Pituitary and Optic Tract Lesions Masquerading As Glaucoma: Highlighting Diagnostic Challenges.

Purpose This study's purpose is to highlight the diagnostic pitfalls in distinguishing glaucomatous optic neuropathy from compressive optic neuropathies caused by intracranial lesions, particularly pituitary macroadenomas. Methodology This research was a retrospective case series of six patients who were originally diagnosed or suspected to have glaucomatous optic neuropathy but later revealed intracranial lesions on neuroimaging. Results All patients had optic disc cupping and/or pallor with visual field defects (VFDs) overlapping with glaucomatous patterns. However, atypical features such as

Isolated optic nerve, chiasm, and tract involvement in Bing-Neel Syndrome.

Central nervous system infiltration of Waldenström's macroglobulinemia is referred to as Bing-Neel Syndrome. We describe 2 patients whose clinical presentation was due to isolated involvement of the anterior visual pathways. The mechanism of visual failure in Bing-Neel Syndrome may involve both infiltrative and autoimmune processes.

"Twin peaks" papilledema: the appearance of papilledema with optic tract atrophy.

A woman with a right middle fossa meningioma causing right optic tract atrophy and papilledema had distinctive funduscopic changes. The ipsilateral eye showed temporal disc pallor and nasal edema. The contralateral disc showed edema of the upper and lower poles which was separated by a horizontal band of optic atrophy. The fluorescein angiographic changes clearly illustrate this pattern. Apart from its theoretical interest this appearance of papilledema and atrophy provides definite localizing information.

Optic tract injury after closed head traumatic brain injury in mice: A model of indirect traumatic optic neuropathy.

Adult male C57BL/6J mice have previously been reported to have motor and memory deficits after experimental closed head traumatic brain injury (TBI), without associated gross pathologic damage or neuroimaging changes detectable by magnetic resonance imaging or diffusion tensor imaging protocols. The presence of neurologic deficits, however, suggests neural damage or dysfunction in these animals. Accordingly, we undertook a histologic analysis of mice after TBI. Gross pathology and histologic analysis using Nissl stain and NeuN immunohistochemistry demonstrated no obvious tissue damage or neuro

Callosal and superior temporal sulcus contributions to receptive field properties in the macaque monkey's nucleus of the optic tract and dorsal terminal nucleus of the accessory optic tract.

To assess the functional contribution of the cortical input to the receptive field properties of nucleus of the optic tract (NOT) and dorsal terminal nucleus (DTN) neurons, a first set of experiments evaluated the response properties of NOT-DTN cells in monkeys with split corpus callosum. With respect to visual latency, direction specificity, directional tuning width, velocity tuning, ocular dominance, and binocular interaction, they were indistinguishable from NOT-DTN neurons in normal monkeys. However, a clear difference was found regarding the extent of the receptive fields. Whereas, in nor

Assessment of optic nerve and optic tract alterations in patients with orbital space-occupying lesions using probabilistic diffusion tractography.

AIM: To investigate the diffusion changes in both the optic nerve and optic tract in orbital space-occupying lesion patients with decreased visual acuity, and its clinical significance using probabilistic diffusion tractography (PDT). METHODS: Twenty patients with orbital space-occupying lesions and 25 age- and gender-matched healthy persons were included. All patients and controls underwent routine orbital magnetic resonance imaging and diffusion tensor imaging (DTI), using a 3.0T magnetic resonance scanner (Trio Tim Siemens). After the image data were preprocessed, each DTI parameters of th

COVID-19-Related Rhino-Orbito-Cerebral Mucormycosis Complicated by the Optic Nerve and Optic Tract Ischemia With Ischemic Neuropathy.

Mucormycosis is a life-threatening invasive fungal infection usually seen in immunocompromised patients and patients with poorly controlled diabetes mellitus with or without diabetic ketoacidosis. We present a rhino-orbito-cerebral mucormycosis (ROCM) case in a coronavirus disease 2019 (COVID-19) patient complicated by the optic nerve and optic tract ischemia with ischemic neuropathy. Both CT and MRI played an essential role in diagnosing ROCM and the accompanying complications in our patient. CT showed sinonasal sinusitis and MRI showed the sinusitis and its progression to ROCM. MRI also show

Tailored TAPS via the Medial Optic-Tract Border for a Thalamomesencephalic Cavernoma.

Thalamomesencephalic cavernous malformations are rare deep lesions; anterior access is constrained by the optic apparatus, internal carotid artery, hypothalamus, oculomotor nerve, major white-matter pathways, and perforating vessels. We present a 51-year-old woman with a 25 × 27 × 31-mm hemorrhagic right thalamomesencephalic cavernous malformation, recurrent neurological exacerbations, left-sided weakness, and a left homonymous visual field defect. Through a right orbito-pterional craniotomy, the translamina terminalis, prechiasmatic, opticocarotid, and carotid-oculomotor corridors were sequen

MRI of optic tract lesions: review and correlation with visual field defects.

