Ischemic optic neuropathy. Still the ophthalmologist's dilemma.

Ischemic optic neuropathy, in its arteritic and idiopathic varieties, presents both diagnostic and therapeutic challenges to the clinician, specifically: the role of temporal artery biopsy in management of giant cell arteritis; the preferred treatment regimen for giant cell arteritis, including decisions to begin tapering therapy and stopping therapy; action to take if major steroid complications arise while the disease is still active; distinguishing anterior ischemic optic neuropathy from idiopathic optic neuritis; whether cerebral arteriography is indicated in the assessment of idiopathic i

Posterior ischemic optic neuropathy. III. Clinical diagnosis.

14 cases of posterior ischemic optic neuropathy (PION) were clinically analyzed, in whom we excluded known etiologies of optic nerve disturbances and confirmed the decreased blood supply to the posterior portion of the optic nerve. On the basis of our clinical findings, we have proposed the following criteria for the diagnosis of idiopathic PION: (1) sudden onset of unilateral visual disturbance in older patients; (2) normal optic disc, subsequently developing simple optic atrophy; (3) hypertensive and arteriosclerotic changes in the retinal vessels; (4) varying degrees of impaired vision, var

Ischemic optic neuropathy in cavernous sinus thrombosis.

Visual loss is uncommon in thrombosis of the cavernous sinus. A patient observed closely in the acute stages developed high introcular and intraorbital pressures. Although the retinal vasculature remained patent, total unilateral blindness occurred within 48 hours. To our knowledge this is the first documented case in which ischemic optic neuropathy caused blindness in a patient suffering from cavernous sinus thrombosis.

Posterior ischemic optic neuropathy: Perioperative risk factors.

Perioperative posterior ischemic optic neuropathy (PION) is a rare but devastating condition. Visual impairment is commonly bilateral, profound, and irreversible. The most frequently associated triggering events are spine surgeries, other orthopedic surgeries, cardiac bypass surgeries, and radical neck dissection. The etiology is multifactorial. The most commonly reported risk factors are severe and prolonged hypotension, anemia, hemodilution, orbital and periorbital edema, direct orbital compression by prone position, and abnormal autoregulation. This review discusses the current literature o

Systemic corticosteroids in nonarteritic ischemic optic neuropathy.

Nonarteritic ischemic optic neuropathy (NAION) is one of the most prevalent optic nerve disorders seen in ophthalmic practice. The role of corticosteroid therapy in NAION remains a highly controversial area of debate in ophthalmology. This brief review will provide an overview of the current clinical evidence on this topic as well as some comment on the medical debate.

Anterior ischemic optic neuropathy associated with udenafil.

We report a case of anterior ischemic optic neuropathy associated with udenafil. A 54-year-old male presented with an acute onset visual field defect of the right eye after udenafil use. Examination revealed a relative afferent pupillary defect and a swollen disc. Automated visual fields revealed an enlarged blind spot and a narrowed visual field. Fluorescein angiography revealed both an inferior choroidal filling delay and an inferior sector filling delay of the optic disc in the arteriovenous phase as well as diffuse leakage of the optic disc in the late phase. Optical coherent tomography re

Surgery for nonarteritic anterior ischemic optic neuropathy.

BACKGROUND: Nonarteritic ischemic optic neuropathy (NAION) is characterized by sudden and painless loss of vision in the eye, accompanied by pallid swelling of the optic disc. Its etiology is unknown and no medical therapy has been proven effective in treating this condition. Optic nerve decompression surgery, a proposed treatment for NAION, involves making two or more slits or a window in the tissue surrounding the optic nerve, thereby allowing cerebrospinal fluid to escape, and theoretically reducing the pressure surrounding the optic nerve. OBJECTIVES: The objective of this review was to a

Treatment of nonarteritic anterior ischemic optic neuropathy.

