Oral treatment of organophosphate poisoning in mice.

OBJECTIVE: Organophosphates are used as pesticides, herbicides, and chemical warfare agents. Treatment of organophosphate poisoning is with intravenous atropine and pralidoxime in addition to supportive care. This study determined the efficacy of oral agents in preventing death from organophosphate poisoning. METHODS: The organophosphate paraoxon (8 mg/kg) was used in a murine model with lethality at four and 24 hours as an end point. For oral treatment, 15 male Balbc mice were given either atropine sulfate (4 mg/kg), or a combination of atropine sulfate (4 mg/kg) with pralidoxime (100 mg/kg)

Case study on chronic organophosphate poisoning.

Ernest and Leslie Rea, two farmers in their mid-fifties, contacted their local County Agricultural Extension Office in Cape May, New Jersey, regarding health problems associated with their use of pesticides. They were referred to Dr. Hamilton and the Environmental and Occupational Health Sciences Institute (EOHSI) clinic for consultation and evaluations. They were diagnosed with chronic intoxication of organophosphate pesticides. Recommendations were made to decrease health risks in the farming operations. These included the usage of personal protective equipment while handling or applying pes

Acute and subacute organophosphate poisoning in the rat.

The intermediate syndrome in organophosphate poisoning is clinically characterized by weakness in the territory of cranial nerves, weakness of respiratory, neck and proximal limb muscles, and depressed deep tendon reflexes. It occurs between the acute cholinergic crisis and the usual onset of organophosphate-induced delayed neurotoxicity. The weakness has been ascribed to muscle fiber necrosis. Fenthion has been the most common cause. This study assesses the occurrence of the necrotizing myopathy in rats in relation to the clinical course and the acetylcholinesterase (AChE) inhibition after po

Delayed organophosphate neurotoxicosis in four heifers.

Oil-containing triaryl phosphates induced delayed organophosphate neurotoxicosis in 4 dairy heifers. Clinical signs developed 10 to 14 days after exposure and included dyspnea, dysuria, polyuria, tympanites, incoordination, hind limb weakness with knuckling of the metatarsophalangeal joints, and flaccid paralysis. Triaryl phosphate concentrations were high in a specimen of fat from 1 heifer, and the fat and 2 waste oil samples used to periodically lubricate a feed bunk motor chain had similar triaryl phosphate-like chromatographic profiles. The diagnosis of delayed organophosphate neurotoxicos

Carbamate and organophosphate poisoning in early childhood.

Twenty-five infants and young children intoxicated by carbamate and organophosphorus compounds are described. Presenting signs and symptoms in children differed from those described in adults and were mainly related to severe CNS depression, coma and stupor, dyspnea, and flaccidity. Other clinical signs such as miosis, excessive salivation and tearing, sweaty, cold skin, and gastrointestinal symptoms were less frequent, while fasciculations and bradycardia were quite uncommon on arrival. Only two patients presented with all typical signs of organophosphate poisoning as described in adults. Sig

Anxiety associated with exposure to organophosphate compounds.

Acute organophosphate poisoning is known to result in substantial behavioral abnormalities. We assessed psychiatric manifestations of exposure in workers less substantially exposed to organophosphate compounds and showing no obvious signs of toxicity. Commercial pesticide sprayers and farmers recently exposed to organophosphate agents were compared to control subjects on personality tests, a structured interview, and cholinesterase level. The commercial sprayers but not the exposed farmers showed elevated of anxiety and lower plasma cholinesterase than control subjects. Assessment of other beh

Advances in the Detection and Management of Organophosphate Poisoning.

BACKGROUND: The growing prevalence of pesticide poisonings, particularly those derived from organophosphate exposure, represents a significant challenge to global public health, because of their high toxicity and widespread agricultural use.Effective and rapid emergency department management of organophosphate (OP) poisonings is essential to mitigate complications associated with acute exposure and ensure patient survival. AREAS OF UNCERTAINTY: We aim to review the literature and present the most up-to-date treatment strategies, and the use of various technological tools, in the detection of

[Electrocardiographic changes in acute organophosphate poisoning].

