Systematic Approach for Comparing Team-based Care Processes.

Team-based care process modeling techniques have focused on understanding and designing solutions for a single site. Less is known about tailoring an effective team-based care process from one site to another, which is necessary for multi-site implementation efforts. We propose an approach for analyzing and comparing a team-based care process performed at two sites to inform redesign opportunities. Our approach includes abstracting the goals and strategies of each process by identifying whether sociotechnical system element differences exist. Element differences may exist for the phase, tasks,

Taking the septic patient to the operating room.

The acutely septic patient is a multifaceted challenge for the anesthetist. Unlike most elective surgery patients, acutely septic patients have severe systemic disease before the physiologic insults of anesthesia and surgery. The decision to operate is usually informed by the urgent or emergent need to correct a severe surgical problem and weighed against the higher risks of morbidity and mortality from the procedure itself. The care of the septic patient in the intensive care unit can help guide operating room management. However, the acuity and time course of intraoperative events, including

Difficult airway response team: a novel quality improvement program for managing hospital-wide airway emergencies.

BACKGROUND: Difficult airway cases can quickly become emergencies, increasing the risk of life-threatening complications or death. Emergency airway management outside the operating room is particularly challenging. METHODS: We developed a quality improvement program-the Difficult Airway Response Team (DART)-to improve emergency airway management outside the operating room. DART was implemented by a team of anesthesiologists, otolaryngologists, trauma surgeons, emergency medicine physicians, and risk managers in 2005 at The Johns Hopkins Hospital in Baltimore, Maryland. The DART program had 3

Navigation-assisted full-endoscopic spine surgery: a technical note.

BackgroundFull-endoscopic spine surgery (FESS) necessitates the use of X-ray fluoroscopy for intraoperative guidance and orientation. However, the two-dimensional X-ray fluoroscopic images do not provide real-time guidance. The authors developed a new real-time three-dimensional (3D) navigation technique for FESS that entails the use of intraoperative cone beam computed tomography (CBCT) in a hybrid operating room (OR).MethodsA total of 23 patients undergoing FESS using real-time 3D navigation system were enrolled. Preoperative and intraoperative CBCT data were registered in the navigation sys

Neuronavigation using three-dimensional proton magnetic resonance spectroscopy data.

BACKGROUND: Applications in clinical medicine can benefit from fusion of spectroscopy data with anatomical imagery. For example, new 3-dimensional (3D) spectroscopy techniques allow for improved correlation of metabolite profiles with specific regions of interest in anatomical tumor images, which can be useful in characterizing and treating heterogeneous tumors that appear structurally homogeneous. OBJECTIVES: We sought to develop a clinical workflow and uniquely capable custom software tool to integrate advanced 3-tesla 3D proton magnetic resonance spectroscopic imaging ((1)H-MRSI) into indu

Hybrid Surgery for Portosystemic Encephalopathy in a Patient with Liver Cirrhosis: a case report.

Regarding the treatment for a portosystemic shunt, surgical or interventional radiological closure of the shunt was established. Interventional radiology including balloon-occluded retrograde transvenous obliteration can worsen portal hypertension and create a large thrombus close to the major venous system in the case of a huge portosystemic shunt. In contrast, it is also difficult to treat some cases through surgery alone when huge complicated shunts exist very deep in the body. Herein, we report a successful case of surgical shunt ligation for portosystemic encephalopathy in a hybrid operat

Detection of arterial pressure waveform error using machine learning trained algorithms.

In critically ill and high-risk surgical room patients, an invasive arterial catheter is often inserted to continuously measure arterial pressure (AP). The arterial waveform pressure measurement, however, may be compromised by damping or inappropriate reference placement of the pressure transducer. Clinicians, decision support systems, or closed-loop applications that rely on such information would benefit from the ability to detect error from the waveform alone. In the present study we hypothesized that machine-learning trained algorithms could discriminate three types of transducer error fro

Issues in the optimal selection of a cranial nerve monitoring system.

Intraoperative nerve monitoring (IONM) is a safe technique that is of clear clinical value in the preservation of cranial nerves in skull base surgery and is rapidly becoming the standard of care. Available nerve monitoring systems vary widely in capabilities and costs. A well-informed surgeon may best decide on monitoring needs based on surgical case selection, experience, operating room space, availability of monitoring personnel, and cost. Key system characteristics that should be reviewed in the decision-making process include the monitoring technique (electromyography, pressure transducer

Influence of cage material, plating and vertebral bone quality on subsidence and long-term outcomes following anterior cervical discectomy and fusion-a retrospective analysis.