Nonarteritic anterior ischemic optic neuropathy (NAION) is the most common clinical presentation of acute ischemic damage to the optic nerve. Most treatments proposed for NAION are empirical and include a wide range of agents presumed to act on thrombosis, on the blood vessels, or on the disk edema itself. Others are presumed to have a neuroprotective effect. Although there have been multiple therapies attempted, most have not been adequately studied, and animal models of NAION have only recently emerged. The Ischemic Optic Neuropathy Decompression Trial, the only class I large multicenter pro

Two presentations of nonarteritic ischemic optic neuropathy.

BACKGROUND: Nonarteritic ischemic optic neuropathy (NAION) is the most common acute optic neuropathy in adults over the age of 50. Patients affected notice sudden and painless loss of vision in 1 eye, often upon awaking. Studies have found that the opposite eye may be affected in approximately 15% to 20% of cases within a 5-year period. NAION vision loss results from an ischemic event often affecting the short posterior ciliary arteries. This results in optic nerve pallor, nerve fiber layer defects, and corresponding visual field defects. CASE REPORTS: Two cases of NAION are discussed here. T

Bilateral anterior ischemic optic neuropathy after liposuction.

A 30-year-old woman had bilateral anterior ischemic optic neuropathy after undergoing large-volume liposuction. Visual function remained stable over a four-month follow-up, with decreased visual acuity and marked constriction of the visual fields. To our knowledge, this is the second reported case of ischemic optic neuropathy in this setting.

Ischemic optic neuropathy after lumbar spine surgery.

OBJECTIVE: Study of clinical features of ischemic optic neuropathy (ION) developing as a complication of multilevel lumbar spine surgery. DESIGN: Review of all cases of ION that developed within 2 weeks of spine surgery at two academic institutions from 1990 to 1992, and a review of adequately reported cases of ION after other non-ophthalmic procedures. RESULTS: Four new cases are reported in patients who ranged in age from 41 to 65 years. All four had undergone uneventful but prolonged (8 to 9 hours) spine surgery, during which blood pressure was deliberately maintained between 85 and 100 m

Posterior ischemic optic neuropathy during general surgery.

We examined two patients who awoke with profound bilateral visual loss after operations under general anesthesia. Their fundi, initially normal, later showed bilateral optic atrophy. Neither patient showed other neurologic deficits, although one demonstrated radiologic evidence of a small cerebral infarction in the deep white matter. These patients probably suffered intraoperative infarction of the retrobulbar segments of both optic nerves, producing posterior ischemic optic neuropathy. Profound systemic hypotension may have been a contributing factor in one patient, the use of the pump-oxygen

Anterior ischemic optic neuropathy of the young.

Anterior ischemic optic neuropathy is a well-recognized syndrome usually occurring in an older adult population. It is unusual for these patients to experience a second attack in an eye already affected. We describe three young patients with anterior ischemic optic neuropathy who have experienced repeat attacks in one or both eyes, leaving the patients severely disabled, without evidence of other ocular or systemic disease. We believe they may represent a new, but rare, syndrome.

Ischemic optic neuropathy associated with Crohn's disease.

A 24-year-old man with Crohn's disease suffered bilateral ischemic optic neuropathy following the resolution of an episode of "uveitis." Additional findings of corneal infiltrates, vitritis, retinovascular sheathing, and arthritis suggest an inflammatory basis for his ischemic optic neuropathy. Vasculitis as an extraintestinal manifestation of inflammatory bowel disease is a possible etiology. To our knowledge, the association of ischemic optic neuropathy and inflammatory bowel disease has not been previously reported.

[Posterior acute ischemic optic neuropathy (author's transl)].

Posterior acute ischemic neuropathy is a new and distinct clinical entity. This syndrome associate unilateral visual disturbance with arcuate or altitudinal defect. Ocular ophthalmoscopic examination and fluorescein fundus angiography are normal at the onset of the disease. Later develop varied degrees of optic atrophy. Association with anterior optic acute ischemic neuropathy on the other eye is possible. Systemic lupus erythematosus, periarteritis nodosa, temporal arteritis, and arteriosclerosis are often involved. Study of three personal cases. Discussion on pathogenesis.
