A 67-year old woman presented with isolated and bilateral external ophthalmoplegia of the myasthenic type following a subacute course and resistant to anticholesterinases and corticosteroids. A biological hyperthyroidism with immunological markers was discovered. An intensive care of corticosteroids was partially effective, but fatal bone marrow aplasia attributed to carbimazole developed. Pure ophthalmoplegia without local signs of inflammation may be the sole manifestation of dysthyroid orbitopathy.

The authors begin by enumerating the various syndromes in which painful ophthalmoplegia may be observed (sphenoidal fissure syndrome, Collier's syndrome, syndromes involving the orbital apex, the cavernous sinus and parasellar syndromes; Raeder's syndrome, Gradenigo's syndrome and Fischer-Brugge syndrome). They then discuss the various causes that must be investigated in all cases of painful ophtalmoplegia. They consider in order: -- ophtalmoplegia due to general causes (especially diabetes) and neurological causes (e.g. multiple sclerosis); -- ophtalmoplegia due to common local canses space-o…

The clinical features and investigative findings of 50 patients with chronic progressive external ophthalmoplegia (CPEO) were analysed. The group was found to be clinically, genetically and histologically heterogeneous. With the possible exception of patients with "ophthalmoplegia plus," namely those who in addition to muscular weakness had evidence of central and/or peripheral nervous system abnormality, there was no apparent justification for separating out from among the group patients' subgroups which were distinctive enough to be recognized as syndromes. CPEO therefore seems to represent …

Two patients presented with an acute syndrome of internal ophthalmoplegia in the absence of external ophthalmoplegia, bilateral involvement of other cranial nerves, and minimal evidence of peripheral neuropathy. Cerebrospinal fluid protein was slightly raised, and moderate slowing of nerve conduction velocity was observed peripherally. It is suggested that these cases may represent a variant of acute post-infectious polyneuritis.

Optokinetic nystagmus in panoramic mono- and binocular optokinetic stimulation was studied in 24 patients with multiple sclerosis and 20 normal subjects. In control group, asymmetry coefficients and slow phase speed were less than 6% and 7.7% respectively. In the patients, these indices were 7.1 and 14% for binocular optokinetic stimulation and 14.6 and 21.7% for monocular one. Using monocular optokinetic stimulation, electronistagmographic signs of internuclear ophthalmoplegia were detected in all the patients. Therefore, panoramic monocular optokinetic stimulation facilitates determination o…

PURPOSE: To present a case of improvement of ocular motility in a patient with chronic progressive external ophthalmoplegia (CPEO) with Coenzyme Q10.
METHODS: Coenzyme Q10 300 mg daily was given for three years with a three-day trial period of 200 mg daily after one year. Ocular ductions were measured by synoptophore.
RESULTS: Ocular ductions improved with treatment with Coenzyme Q10.
CONCLUSION: Coenzyme Q10, is effective in limiting the severity of ophthalmoplegia in this case.

A case of transient complete bilateral paresis of abduction is reported without esotropia. Loss of vestibular ocular reflex (VOR) responses of the abducting eye and intactness of all other slow VOR reactions were the leading symptoms. Since bilateral lesions of the pontine gaze centers in this initially comatose patient without signs of increased intracranial pressure were excluded, bilateral lesions of the abducens nerves appeared improbable. In view of normal function of the ascending tract of Deiters bilateral paramedian lesions between the vestibular and abducens nuclei appeared to be more

Chronic progressive external ophthalmoplegia (CPEO) is a descriptive term for a heterogenous group of disorders characterized by chronic, progressive, bilateral, and usually symmetric ocular motility deficit and ptosis. Significant pain, proptosis, or pupil involvement are not features of CPEO and should prompt evaluation for alternative etiologies. Mitochondrial DNA mutations are increasingly being recognized as the etiology for CPEO syndromes. Clinicians should recognize the specific syndromes associated with CPEO, characterized by variable systemic, neurologic, or other findings. Treatment …

Two cases of chronic progressive external ophthalmoplegia were described. Both of them presented with progressive bilateral ptosis and gradual impairment of ocular mobility. One of the patients had abnormal cerebrospinal fluid protein level. Another patient had muscle biopsy which was compatible with mitochondrial myopathy. Other possible causes of chronic progressive external ophthalmoplegia had been excluded by appropriate investigations. Chronic progressive external ophthalmoplegia is now considered as one type of mitochondrial diseases. Missed diagnosis of this syndrome is common in clinic…

66-year-old female patient presents palpebral bilateral ptosis and external ophthalmoplegia with a progressive evolution. The clinical aspects are completed with the affectation of the sensitive and motor trigemen, with the masseter atrophy and bilateral trophic keratitis. The affectation of the facial and acoustic nerves was associated with bilateral perception surditate. The hereditary transmission was dominant, with incomplete penetration. The neurogenic pathogeny was sustained on the basis of the association of neurological multilesions with III, IV, V, VI, VII, VIII nerves, with progressi…
