Use of a titanium device in lateral sinus floor elevation: an experimental study in monkeys.

AIM: To evaluate the effect of a space-maintaining device fixed to the lateral wall of the maxillary sinus after the elevation of the sinus mucosa on bone filling of the sinus cavity. MATERIAL AND METHODS: Immediately after the elevation of the maxillary sinus Schneiderian membrane accomplished through lateral antrostomy in four monkeys, a titanium device was affixed to the lateral sinus wall protruding into the sinus cavity to maintain the mucosa elevated without the use of grafting material. The healing of the tissue around the implants was evaluated after 3 and 6 months. Ground sections we

Surgical considerations for the prosthodontic patient.

Surgical procedures for the prosthodontic patient have advanced greatly over the last decade. Both the aesthetics and function of the final restoration can be greatly enhanced by various surgical procedures. Some are very predictable, and others are still in the developmental stages. This review is divided into three basic categories: aesthetic root coverage; crown lengthening; and ridge augmentation procedures.

Prefabrication of combined scapula flaps for microsurgical reconstruction in oro-maxillofacial defects: a new method.

Nowadays, in congenital or acquired large oro-maxillofacial defects, microsurgical reconstruction is mainly performed by revascularized osseous, osteocutaneous, or osteomyocutaneous distant flaps. The aims of reconstruction include not only restoration of stability and aesthetic contour, but also the restoration of a functioning 'chewing organ'. In addition to bulkiness of the flaps, the stepwise surgical procedure (microvascular reconstruction, osseointegration of implants, secondary correction of flaps including preprosthetic surgery, etc.) prevents physiological oral function for a long tim

Use of fresh frozen bone graft in rehabilitation of maxillar atrophy.

AIM: Fresh frozen bone (FFB) is homologous bone obtained from human donors, provided by Tissue Banks. It is a graft material in reconstructive surgery; it is currently and widely used in orthopedic surgery and lately it has been introduced in oral and maxillofacial surgery. The purpose of this work was to show the use of fresh frozen homologous bone for bony augmentation of the maxilla and mandible in preparation for dental reconstruction with endosseous implants, as an effective alternative to harvesting and grafting autogenous bone from intra- or extra-oral donor sites. METHODS: The case pr

Pre-prosthetic schuchardt osteotomy with sinus floor elevation.

Premature loss of teeth leads to an inevitable egression of opposing teeth. Schuchardt osteotomy is a quick and efficient alternative to create a prosthetic space compatible with prosthetic rehabilitation. During the surgical procedure, it is difficult, if not impossible, to preserve the integrity of the sinus membrane and thus to proceed at the same time with a bone graft in the adjacent sub-sinus area to address a vertical alveolar deficiency. In this technical note, the author describes a new technique by combining a sinus suspension during a Schuchardt osteotomy. This innovative approach a

Patient screening and medical evaluation for implant and preprosthetic surgery.

Implant and preprosthetic surgeries aim to restore normal anatomic contours, function, comfort, aesthetics, and oral health. As such, they are not life-saving procedures. The prime concern must therefore be to not undermine the patient's overall health and safety. It is then that every step must be taken to select the appropriate treatment plan and maximize the longevity of the implanted system, including the overlying prostheses. One important category into which a number of possible complications may fall is the inadequate systemic screening of patients prior to implant and biomaterial inser

Benefit of the replaceable bony window in lateral maxillary sinus augmentation: clinical and histologic study.

PURPOSE: This retrospective study demonstrates, by clinical and histologic evaluation, the benefit of the replaceable bony window as a homologous bony barrier in lateral sinus augmentation. MATERIALS AND METHODS: A total of 103 maxillary sinus augmentations were performed using a piezoelectric device and a Er,Cr:YSGG laser apparatus. The detached bony windows were repositioned over various bone grafts. Sixteen bone biopsy specimens were harvested at the time of uncovering or implant placement. RESULTS: Three sinuses developed postoperative infections (2.91%). After 6 to 8 months of healing,

Maxillary sinus membrane repair: update on technique for large and complete perforations.

Maxillary sinus membrane perforation is the most common complication that occurs with sinus elevation augmentation surgery. A technique using a slow resorbing type I collagen membrane for repair of large and complete sinus membrane perforations is described. The biocompatibility and semirigid structural integrity of this membrane, along with external tack fixation, allows for optimal membrane stabilization and maintenance.

[Reduction of the alveolar ridge: causes, sequelae and prevention].