Visual field defects are a conglomerate of patterns of visual impairment derived from diseases affecting the optic nerve as it extends from the globe to the visual cortex. They are complex signs requiring perimetry or visual confrontation for delineation and are associated with diverse aetiologies. This review considers the chiasmatic and post-chiasmatic causes of visual disturbances, with an emphasis on magnetic resonance imaging (MRI) techniques. Newer MRI sequences are considered, such as diffusion-tensor imaging. MRI images are correlated with perimetric findings in order to demonstrate lo

Remote effect of optic tract trauma in the occipital lobe (case report).

Traumatic brain damage can result in severe visual impairments including hemianopia. Lesions correlated with hemianopia can be located in any part of the retrochiasmatic optic pathway. However, traumatic lesions of the optic tract are relatively rare. We present a case with posttraumatic left homonymous hemianopia who had signal intensity change at the ipsilateral optic tract on MR imaging and ipsilateral occipital hypoperfusion on SPECT imaging.

Optic Tract Edema on MRI in Spontaneous Intracranial Hypotension with Sagging Brain.

Spontaneous intracranial hypotension is caused by a spinal cerebrospinal fluid leak. It is typically associated with orthostatic headache and a broad range of additional neurological symptoms. Brain MRI features include pachymeningeal enhancement, subdural collections, pituitary enlargement, venous sinus engorgement and downward displacement of the brain. Ocular involvement has been described as optic neuropathy and changes in the optic nerve sheath fluid, but signal changes in the optic tracts have not been systematically reported. We retrospectively reviewed our patients with spontaneous int

Optic tract edema in craniopharyngioma as a predictor of BRAFV600E mutation presence.

OBJECTIVE: the advent of BRAF inhibitors for preoperative treatment of craniopharyngioma has necessitated the identification of BRAFV600E status. Hence, we investigated predictors of BRAFV600E mutation in craniopharyngiomas. METHODS: this retrospective study utilized data from 30 patients who were newly diagnosed with craniopharyngioma between 2011 and 2021. Magnetic resonance imaging (MRI) and computed tomography were performed within 1 week prior to surgery. Genetic analysis for BRAF mutation was performed using the Oncomine next-generation sequencing panel or Sanger sequencing. The relatio

Congenital optic tract syndrome: magnetic resonance imaging and scanning laser ophthalmoscopy findings.

Lesions of the optic tract produce a distinctive pattern of optic atrophy and visual field loss and may be due to either congenital or acquired causes. We report a case of a congenital optic tract syndrome and correlate the magnetic resonance imaging findings with the appearance of nerve fiber layer defects found by confocal scanning laser ophthalmoscopy.

Impaired Direction Selectivity in the Nucleus of the Optic Tract of Albino Mice.

PURPOSE: Human albinos have a low visual acuity. This is partially due to the presence of spontaneous erroneous eye movements called pendular nystagmus. This nystagmus is present in other albino vertebrates and has been hypothesized to be caused by aberrant wiring of retinal ganglion axons to the nucleus of the optic tract (NOT), a part of the accessory optic system involved in the optokinetic response to visual motion. The NOT in pigmented rodents is preferentially responsive to ipsiversive motion (i.e., motion in the contralateral visual field in the temporonasal direction). We compared the

Dimensions of the optic nerves, chiasm, and tracts: MR quantitative comparison between patients with optic atrophy and normals.

OBJECTIVE: The dimensions of the optic nerves, chiasm, and tracts were determined in normal patients and in patients with clinical evidence of optic atrophy to establish whether there was a significant difference between the two groups. MATERIALS AND METHODS: The authors analyzed thin section coronal MRI of 110 patients. Seventy-seven patients were normal, 20 had visual impairment without funduscopic evidence of optic atrophy, and 13 had visual impairment with funduscopic evidence of optic atrophy. RESULTS: The normal mean dimensions were (height x width): optic tracts 2.8 x 5.1 mm; nerves 3

Bilateral visual failure with optic tract involvement in sellar-suprasellar lymphoma: illustrative case.

BACKGROUND: Visual dysfunction related to sellar lesions is usually attributed to chronic compression of the optic chiasm, typically following an insidious, progressive course. Abrupt or disproportionately rapid visual failure should therefore prompt careful evaluation for alternative or additional mechanisms. OBSERVATIONS: A 69-year-old woman experienced progressive visual deterioration over 3 months, followed by near-complete bilateral visual loss within 1 week. Visual field testing showed severe bilateral generalized depression. Dedicated sellar enhanced MRI revealed a large sellar-suprase
