Organ-at-risk (OAR) delineation is a key step for cone-beam CT (CBCT) based adaptive radiotherapy planning that can be a time-consuming, labor-intensive, and subject-to-variability process. We aim to develop a fully automated approach aided by synthetic MRI for rapid and accurate CBCT multi-organ contouring in head-and-neck (HN) cancer patients. MRI has superb soft-tissue contrasts, while CBCT offers bony-structure contrasts. Using the complementary information provided by MRI and CBCT is expected to enable accurate multi-organ segmentation in HN cancer patients. In our proposed method, MR ima

PURPOSE: To demonstrate that temporary organ displacement (TOD) by drainage catheter placement and hydrodissection is feasible and reproducible for simulation (SIM) and stereotactic body radiation treatment (SBRT).
MATERIALS AND METHODS: Between February 2010 and December 2018, 31 consecutive patients (20 men and 11 women; median age, 59 years; range 20-80 years) received both SIM and SBRT with TOD. The minimum required displacement was 10 mm between the gross tumor volume (GTV) and the organ at risk (OAR). Complete displacement was defined as the ability to displace the OAR from the GTV a mi

This study reports clinical performance in the sparing of infrahyoid swallowing organs at risk (SWOARs) in oropharynx cancer intensity-modulated radiation therapy (IMRT) plans. Rates of meeting dose-volume planning goals are reported and compared with geometry-based estimates of what is achievable. This study also develops 3 measures of target-SWOAR geometry and tests their usefulness in providing geometry-based dose-volume planning goals. A total of 50 oropharynx cancer IMRT plans were reviewed. Success rates in meeting institutional dose-volume goals were determined for the glottic larynx (G

Constraint definitions in intensity modulated radiation therapy is a key point factor during the treatment planning process. In literature some data are available about dose constraints and volumes according to the tissue architectures. Following ICRU recommendations, organs at risk organized in a parallel structure could receive an acceptably small proportion of high dose component. Mean dose and dose volume histogram is a most convenient tool for incorporating such constraints. Organs described as a serial structure are supposed to receive less than the given maximum dose, directly linked to

PURPOSE: Accurate segmentation of head and neck organs-at-risk remains a critical challenge in radiation therapy planning, where current single-modality approaches often fail to address the inherent complexity of soft-tissue differentiation and interpatient anatomic variations. This study aims to develop a clinically robust auto-segmentation framework that synergistically integrates multimodal imaging features while optimizing computational efficiency.
METHODS AND MATERIALS: We present multimodality multimask and multitask auto-segmentation network (M3-Net), a triple-interlocked deep learning

Objective.Deep learning auto-segmentation has greatly advanced contouring in radiotherapy. However, quality assurance remains necessary due to performance fluctuation among individual patients. This manual process reintroduces variability and partially reduces time-saving benefits. As a solution, uncertainty quantification (UQ) is increasingly explored for its ability to estimate output confidence. While numerous methods to quantify uncertainty exist, their comparative reliability remains underexplored. This study compares the reliability of commonly used UQ approaches for auto-segmentation in

PURPOSE: Delineation of the clinical target volume (CTV) and organs-at-risk (OARs) is important in cervical cancer radiotherapy. But it is generally labor-intensive, time-consuming, and subjective. This paper proposes a parallel-path attention fusion network (PPAF-net) to overcome these disadvantages in the delineation task.
METHODS: The PPAF-net utilizes both the texture and structure information of CTV and OARs by employing a U-Net network to capture the high-level texture information, and an up-sampling and down-sampling (USDS) network to capture the low-level structure information to acce

The objective of this article is to automatically segment organs at risk (OARs) for thoracic radiology in computed tomography (CT) scan images. The OARs in the thoracic anatomical region during the radiotherapy treatment are mainly the neighbouring organs such as the esophagus, heart, trachea, and aorta. The dataset of 40 patients was used in the proposed work by splitting it into three parts: training, validation, and test sets. The implementation was performed on the Google Colab Pro+ framework with 52 GB of RAM and 265 GB of storage space. An ensemble model was evolved for the automatic seg

BACKGROUND AND PURPOSE: In a separate article (PART 1), a rationale and explanation of the physiology-and-anatomy-based concept of Functional Swallowing Units (FSUs) was presented. FSUs are swallowing muscles not included in the set of commonly defined swallowing organs at risk (SWOARs). They are involved in three crucial swallowing components: hyolaryngeal elevation (HLE), tongue base retraction (TBR) and tongue motion. This paper is a continuation of PART 1 and it provides detailed computed tomography (CT)-based delineation guidelines for FSUs, which presumably are also at risk of radiation-

