AI-Assisted Detection of Macular OCT Abnormalities by Optometrists: A Retrospective Reader Study.

BACKGROUND: Optical coherence tomography (OCT) is a key imaging modality for diagnosing retinal disease, but accurate interpretation may require specialist expertise. Artificial intelligence (AI)-assisted OCT has the potential to enhance detection of retinal abnormalities in clinical practice. However, evidence on how AI decision-support tools influence optometrist diagnostic performance in real-world OCT interpretation remains limited. OBJECTIVE: This study aimed to evaluate the impact of an AI-integrated OCT interpretation tool, CIRRUS® PathFinder™ (PF), on optometrists' ability to detect r

Providing capacity in glaucoma care using trained and accredited optometrists: A qualitative evaluation.

INTRODUCTION: The role of optometrists in glaucoma within primary and secondary care has been well described. Whilst many studies examined safety and clinical effectiveness, there is a paucity of qualitative research evaluating enablers and barriers for optometrists delivering glaucoma care. The aims of this study are to investigate qualitatively, and from a multi-stakeholder perspective whether optometric glaucoma care is accepted as an effective alternative to traditional models and what contextual factors impact upon their success. METHODS: Patients were recruited from clinics at Mancheste

Management of ocular adverse reactions to antiglaucoma medications-Survey of optometrists in Ghana.

Glaucoma, characterized by chronic and progressive degeneration of retinal ganglion cells leading to a gradual decline in the visual field, poses a significant challenge in eye care delivery. Optometrists play a vital role in detecting and managing glaucoma. This study aims to determine how Ghanaian optometrists manage ocular adverse reactions (OADRs) in patients using antiglaucoma medications. A cross-sectional survey was conducted among 139 licensed optometrists using a pre-tested, semi-structured questionnaire. Data collected included socio-demographic characteristics, reported adverse reac

Clinical indicators for diabetic eyecare delivered by optometrists in Australia: a Delphi study.

CLINICAL RELEVANCE: Valid and updated clinical indicators can serve as important tools in assessing and improving eyecare delivery. BACKGROUND: Indicators for diabetic eyecare in Australia were previously developed from guidelines published before 2013 and then used to assess the appropriateness of care delivery through a nationwide patient record card audit (the iCareTrack study). To reflect emerging evidence and contemporary practice, this study aimed to update clinical indicators for optometric care for people with type 2 diabetes in Australia. METHODS: Forty-five candidate indicators, in

Referrals from community optometrists in England and their replies: A mixed methods study.

PURPOSE: Community optometrists, through routine eye examinations, identify patients with disease or ocular abnormalities requiring referral to the Hospital Eye Service. In many cases no reply to the referral letter is received, resulting in some patients being re-referred unnecessarily, potentially increasing the number of other patients who lose sight whilst on hospital waiting lists. This study investigated, qualitatively and quantitatively, factors influencing optometric referrals and replies. METHODS: The three-phase, sequential mixed methods study started with a literature review and qu

Comparison of Outcomes of Laser Trabeculoplasty Performed by Optometrists vs Ophthalmologists in Oklahoma.

IMPORTANCE: Oklahoma is one of the few states where optometrists have surgical privileges to perform laser trabeculoplasty (LTP). Optometrists in other states are lobbying to obtain privileges to perform LTP and other laser procedures. Little is known whether outcomes of patients undergoing this procedure by optometrists are similar to those undergoing LTP by ophthalmologists. OBJECTIVE: To compare outcomes of LTPs performed by ophthalmologists with those performed by optometrists to determine whether differences exist in the need for additional LTPs. DESIGN, SETTING, AND PARTICIPANTS: This

Optometrists referrals for glaucoma assessment: a prospective survey of clinical data and outcomes.

AIM: The aim of this study is to determine the outcomes following referral for glaucoma from routine optometric practice and the positive predictive value (PPV). METHODS: A prospective study of 441 referrals for glaucoma in the Portsmouth area was performed over 6 months. A positive outcome was defined when the patient had the diagnosis of glaucoma made or if there was a high index of suspicion of glaucoma requiring follow-up. The PPV was determined from positive outcome number/referral number. RESULTS: The overall PPV was 0.37 (95% confidence interval 0.33-0.42). Open-angle glaucoma (OAG) w

Decision making in chronic glaucoma--optometrists vs ophthalmologists in a shared care service.

PURPOSE: Shared care schemes have become popular in glaucoma management in the UK over the past 20 years. Published audits of decision making processes by non-ophthalmologists working in such schemes are however rare. Our aim was to audit the appropriateness of optometrists' decision-making when working in an in-house shared care glaucoma scheme. METHODS: A single masked comparison study utilising retrospective consensus decisions by two ophthalmologists compared with 'live' optometrist decisions from three optometrists who reviewed patients with glaucoma, suspect glaucoma and ocular hyperten

Three-year outcomes of an optometrist-led virtual clinic for new glaucoma referrals.

INTRODUCTION: The purpose of this study was to describe and evaluate the outcomes of an optometrist-led virtual glaucoma clinic (VGC). METHODS: New patients referred to the glaucoma service who were consultant triaged as 'low risk' were assessed virtually by specialist-trained optometrists in the VGC and either discharged or monitored for a period of 3 years. Ten percent of virtual case notes were audited by a glaucoma consultant to verify quality and generate learning objectives. Retrospective case-note review and analysis of all patients seen in the virtual clinic between 2014 and 2016 was

Investigating target refraction advice provided to cataract surgery patients by UK optometrists and ophthalmologists.

PURPOSE: To determine whether UK optometrists and ophthalmologists provide target refraction advice to patients prior to cataract surgery, and when this should first be discussed. METHODS: Optometrists and ophthalmologists were asked to complete a survey of two clinical vignettes (both older patients with cataract; a pre-operative myope who routinely read without glasses and a patient using a monovision approach), plus multiple choice and short answer questions either using hard copy or online. RESULTS: Responses were obtained from 437 optometrists and 50 ophthalmologists. Optometrists who r
