Coordinated Care Organizations and mortality among low-income infants in Oregon.

OBJECTIVE: To examine the impact of Oregon's Coordinated Care Organizations (CCOs), an accountable care model for Oregon Medicaid enrollees implemented in 2012, on neonatal and infant mortality. DATA SOURCES: Oregon birth certificates linked with death certificates, and Medicaid/CCO enrollment files for years 2008-2016. STUDY DESIGN: The sample consisted of the pre-CCO birth cohort of 135 753 infants (August 2008-July 2011) and the post-CCO birth cohort of 148 650 infants (August 2012-December 2015). We used a difference-in-differences probit model to estimate the difference in mortality bet

Sustainment of smoking cessation programs in substance use disorder treatment organizations.

INTRODUCTION: The majority of individuals who enter substance use disorder (SUD) treatment also use tobacco. Integrating smoking cessation services into SUD treatment may have substantial public health benefits, but few studies have examined whether organizations offering counseling-based smoking cessation programs sustain them over time. METHODS: This study examines sustainment of smoking cessation programs using 2 waves of data collected from 150 SUD treatment organizations. Data were collected in 2006-2008 and 2009-2010 using face-to-face interviews, telephone interviews, and mailed survey

Beyond expectations: exploring the multifaceted role of positive gossip in organizations.

INTRODUCTION: Gossip is a fundamental aspect of human communication that facilitates the expression of attitudes, beliefs, and opinions related to emotions. Within organizational settings, gossip frequently pertains to third parties who are not physically present and can be classified as either positive or negative based on its valence. This study focuses on positive gossip, specifically how it is perceived and experienced by employees as an organizational phenomenon. METHODS: The study employed a phenomenological approach, with data collected through in-depth interviews conducted in two dist

Medicare Accountable Care Organizations: Clinical Performance for Patients With Heart Failure.

OBJECTIVES: Objective: To understand factors that distinguish higher-performing accountable care organizations (ACOs) in the Medicare Shared Savings Program from those with average levels of clinical performance for patients with heart failure (HF), a common and complex condition. STUDY DESIGN: We used a 2-step design. First, we measured ACOs' clinical performance using Medicare data to calculate risk-standardized admission rates over time (2014-2017) for patients with HF. Next, we conducted 16 videoconference interviews with key informants in 2 ACOs that were average performers on this measu

Provision of Social Care Services by Mental and Behavioral Health Organizations.

OBJECTIVE: Despite substantial interest in identifying and addressing patients' social needs in health care settings, little is known about the characteristics of mental health organizations associated with providing social care services (SCSs; i.e., services that may help to address patients' social needs). This study aimed to characterize the prevalence of 10 SCSs among mental health organizations in the United States. METHODS: Cross-sectional data from the Substance Abuse and Mental Health Services Administration's 2018-2020 National Mental Health Services Survey (2018, N=11,580; 2019, N=1

Perceptions and behaviors toward first-generation, low-income individuals in organizations.

As universities and employers strive for greater socioeconomic diversity, understanding First-Generation, Low-Income (FGLI) status as a dimension of diversity is crucial. This review examines how FGLI individuals-who are the first in their families to attain higher education, achieve professional occupations and/or come from low-income backgrounds-are perceived and treated in academic and professional settings. Our review shows negative perceptions of FGLIs on traits like agency and cultural fit often lead to their exclusion. We explore the accuracy of these perceptions, finding that many perc

The impact of health maintenance organizations on improving cardiac surgery outcomes.

BACKGROUND AND OBJECTIVES: California is one of a few states with mandatory reporting of mortality after coronary artery bypass graft (CABG) surgery. The Affordable Care Act restructured Medicaid, preferentially penalizing patients experiencing poverty because payments to hospitals for isolated surgical events overshadow payments to primary care clinicians. We propose outcomes are superior when hospital networks organize surgical episodes within the context of primary care inside that same network. DESIGN AND METHODS: We listed factors impacting outcomes after CABG. CABG surgery outcome depen

Normothermic Regional Perfusion Experience of Organ Procurement Organizations in the US.

IMPORTANCE: Despite the unmet need for donor organs, organ use from donation after circulatory determination of death (DCD) donors has been limited by inferior transplant outcomes. Normothermic regional perfusion (NRP) improves recipient outcomes and organ utilization from DCD donors. There is variability in NRP policies and experience among US organ procurement organizations (OPOs). OBJECTIVES: To determine OPO experience, identify operational inconsistencies, and explore needs related to NRP. DESIGN, SETTING, AND PARTICIPANTS: This survey study included 55 OPOs in the US that had recovered

The Future of Contract Research Organizations in Dermatology and Aesthetic Research.

OBJECTIVE: Dermatology and aesthetic medicine has experienced significant growth in recent years, driven by industry-sponsored research performed by clinical investigators. Contract research organizations (CROs) play an important role to ensure clinical trials are run efficiently, ethically, and according to Good Clinical Practice. An advisory group of dermatologists and aesthetic researchers was assembled to obtain feedback about CRO experiences for developing a "next-generation" specialty CRO for dermatology and aesthetic medicine. METHODS: Experienced dermatologists and aesthetic physician

Fighting the 400-Year Pandemic: Racism Against Black People in Organizations.

BACKGROUND: The present pandemic COVID-19 is caused by SARS-CoV-2, a single-stranded positive-sense RNA virus from the Coronaviridae family. Due to a lack of antiviral drugs, vaccines against the virus are urgently required. METHODS: In this study, validated computational approaches were used to identify peptide-based epitopes from six structural proteins having antigenic properties. The Net-CTL 1.2 tool was used for the prediction of CD8+ T-cell epitopes, while the robust tools Bepi-Pred 2 and LBtope was employed for the identification of linear B-cell epitopes. Docking studies of the identi

Manipulating Extracellular Matrix Organizations and Parameters to Control Local Cancer Invasion.

Metastasis contributes to over 90 percent of cancer mortalities and may be influenced by the extracellular matrix (ECM). ECM microenvironments differ in matrix organization, cell-matrix adhesions, and fiber rigidity, which may affect cancer migration and, thus, should be investigated. To understand the interactions between cancer cells and the ECM, we simulate local invasion through ECM organizations of varying determinants. Randomly curved organizations of normal ovarian stroma exhibit minimal local invasion. In contrast, wave-like and parallel linear structures in reorganized ECM organizatio

Empowering Change Agents in Hierarchical Organizations: Participatory Action Research in Prisons.

Participatory action research (PAR) approaches harness collaborative partnerships to stimulate change in defined communities. The purpose of this article is to illustrate key methodological strategies used in the application of PAR methods in the particularly challenging environment of a hierarchical organization. A study designed to promote sustainable, insider-generated system-level changes in the provision of end-of-life (EOL) care in the restrictive setting of six state prisons is used as an exemplar of the application of three cardinal principles of PAR. First, development of a collaborat

Public health research priorities in Europe seen by non-governmental organizations.

BACKGROUND: Public health research is concerned with population health, determinants of health, health systems research, health promotion, environmental health, health protection, disease prevention and research in other fields of public health. During the last decades, non-governmental organisations (NGOs) are more often entering the field of public health research. This paper presents results of work within SPHERE (Strengthening Public Health Research in Europe), a European Commission funded study aimed to gather information and produce knowledge on the state of public health research in Eur