BACKGROUND: Carbon ion radiotherapy (CIRT) treatment planning is based on relative biological effectiveness (RBE) weighted dose calculations. A large amount of clinical evidence for CIRT was collected in Japan with RBE estimated by the modified microdosimetric kinetic model (MKM) while all European centres apply the first version of the local effect model (LEM). Japanese schedules have been used in Europe with adapted prescription dose and organs at risk (OAR) dose constraints. Recently, less conservative adapted LEM constraints have been implemented in clinical practice. The aim of this study

INTRODUCTION: The German Hodgkin Study Group (GHSG) HD17 trial established the omission of radiotherapy (RT) for patients with early-stage unfavorable Hodgkin lymphoma being PET-negative after 2 cycles of BEACOPP escalated plus 2 cycles of ABVD. This patient group reveals heterogeneity in characteristics and disease extent which prompted us to perform a decisive dosimetric analysis according to GHSG risk factors. This may help to tailor RT individually balancing risks and benefits.
METHODS: For quality assurance, RT-plans were requested from the treating facilities (n= 141) and analyzed centr

Accurate segmentation of multiple organs is essential for the diagnosis and treatment of head and neck cancer. However, the intricate anatomical structure and dense organ distribution in the head and neck region pose significant challenges for existing automated segmentation models, which predominantly target single organs and rely on single-modality imaging. Achieving comprehensive, one-step segmentation of organs-at-risk (OARs) remains challenging. To this end, we propose a Point-cloud Matrix Fusion-based Segmentation Model (PMFM) that leverages an improved multi-modal data fusion strategy f

PURPOSE: Indications for radiotherapy (RT) of the internal mammary nodes (IMN) in early breast cancer vary between countries. While studies indicate benefits, IMN RT increases the dose to the heart and lungs, and the risk-benefit ratio of this treatment is debated. This study investigates how IMN RT affects dose to organs at risk (OAR) and pneumonitis incidence in a clinical setting.
METHODS: This retrospective study includes breast cancer patients receiving adjuvant locoregional RT with and without IMN included in the target volume at Skåne University Hospital, Sweden, from 2018 to 2021. Tre

Delineation of the prostate and nearby organs at risk (OARs) is a fundamental step in prostate cancer radiation therapy planning. Such contouring is often done manually, which can be a time-consuming and highly variable process. To alleviate these issues, we propose a fully automated two-step deep learning approach to segment the prostate, bladder, rectum, seminal vesicles, and femoral heads from CT images. The first step localizes the organs of interest using a modified 3D UNet architecture that contains an axial cross-attention module. Final segmentations are then computed for each organ ind

BACKGROUND AND PURPOSE: To quantify the increase in bladder and rectum dose of a bone marrow sparing (BMS) VMAT strategy for primary treatment of locally advanced cervical cancer (LACC).
MATERIALS AND METHODS: Twenty patients with stage IB-IVA cervical cancer were selected for this study. The whole Pelvic Bones (PB) was taken as substitute for bone marrow. For every patient, Pareto-optimal plans were generated to explore the trade-off between rectum, bladder, and PB mean dose. The PB mean dose was decreased in steps of 1 Gy. For each step, the increase in rectum and bladder mean dose was quan

BACKGROUND: Stereotactic radiosurgery (SRS) can be an effective primary or adjuvant treatment option for intracranial tumors. However, it carries risks of various radiation toxicities, which can lead to functional deficits for the patients. Current inverse planning algorithms for SRS provide an efficient way for sparing organs at risk (OARs) by setting maximum radiation dose constraints in the treatment planning process.
PURPOSE: We propose using activation maps from functional MRI (fMRI) to map the eloquent regions of the brain and define functional OARs (fOARs) for Gamma Knife SRS treatment

INTRODUCTION: Contouring organs at risk (OARs) is a time-intensive task that is a critical part of radiation therapy. Atlas-based automatic segmentation has shown some success at reducing this time burden on practitioners; however, this method often requires significant manual editing to reach a clinically accurate standard. Deep learning (DL) auto-segmentation has recently emerged as a promising solution. This study compares the accuracy of DL and atlas-based auto-segmentation in relation to clinical 'gold standard' reference contours.
METHODS: Ninety CT datasets (30 head and neck, 30 thorac

BACKGROUND: The safe use of radiotherapy (RT) requires compliance with dose/volume constraints (DVCs) for organs at risk (OaRs). However, the available recommendations are sometimes conflicting and scattered across a number of different documents. Therefore, the aim of this work is to provide, in a single document, practical indications on DVCs for OaRs in external beam RT available in the literature.
MATERIAL AND METHODS: A multidisciplinary team collected bibliographic information on the anatomical definition of OaRs, on the imaging methods needed for their definition, and on DVCs in genera