Cardiac complications often accompany poisoning with organophosphates. These may be serious and often fatal, being represented by cardiac arrhythmias, electrocardiographic abnormalities and conduction defects, as well as myocardial infarction, a rarely reported complication of acute pesticide poisoning. The extent and pathogenesis of cardiac toxicity from these compounds is not yet clearly defined. We report the case of a 57-year-old woman who presented to our emergency department with coma and acute non-cardiogenic pulmonary edema, as a result of organophosphates ingestion. She was resuscitat

Neurological syndromes following organophosphate poisoning.

Organophosphorous compounds, the anticholinesterases, produce significant morbidity and mortality in India. Although exact estimates are not available, hospital based statistics suggest that nearly half of the admissions to emergency with acute poisoning are due to organophosphates. Following accidental or suicidal exposure, these anticholinesterases lead to three well defined neurological syndromes i.e. initial life threatening acute cholinergic crisis which often requires management in intensive care unit, intermediate syndrome in which cranial nerve palsies, proximal muscle weakness and res

Organophosphate poisoning versus brainstem stroke.

Two patients presented unconscious after deliberate organophosphate ingestion. Both were initially misdiagnosed as having brainstem stroke, and plans were made for withdrawing treatment within 24 hours. Once correctly diagnosed and appropriately treated, both recovered, illustrating the importance of considering a wide differential diagnosis before withdrawing support and of not relying on routine "drug screens" to detect organophosphates.

Organophosphate poisoning presenting as diabetic ketoacidosis.

A 3-year-old boy was admitted to hospital following rapid-onset coma. Laboratory tests demonstrated hyperglycemia, glycosuria and keto-acidosis. Organophosphorus poisoning was the cause of the coma since he had been in contact with Parathion, serum cholinesterase activity was undetectable and his condition returned to normal under atropine therapy.

The Use of Amantadine in Treating Extrapyramidal Symptoms in Organophosphates Poisoning in a Child.

Background. Organophosphates are chemicals that lead to the accumulation of acetylcholine, causing muscarinic symptoms such as salivation and nicotinic manifestations like muscle weakness and hypertension and rarely leading to basal ganglia impairment, manifesting as extrapyramidal symptoms. Literature reported that the use of amantadine, an amine that has both antiviral and dopaminergic activities, improves extrapyramidal manifestations. Most of the studies exploring the effect of amantadine were conducted on adults and there are extremely limited data in regards to this topic in Saudi Arabia

Cholinesterase reactivators: the fate and effects in the organism poisoned with organophosphates/nerve agents.

Understanding the mechanism of action of organophosphates (OP)/nerve agents -- irreversible acetylcholinesterase (AChE, EC 3.1.1.7) inhibition at the cholinergic synapses followed by metabolic dysbalance of the organism -- two therapeutic principles for antidotal treatment are derived. The main drugs are anticholinergics that antagonize the effects of accumulated acetylcholine at the cholinergic synapses and cholinesterase reactivators (oximes) reactivating inhibited AChE. Anticonvulsants such as diazepam are also used to treat convulsions. Though there are experimental data on a good therapeu

An enantiomer-based virtual screening approach: Discovery of chiral organophosphates as acetyl cholinesterase inhibitors.

Chiral organophosphates (OPs) have been used widely around the world, very little is known about binding mechanisms with biological macromolecules. An in-depth understanding of the stereo selectivity of human AChE and discovering bioactive enantiomers of OPs can decrease health risks of these chiral chemicals. In the present study, a flexible molecular docking approach was conducted to investigate different binding modes of twelve phosphorus enantiomers. A pharmacophore model was then developed on basis of the bioactive conformations of these compounds. After virtual screening, twenty-four pot