The alveolar ridge (of upper and lower jaw) serves as a supporting tissue for the teeth. This function disappears after natural dentition has been extracted. Even so, the alveolar ridge remains very important, serving as it does to support and give retention to dentures. However, after removal of the natural dentition alveolar ridge reduction occurs, initially at a fairly rapid rate, slowing down later, but in an ever-continuing process, which, particularly in the long run, may have unpleasant consequences. As many factors may affect this reduction process unfavourably, it is very difficult to

Minimally invasive technique for transcrestal sinus floor elevation: a case report.

The present report describes a minimally invasive technique for transcrestal sinus floor elevation, namely the Smart Lift technique, which is based on the controlled use of drills and osteotomes. The rationale of this technique is to provide predictable vertical bone augmentation into the sinus cavity by (1) using the existing residual bone as a viable graft and (2) limiting the incidence of membrane perforation and postsurgery morbidity. Surgical steps of the procedure are thoroughly illustrated by means of a paradigmatic case. The Smart Lift procedure was compatible with sinus floor elevatio

Summers' technique modification for sinus floor elevation using a connective tissue graft. A case report.

The sinus floor elevation technique with Summers' osteotome is simpler and less invasive than access through the lateral wall of the sinus; however, it is susceptible to a higher level of sinusal membrane rupture. The objective of the present study was to display a modification of the osteotomy technique by connective tissue interposition in order to weaken the impact of sinus cortical fracture, and therefore prevent perforations in the sinusal membrane. A case is reported in which a patient presented with an absent tooth #4 for osseointegrated implant rehabilitation. Maxillary sinus elevation

Sinus graft complications. Problem solving.

Restoration of the posterior maxilla involving sinus bone grafting demonstrates both surgical and prosthetic problems that treating clinicians should recognize and explain to patients prior to any invasive treatment. If a sinus graft is lost, the alternatives to treatment are explained. However, if the practitioners follow a step-by-step protocol and use retrievable prostheses, a long-term, favorable prognosis results.

[Mastoid bone graft in implantology. A case report].

INTRODUCTION: In implantology, autogenous bone remains currently a useful-material to rebuild horizontal and vertical osseous defects. Usually, it is the needed osseous volume that indicates the grafting procedure, intraoral one for small and middle volume and extraoral one for severely atrophied jaws. The originality of this case report is to show an alternative to usual grafting procedure borrowed from ENT surgeons: osseous mastoid graft. TECHNICAL NOTE: A 63-year-old patient with maxillary sinus pathology underwent during same surgical time maxillary sinus curetting and osseous grafting to

Maxillary reconstruction and placement of dental implants after treatment of a maxillary sinus fungus ball.

A fungus ball is one of the fungal diseases that can affect the paranasal sinuses. It requires surgical treatment. Because there is only one previously reported case of dental implant placement after treatment of a maxillary sinus fungus ball, the authors here report on a case of a maxillary sinus fungus ball with bone erosion that was treated surgically with a combined endoscopic endonasal and endoral (Caldwell-Luc) approach. One year later, a graft from the ilium was obtained and a sinus elevation was performed to allow the placement of dental implants. Three months later, the dental implant

Use of negative air pressure by nasal suction during maxillary sinus floor lift: audit of 13 consecutive sinus grafts.

A common and serious intraoperative complication of sinus floor lift is perforation of the sinus lining. Several strategies to prevent or treat it have had varying results. We report the results of an audit of 13 consecutive sinus grafts in 11 patients in which nasal suction was used to facilitate raising the sinus lining, and to reduce the risk of perforation.

Use of porous hydroxylapatite blocks for augmentation of atrophic mandibles.

A clinical study of the use of porous blocks of hydroxylapatite (Interpore 200) for augmentation of atrophic residual mandibular ridges was performed. Fifteen patients each had three preoperatively customized blocks placed. They were then examined clinically and radiographically for at least 2 years. Six patients had skin-graft vestibuloplasties performed over the blocks 3 months after implantation; dentures were made for 11 of the patients. No clinical or radiographic evidence of migration or resorption of the blocks was found; however, all 15 patients suffered complications. Eleven developed

The role of Nasoalveolar molding: A 3D Prospective analysis.

Nasoalveolar molding (NAM) is commonly employed to reduce the alveolar segments into proper alignment and to improve nasal symmetry in patients with cleft lip and palate. This study examines the periodical progression of NAM treatment over time. 20 patients with complete unilateral cleft lip and palate were prospectively recruited. A 2 stage NAM treatment protocol was applied. Stage 1 involved adjustment of the alveolar segments (mean age 15.6 days), while Stage 2 added nasal stents and started average 43 days after stage 1. 3D images (n = 241) were obtained prior to NAM initiation and weekly