BACKGROUND: Anterior cervical discectomy and fusion (ACDF) is a surgical procedure used to treat degenerative conditions of the cervical spine. Cage subsidence is a known post-operative complication, resulting in loss of indirect decompression, deformity, and radicular symptoms. The objectives of this study are to: (I) investigate associations between cage-plate constructs and postoperative subsidence after single-level ACDF; (II) determine relationships between cervical vertebral bone quality (C-VBQ) scores, subsidence, and fusion construct; and (III) identify potential risk factors that may

Postoperative nausea and vomiting in the post-anesthetic care unit, a 5-year survey of a quality assurance program in surgical cancer patients.

INTRODUCTION: PostOperative Nausea and Vomiting (PONV)is a major side effect related to surgery and anesthesia. Our institution is equipped with Anesthesia Information Management System (AIMS). We used this database to assess and follow the effect of our quality assurance program for PONV. METHODS: Our AIMS system permits automatic storage of vital signs while other information are indexed by anesthesia providers and PACU personnel. Intra-operative and PACU events were extracted from a database by sequential query language (SQL) interrogation from year 2005 to 2010. A new prophylactic antieme

Robotics in reproductive surgery: strengths and limitations.

Minimally invasive surgical techniques are becoming increasingly common in gynecologic surgery. However, traditional laparoscopy can be challenging. A robotic surgical system gives several advantages over traditional laparoscopy and has been incorporated into reproductive gynecological surgeries. The objective of this article is to review recent publications on robotically-assisted laparoscopy for reproductive surgery. Recent clinical research supports robotic surgery as resulting in less post-operative pain, shorter hospital stays, faster return to normal activities, and decreased blood loss.

Beyond pain management: Nurses' role in comprehensive endometriosis care.

Far more complex than merely a single system disorder, endometriosis requires a nuanced, patient-centered approach grounded in multidisciplinary care. Nurses play a vital role in this framework, working collaboratively with gynecologists, pain specialists, surgeons, mental health practitioners, and others in both operating room and clinic settings. Nurses' broad presence across the care spectrum uniquely positions them to address both physical and psychosocial challenges associated with endometriosis, contributing significantly to improved patient-centered outcomes. This article discusses nurs

The contribution of semantic distance knowledge to size constancy in perception and grasping when visual cues are limited.

To achieve a stable perception of object size in spite of variations in viewing distance, our visual system needs to combine retinal image information and distance cues. Previous research has shown that, not only retinal cues, but also extraretinal sensory signals can provide reliable information about depth and that different neural networks (perception versus action) can exhibit preferences in the use of these different sources of information during size-distance computations. Semantic knowledge of distance, a purely cognitive signal, can also provide distance information. Do the perception

High-Efficiency Asymmetric Spin Transport Enabled by Nanocolumn Molecular Semiconductors.

Owing to weak spin-orbit coupling, molecular semiconductors are among the few materials supporting room-temperature spin functionality, yet their low spin-transport efficiency (ηs, ∼5%) limits applications. Here, we report molecular spintronic devices featuring vertically asymmetric nanocolumn channels formed by phase separation. These channels confine spins and generate built-in electric fields, boosting room-temperature ηs to 20%-the highest value reported to date, over five times that of unstructured films. Simultaneously, the nanocolumn channels induce pronounced bias-dependent asymmetry,

Contribution of Coordination Theories to the Determination of Human Factors Associated With Operating Room Perceived Performance.

BACKGROUND: The efficient and fluid organization of surgical interventions in an operating room (OR) and operating suite (OS) is important as these are among the most expensive units to run in medical-surgical facilities. The complexity of OS organization requires careful coordination, defined here as the directing of individuals' efforts toward achieving common and explicitly recognized goals. There is currently sparse literature on OS coordination, especially in the French context. This study aimed to respond to this gap by reporting on the coordination mechanisms associated with the perceiv

Improving Transfusion Safety in the Operating Room With a Barcode Scanning System Designed Specifically for the Surgical Environment and Existing Electronic Medical Record Systems: An Interrupted Time Series Analysis.

BACKGROUND: Manual processes for verifying patient identification before blood transfusion and documenting this pretransfusion safety check are prone to errors, and compliance with manual systems is especially poor in urgent operating room settings. An automated, electronic barcode scanner system would be expected to improve pretransfusion verification and documentation. METHODS: Audits were conducted of blood transfusion documentation under a manual paper system from January to October 2014. An electronic barcode scanning system was developed to streamline transfusion safety checking and aut

Motion sickness and development of synergy within the spatial orientation system. A hypothetical unifying concept.

Adaptation to research paradigms such as rotating rooms and optical alteration of visual feedback during movement results in development of perceptual-motor programs that provide the reflexive assistance that is necessary to skilled control of movement and balance. The discomfort and stomach awareness that occur during the adaptation process has been attributed to conflicting sensory information about the state of motion. Vestibular signals depend on the kinematics of head movements irrespective of the presence or absence of signals from other senses. We propose that sensory conflict when vest
