BACKGROUND: Prophylactic treatment of unruptured small brain aneurysms is still controversial due to the low risk of rupture. Distinguishing which small aneurysms are at risk for rupture has become important for treatment. Previous studies have indicated a variety of hemodynamic properties that may influence aneurysm rupture. This study uses hemodynamic principles to evaluate these in the context of ruptured and unruptured small aneurysms in a single location.
METHODS: Eight small internal carotid artery-ophthalmic artery (ICA-Oph) aneurysms (<10 mm) were selected from the University of Calif

Arterial pressure in the ophthalmic artery was studied in 73 patients (135 eyes) with autoimmune thyroiditis in different thyroid function. A rise of diastolic and systolic pressure in the ophthalmic artery was noted in all groups of patients. It could be accounted for by a high degree of activation of the hypothalamic region as a result of sharp deficiency of thyroid hormones. Raised arterial pressure in the ophthalmic artery in patients with euthyroidism made it possible to recommend this method for early diagnosis of autoimmune thyroiditis.

The poor natural history of central retinal artery occlusion (CRAO) is usually not modified with conventional, conservative management techniques. Guidelines for selective intraarterial ophthalmic thrombolysis are still lacking. While many centers continue to perform this procedure with promising results, others are reluctant due to conflicting findings in recent studies. We present our experience in a 36-year-old male with CRAO. Based on the patient's clinical presentation, we planned to perform selective intraarterial ophthalmic thrombolysis via the ophthalmic artery. When angiography demons

PURPOSE: To investigate and classify the ophthalmic artery (OA) flow patterns in patients with occlusive carotid artery disease (OCAD).
METHODS: Forty-three patients (52 eyes) with documented OCAD of > or =70% underwent orbital color Doppler imaging. The eyes were first divided into four groups by peak systolic velocity in OA (PSV(OA)): group A, PSV(OA) < or = 0; group B, 0 < PSV(OA) < or = 10; group C, 10 < PSV(OA) < or = 40; and group D, PSV(OA) > 40 cm/s, then further classified by the shape of the OA flow wave. The groups were then compared with respect to the collateral pathway (Co-Path)

UNLABELLED: OBJECT.: The benefits of treating intracranial aneurysms in the region of the anterior visual pathways are well understood. However, the adverse effects of endovascular stenting across the ophthalmic artery have received little attention. The authors reviewed their experience with patients who had stents deployed across the ophthalmic artery origin.
METHODS: Patients' medical charts and imaging studies were reviewed to identify all patients with a non-flow diverting stent deployed over the ophthalmic artery origin for the treatment of intracranial aneurysms. All patients with neur

PURPOSE: The ophthalmic artery is often involved in suprasellar and parasellar surgeries, but the anatomical structure where the ophthalmic artery originates has not been fully clarified from the perspective of an endoscopic endonasal approach (EEA).
METHODS: A total of 10 fresh cadaveric heads (20 sides) were dissected through an EEA, and the origin of the bilateral ophthalmic arteries and their adjacent structures were observed from a ventral view. The origin of the ophthalmic artery in 50 healthy people was retrospectively studied on computed tomography angiography imaging.
RESULTS: The o

BACKGROUND: Before the development of the adult ophthalmic artery (OA), the primitive maxillary artery (MA), the primitive dorsal OA, and the primitive ventral OA contribute to the vascularization of early ocular structures, whereas the primitive olfactory artery (OlfA) forms in the vicinity of the optic vesicle. These vessels are involved in several OA origin variants.
OBJECTIVE: To clarify the developmental history of the OA, emphasizing in particular the criteria used to define persistent primitive OAs.
METHODS: Eight rare variants relevant to the discussion of aberrant OA origins are pre

BACKGROUND: There is little documentation of the course and relations of the ophthalmic artery (OA) and posterior ciliary arteries (PCAs).
METHODS: The anatomic characteristics of the OA and PCAs were determined from a dissection of 19 neoprene-injected cadaver heads.
RESULTS: The intraorbital OA had three segments, considering its relation to the optic nerve in the sagittal plane. The first segment extended from the point where the OA entered the orbit to its curving point. The second segment coursed superomedially from the inferolateral part of the optic nerve, crossing the optic nerve eit

Vascular reactivities to vasoactive substances were compared with ophthalmic and ciliary arteries (OAs and CAs) of Japanese monkeys. These two kind of arteries were perfused with Tyrode solution under a constant flow rate at 37 degrees C, respectively. Each drug solution was given by a microinjector into the endothelial side of the artery through a cannulated tubing, and responses were obtained as changes in perfusion pressure. Results were as follows: 1) Norepinephrine induced vasoconstriction in a dose-related manner in both arteries. The threshold dose was approximately 0.01 microgram in OA

AIM: The aim of this paper was to provide an insight on the role of the ophthalmic artery blood flow changes due to significant carotid stenosis and the effects of carotid revascularization on the eye and cerebral circulation.
METHODS: An electronic search (Medline) of the English literature was attempted. Measurements of Peak Systolic Velocity (PSV), end-diastolic velocity (EDV), mean velocity (Vmean), Resistance Index (RI) and flow direction, obtained in OA and its branches using transcranial Doppler, in to patients with significant stenosis >70% subjected to surgical or endovascular treatm

A 72 year old woman complained of transient loss of vision in the left eye. She had undergone a left carotid endarterectomy 10 years previously. Reduced ophthalmic artery pressure was found on noninvasive carotid artery testing and cerebral angiography was performed. No lesion was evident in the carotid artery, but significant ophthalmic artery stenosis was identified. We report this case as showing the occurrence of amaurosis fugax in association with ophthalmic artery stenosis.

Ophthalmic artery occlusion usually presents as a sudden onset profound decrease in vision in the middle-aged and elderly patients following periocular procedures (retrobulbar injection/glabellar fat injection), embolism from the heart or after prolonged systemic surgery. In this report, we describe three children with spontaneous ophthalmic artery occlusion who presented with unilateral loss of vision and diagnosed elsewhere as optic atrophy whose detailed history and examination were suggestive of ophthalmic artery occlusion. Detailed systemic and laboratory evaluation revealed hyperhomocyst

Fenestration of a cranial nerve by an internal carotid artery aneurysm is rarely identified at surgical repair of intracranial aneurysms. Classically this condition is associated with visual disturbance in either acuity, fields or diplopia. We describe a patient with optic nerve fenestration by a carotico-ophthalmic artery aneurysm and discuss its likely pathophysiology. It is important to consider this anatomical variation during microsurgical approaches to the pericarotid and perioptic cisterns to prevent inadvertent damage to the optic apparatus.
