parotidectomy by surgical oncologists at a single institution over 10 years (2015-2025). Demographics, perioperative characteristics, and complications were compared. Associations between clinical factors and complications were evaluated by multivariate logistic regression. RESULTS: Most patients were White (85.1%) and male

Barriers to publishing early phase clinical trials: the oncologists' perspective.

INTRODUCTION: Findings from early phase studies are not always placed in the public domain. This study aims to explore why many early phase clinical oncology studies are not published, as well as identify the potential barriers investigators encountered in the publication process. METHODS: Semi-structured interviews were conducted among investigators with experience in early phase clinical oncology studies. Interviews were analyzed using reflexive thematic analysis. RESULTS: Twenty-one investigators were interviewed. The majority worked in Europe (n = 13), while other investigators were base

Anaesthesia, analgesia, and cancer outcomes: time to think like oncologists?

Cao and colleagues present a follow-up analysis of a previous RCT among >1200 older adults (mean age 72 yr) undergoing cancer surgery, originally designed to evaluate the effect of propofol or sevoflurane general anaesthesia on delirium, here to evaluate the effect of anaesthetic technique on overall survival and recurrence-free survival. Neither anaesthetic technique conferred an advantage on oncological outcomes. We suggest that although it is entirely plausible that the observed results are truly robust neutral findings, the present study could be limited, like most published studies in the

Placenta accreta spectrum: a non-oncologic challenge for gynecologic oncologists.

Placenta accreta spectrum disorders are a major risk factor for severe postpartum hemorrhage and maternal death worldwide, with a rapidly growing incidence in recent decades due to increasing rates of cesarean section. Placenta accreta spectrum disorders represent a complex surgical challenge, with the primary concern of massive obstetrical hemorrhagic sequelae and organ damage, occurring in the context of potentially significant anatomical and physiological changes of pregnancy.  Most international obstetrical organizations have published guidelines on placenta accreta spectrum, embracing the

Burden on oncologists when communicating the discontinuation of anticancer treatment.

OBJECTIVE: Communicating the discontinuation of anticancer treatment to patients is a difficult task. The primary aim of this study was to clarify the level of oncologist-reported burden when communicating about discontinuation of an anticancer treatment. The secondary aims were (i) to identify the sources of burden contributing to their levels and (ii) to explore the useful strategies to alleviate their burden. METHODS: A multicenter nationwide questionnaire survey was conducted on 620 oncologists across Japan (response rate, 67%). RESULTS: High levels of perceived burden were reported by 4

[Interdisciplinary psychoeducational intervention by oncologists proved helpful for cancer patients].

OBJECTIVES: We prospectively evaluated the effects of a six-session psychoeducational intervention held by medical doctors or psychologists in a German acute cancer center setting. METHODS: A cluster randomization was used to assign n=108 oncologic patients (55 female, 53 male; mean age=58.5) to the intervention or the control group. The self-rated amount of information about cancer-specific topics, quality of life (EORTC), coping (TSK) and anxiety and depression (HAD-S) were measured at the beginning of the intervention (t0) as well as two and four months later (t1). RESULTS: At t1 the leve

Sinonasal phosphaturic mesenchymal tumour: radiation oncologist's perspective.

Tumour-induced osteomalacia is a rare cause of osteomalacia, the majority of which is of mesenchymal origin. Oncogenic osteomalacia is a potentially curable condition caused by phosphaturic mesenchymal tumours. We present the case of a woman in her 30s with a sinonasal phosphaturic mesenchymal tumour, treated with surgical excision followed by adjuvant intensity-modulated radiotherapy and subsequent adjuvant chemotherapy. The patient experienced minimal adverse effects during radiation. There was good local control and cosmetic outcomes with no radiation-related toxicity at a follow-up period

Artificial Intelligence in Gynaecological Malignancies: Perspectives of a Clinical Oncologist.

Gynecological malignancies are treated with a multi-disciplinary approach. There are two important factors, the stage and Karnofsky performance status scale (KPS) of patients, which guide the treatment strategy by single or multiple modalities in terms of surgery, radiotherapy, or chemotherapy. Various aspects are included in the workflow of gynecological malignancies, like screening, diagnosis in each individual, treatment modalities, and finally, follow-up to see for outcomes leading to the development of new research protocols. The quality data plays an important role in every step. Artific

Ocular Toxicity in Breast Cancer Management: Manual for The Oncologist.

Ocular adverse events are common to many antineoplastic agents, although often misunderstood. In most cases, they are easily manageable, but sometimes they require instrumental diagnostics and specific treatments. There are currently no international guidelines for the management of these toxicities. In this review we summarized the main ocular adverse events related to the antineoplastic agents used in the treatment of breast cancer, analyzing their clinical presentation and management, trying to provide a useful tool to be used in clinical practice.

