Ventilatory motion management strategies in conscious individuals for improved radiotherapy and diagnostic imaging: an evidence map.

BACKGROUND: Respiration-induced motion hampers accurate radiotherapy, and diagnostic image quality. Ventilatory motion management strategies (VMMS, i.e. continuous positive airway pressure, jet ventilation, non-invasive mechanical ventilation, percussive ventilation and high-flow nasal oxygen) have emerged as novel respiratory motion management strategies. This evidence map aims to summarise these approaches of non-invasive respiratory motion-control to assess feasibility, comfort, potential benefits for radiotherapy and diagnostic imaging, and the identification of current research gaps. MRI-compatible abdomen phantom to mimic respiratory-triggered organ movement while performing needle-based interventions.

PURPOSE: In vivo studies are often required to prove the functionality and safety of medical devices. Clinical trials are costly and complex, adding to ethical scrutiny of animal testing. Anthropomorphic phantoms with versatile functionalities can overcome these issues with regard to medical education or an effective development of assistance systems during image-guided interventions (e.g., robotics, navigation/registration algorithms). In this work, an MRI-compatible and customizable motion phantom is presented to mimic respiratory-triggered organ movement as well as human anatomy. METHODS:

An automated method to establish patient specific asymmetric intrafraction motion monitoring tolerances for prostate radiotherapy.

PURPOSE: Intrafraction motion management is recognized as a critical component of radiotherapy. While clinical trials often recommend its use, they rarely define motion management tolerances. Consequently, institutions set tolerance levels independently, often without considering patient specific anatomy, dose distributions or direction of motion. This study introduces an automated method that can be used to derive patient specific, asymmetric intrafraction motion monitoring tolerances based on individualized treatment plans. METHODS: Treatment plans for 20 prostate cancer patients receiving

A study to investigate the influence of cardiac motion on the robustness of pencil beam scanning proton plans in oesophageal cancer.

While proton therapy offers an excellent dose conformity and sparing of organs at risk, this can be compromised by uncertainties, e.g. organ motion. This study aimed to investigate the influence of cardiac motion on the contoured oesophagus using electrocardiogram-triggered imaging and to assess the impact of this motion on the robustness of proton therapy plans in oesophageal cancer patients. Limited cardiac-induced motion of the oesophagus was observed with a negligible impact on the robustness of proton therapy plans. Therefore, our data suggest that cardiac motion may be safely ignored in

Modeling intra-fractional abdominal configuration changes using breathing motion-corrected radial MRI.

Abdominal organ motions introduce geometric uncertainties to gastrointestinal radiotherapy. This study investigated slow drifting motion induced by changes of internal anatomic organ arrangements using a 3D radial MRI sequence with a scan length of 20 min. Breathing motion and cyclic GI motion were first removed through multi-temporal resolution image reconstruction. Slow drifting motion analysis was performed using an image time series consisting of 72 image volumes with a temporal sampling rate of 17 s. B-spline deformable registration was performed to align image volumes of the time series

Spatial and temporal motion characterization for x-ray CT.

BACKGROUND: Motion induced image artifacts have been the focus of many investigations for x-ray computed tomography (CT). Methodologies of combating patient motion include the use of gating devices to optimize the data acquisition, reduction in patient scan time via faster gantry rotation and large detector coverage, and the development of advanced reconstruction and post-processing algorithms to minimize motion artifacts. PURPOSE: Previously proposed approaches are generally "global" in nature in that motion is characterized for the entire image. It is well known, however, that the presence

A dynamic anthropomorphic phantom for end-to-end testing in image- and surface-guided adaptive radiotherapy.

BACKGROUND: Respiratory and digestive motion can compromise the accuracy of radiotherapy (RT), affecting tumor targeting and healthy tissue sparing. Current phantoms often fail to replicate complex organ movements and lack compatibility with advanced imaging modalities like magnetic resonance imaging (MRI) and 4D computed tomography (4DCT), limiting their utility in adaptive radiotherapy. The BRaVIDA (Breathing Radiotherapy Visual monitoring, Imaging, and Dosimetric Anthropomorphic) phantom addresses these limitations by simulating realistic organ motion during breathing and digestion. Its MRI

Motion-flow-guided recurrent network for respiratory signal estimation of x-ray angiographic image sequences.

Motion compensation can eliminate inconsistencies of respiratory movement during image acquisitions for precise vascular reconstruction in the clinical diagnosis of vascular disease from x-ray angiographic image sequences. In x-ray-based vascular interventional therapy, motion modeling can simulate the process of organ deformation driven by motion signals to display a dynamic organ on angiograms without contrast agent injection. Automatic respiratory signal estimation from x-ray angiographic image sequences is essential for motion compensation and modeling. The effects of respiratory motion, c

Development of an abdominal dose accumulation tool and assessments of accumulated dose in gastrointestinal organs.

Objective.Online adaptive radiotherapy has demonstrated improved dose conformality in response to inter-fraction geometric variations in the abdomen. The dosimetric impact of intra-fractional variations in anatomic configuration resulting from breathing, gastric contraction and slow configuration motion, however, have been largely ignored, leading to differences between delivered and planned. To investigate the impact of intra-fractional abdominal motions on delivered dose, anatomical deformations due to these three motion modes were extracted from dynamic MRI data using a previously developed

A modular motion compensation pipeline for prospective respiratory motion correction of multi-nuclear MR spectroscopy.