Management of oncologic transplant recipient: the profile of the radiation oncologist.

Long-term survival after organ transplantation is increasing. As a result, many physicians may encounter various solid organ cancers during follow up. We review papers who address their attention on those malignancies and their treatment. Aim of this work is to discuss the role of radiotherapy especially when helped by new technologies in the management of brain, skin, head and neck, lung, breast, prostate and anal cancer. In our institution we are used to treat such patients by Helical Tomotherapy in order to avoid graft and to limit high risk toxicity. As example, we include a scalp and pelv

Deep vein thrombosis and malignancy: a surgical oncologist's perspective.

Oncology patients are at increased risk of developing deep vein thrombosis (DVT) and its potentially fatal sequel, pulmonary embolism. This is due to multiple factors, including the presence of the malignancy itself, comorbid factors and therapy-related interventions. Issues that are peculiar to venous thrombosis in the oncology setting are discussed, based on a MEDLINE search of the English literature. These include the need to screen for malignancy in idiopathic DVT, a high index of suspicion for venous thrombosis in the cancer patient, the use of vena cava filters, and the anti-neoplastic e

[Diagnosis and treatment of carcinoid meningitis: a challenge to the neurologist and oncologist].

BACKGROUND: Two clinical types of leptomeningeal metastases from solid tumors are observed: local and disseminated. The former (meningeal carcinomatosis) consists in nodular infiltration of leptomeninges, while the latter (carcinomatous meningitis)--in tumor cells free floating in the cerebrospinal fluid and adhering as a monolayer to the surface of neural structures. Despite the same etiology, the two types of metastasis differ in their clinical manifestation and prognosis. Meningeal carcinomatosis is more frequently diagnosed nowadays due to advances in neuroimaging techniques and the long s

Hereditary Risk of Breast and Ovarian Carcinoma: The Role of the Oncologist.

The American Society of Clinical Oncology has affirmed the role of clinical oncologists in identifying and managing patients with familial cancer risk. Inherited mutations in the genes BRCA1 and BRCA2 are responsible for the majority of hereditary breast and ovarian cancers, and these mutations also increase the risk of second cancers in women already diagnosed with breast malignancy. Understanding the likelihood of breast and ovarian cancer associated with mutations in BRCA1 and BRCA2 begins with consideration of the biological basis of hereditary cancer risk. Identifying patients with heredi

Oncologist Perspectives on Timely Hospice Referral: A Qualitative Study.

IntroductionLate hospice referral rates are on the rise and are associated with negative outcomes at the end of life (EoL). Rates of late hospice referral vary drastically from oncologist to oncologist, and behavioral and psychological factors among individual oncologists have been identified as potential contributors to this variability. However, there remains a lack of clarity about how these oncologist-specific factors affect hospice referral practices among oncologists.MethodsThe Communication Optimization and Methodology For Improving Oncologist Hospice Referral Timing (COMFORT) study exp

Identification and stratification of placenta percreta with gynecologic oncologist management.

OBJECTIVE: Gynecologic oncologist involvement in the surgical team of patients with placenta percreta has shown improved patient outcomes. Yet, stratification of cases is dependent on identification of placenta percreta by ultrasonography which has a poor detection rate. To allow patients to receive optimal team management by pre-operative stratification our objective was to identify the pre-operative characteristics of patients with previously underdiagnosed placenta percreta. METHODS: A retrospective single institution case-control study was performed from January 2010 to December 2022 of s

FROM ONCOLOGIST TO SURGEON - GENETICS IN COLORECTAL METASTASIS FOR SURGEONS.

Colorectal cancer (CRC) is a common disease, with incidence in Brazil of 45,630 new cases per 100,000 inhabitants between 2023-2025. Risk factors for CRC can be evaluated between environmental and hereditary and their mode of presentation are classified as sporadic, inherited and familial. Sporadic disease is characterized by the absence of a family history and accounts for approximately 70% of all colorectal cancers, being more common over 50 years of age, with dietary and environmental factors implicated in its pathogenesis. Sporadic disease is characterized by the absence of a family histor

The glymphatic system in oncology: from the perspective of a radiation oncologist.

The brain and spinal cord, which constitute the central nervous system, were historically considered immune-privileged sites, as it was believed they lacked an equivalent to the systemic lymphatic system. However, in 2013, a pathway facilitating the clearance of waste products through the brain parenchyma via the perivascular space was proposed, garnering attention as the 'glymphatic system'. Similar to the systemic lymphatic system, the glymphatic system plays a critical role in immune responses and has been implicated not only in Alzheimer's disease and inflammatory brain disorders but also