Magnetic resonance (MR) acquisitions of the torso are frequently affected by respiratory motion with detrimental effects on signal quality. The motion of organs inside the body is typically decoupled from surface motion and is best captured using rapid MR imaging (MRI). We propose a pipeline for prospective motion correction of the target organ using MR image navigators providing absolute motion estimates in millimeters. Our method is designed to feature multi-nuclear interleaving for non-proton MR acquisitions and to tolerate local transmit coils with inhomogeneous field and sensitivity distr

Function of otolith organ in goldfish revealed from analysis of eye movement induced by acceleration.

An otolith organ on ground behave as a detector of both gravity and linear acceleration, and play an important role in controlling posture and eye movement for tilt of the head or translational motion. On the other hand, a gravitational acceleration ingredient to an otolith organ disappears in microgravity environment. However, linear acceleration can be received by otolith organ and produce a sensation that is different from that on Earth. It is suggested that in microgravity signal from the otolith organ may cause abnormality of posture control and eye movement. Therefore, the central nervou

[Motion sickness in motion: from carsickness to cybersickness].

- Motion sickness is not a disorder, but a normal response to a non-normal situation in which movement plays a central role, such as car travel, sailing, flying, or virtual reality.- Almost anyone can suffer from motion sickness, as long as at least one of the organs of balance functions. If neither of the organs of balance functions the individual will not suffer from carsickness, seasickness, airsickness, nor from cybersickness. - 'Cybersickness' is a form of motion sickness that is stimulated by artificial moving images such as in videogames. Because we are now exposed more often and for lo

Gastrointestinal Motility-Induced Interplay in Pancreas Proton Therapy: Motion Simulation and Dosimetric Impact.

PURPOSE: To simulate motion and evaluate its dosimetric interplay from gastrointestinal (GI) motility in pancreas pencil-beam scanning (PBS) proton therapy and to assess how fractionation and motility amplitude affect target coverage and organ-at-risk (OAR) doses. METHODS AND MATERIALS: A novel, physiology-informed GI motion simulator was developed to quantify GI motion by generating time-resolved motion modes (peristalsis, rhythmic segmentations, and high-amplitude propagated contractions) parameterized along organ centerlines (stomach, duodenum, and small bowel). The simulator was integrate

Evaluation of volumetric breathing motion prediction of the stomach using dynamic golden-angle radial MRI.

Objective. The motion of abdominal organs complicates accurate radiotherapy (RT) planning and delivery. Accurate short-term prediction of respiratory motion is essential for image-guided adaptive RT in the abdomen. This study evaluates the performance of a kernel ridge regression-based breathing motion prediction framework (KRR-BMP) for abdominal organs, with particular emphasis on the stomach and its centerline.Approach. Dynamic MRI datasets from 27 scans acquired in 16 patients were processed to generate low-rank breathing motion models based on principal component analysis (PCA) of deformat

Close-to-critical Hopf oscillators boost cochlear gain in realistic models with fluid focusing.

Critical Hopf oscillators have been proposed for modeling the cochlear micromechanics, and the generation of spontaneous otoacoustic emissions. In such top-down models, the cochlear micromechanical elements are abstractly represented through equations written in normal form. There is a gap to be filled between this abstract formulation and the complexity of the motion of the Organ of Corti as revealed by recent optical coherent tomography measurements. The aim of the present work is bridging this gap using a bottom-up approach: starting from realistic equations schematizing the actual physical

[Image-guided radiotherapy for gynaecological cancer].

Intensity-modulated radiation therapy reduces the dose to organs at risk in patients treated for gynecological malignancies. Given the steep dose gradients around the planning target volume, internal movement of organs during treatment may cause a planning target volume underdosage and organs at risk overdosage in case of systematic error. It is therefore important to take into account the movement of the pelvic organs and to investigate potential solutions provided by image-guided radiotherapy. A review of the literature was carried out using the following databases: Medline, Embase, Cochrane

Cochlear partition anatomy and motion in humans differ from the classic view of mammals.

Mammals detect sound through mechanosensitive cells of the cochlear organ of Corti that rest on the basilar membrane (BM). Motions of the BM and organ of Corti have been studied at the cochlear base in various laboratory animals, and the assumption has been that the cochleas of all mammals work similarly. In the classic view, the BM attaches to a stationary osseous spiral lamina (OSL), the tectorial membrane (TM) attaches to the limbus above the stationary OSL, and the BM is the major moving element, with a peak displacement near its center. Here, we measured the motion and studied the anatomy

Improved patient models for simulation of clinically realistic 68Ga-SSTR PET and 177Lu-PRRT SPECT studies.

BACKGROUND: The aim was to improve the realism of patient models for Monte-Carlo based evaluation of image-based quantification of tumour volume and activity in [68Ga]Ga-DOTA-TOC PET and [177Lu]Lu-DOTA-TATE SPECT images. METHODS: Three phantoms from the XCAT population were used as basis. Tumour structures were obtained from segmentation of patient 68Ga-PET images and positioned at relevant positions in the phantoms. Respiratory motion was modelled, and the centre-of-mass of tumours displaced according to the organ motion in the respiration. Tumour activity concentrations were sampled from a
