Adjunctive Aesthetic Procedures in Orthognathic Surgery.

An important aesthetic goal in orthognathic planning is to improve facial balance, harmony, volume, and symmetry. It is therefore logical that adjunctive aesthetic procedures become a part of the overall orthognathic treatment plan and that their possibilities are discussed with orthognathic candidates. Such procedures help to improve the final outcome of the orthognathic treatment and enhance patient satisfaction. Training and experience are of utmost importance when offering and performing aesthetic facial surgery. This article discusses various facial aesthetic procedures that can be combin

Orthognathic Surgery Rate in Cleft Care.

PURPOSE: To determine the true need for orthognathic surgery in patients with repaired cleft lip and/or palate (CL/P) at a high-volume craniofacial center. METHODS: An institutional retrospective review of patients with CL/P born between 1975 and 2008 was performed. Patients with adequate documentation reflecting cleft care who were ≥ 18 years at the time of last craniofacial/dentistry follow-up were included. Patients with non-paramedian clefts or a comorbid craniofacial syndrome were excluded. Primary outcome variable was the total proportion of patients with CL/P who either underwent or we

Orthodontic Considerations for Cleft Orthognathic Surgery.

Preparation and planning for orthognathic surgery in late adolescence depends on the complexity of unresolved problems with which the patient presents. Different strategies are presented to address these unresolved problems in the adult patient with cleft lip and palate. Different surgical and orthodontic treatments are presented to correct the class III malocclusion in patients with cleft lip and palate in ranges that are analogous to the envelope of discrepancy. For complex cases, the principles of achievability, stability, and esthetics should guide the decision-making process for planning

Accuracy of computer-assisted orthognathic surgery.

INTRODUCTION: The purpose of this study was to retrospectively evaluate the difference between the planned and the actual movements of the jaws, using three-dimensional (3D) software for PC-assisted orthognathic surgery, to establish the accuracy of the procedure. MATERIAL AND METHODS: A retrospective study was performed with 49 patients who had undergone PC-guided bimaxillary surgery. The accuracy of the protocol was determined by comparing planned movements of the jaws with the actual surgical movements, analysing frontal and lateral cephalometries. RESULTS: The overall results were deemed

[Temporo-mandibular joints and orthognathic surgery].

Temporo-Mandibular Joints (TMJ) and orthognathic surgery are closely linked. In the past, some authors have even described (with mixed results) the correction of some dysmorphosis through direct procedures on the TMJs. Nowadays, performing orthognathic surgery involves the TMJ in three different occasions: (1) TMJ disorders potentially responsible for dento-maxillary dysmorphosis, (2) effects of orthognathic surgery on TMJs, and (3) condylar positioning methods in orthognathic surgery. These three chapters are developed in order to focus on the close relationships between TMJ and orthognathic

Haematic cyst formation after orthognathic surgery.

A 34-year-old man suffered from discomfort originating from a mass located in his right zygomatic area. He developed orbital symptoms, such as diplopia, which lasted for 5 months. He also suffered from Crouzon's syndrome and had undergone a Lefort III osteotomy 9 years prior for correction of midfacial hypoplasia. The size of the mass slowly increased and his orbital symptoms developed further, eventually leading to surgical exploration. During the operation, a mass was found near titanium plates which had been used to fix bone segments during a previous surgery. The total mass, including the

Temporomandibular joint considerations in orthognathic surgery.

The relationship between the temporomandibular joint and developmental jaw deformities is both undeniable as well as controversial as to its magnitude or importance. This article addresses the importance of a complete temporomandibular joint and orthognathic diagnostic work-up prior to the choice of either temporomandibular joint or orthognathic surgery. The surgical procedure is determined by the internal derangement that is present as well as its response to non-surgical care. The intraoperative techniques for handling the bony fragments, seating the condyle and rigid fixation techniques are

Disseminated intravascular coagulation following orthognathic surgery.

When performing any type of surgery, the surgeon must be cognizant of hidden bleeding. In the case presented, Le Fort I osteotomy is performed to correct a disharmonious maxillomandibular relationship of a 19-year-old man with diabetes insipidus. Postoperatively, excessive bleeding is seen at the surgical site, IV catheter insertion area, and rectum. It was later determined that the blood originated from the oral structure and traveled through the stomach and rectum. Examination of the blood revealed disseminated intravascular coagulation.

Ophthalmic complications associated with orthognathic surgery.

Ophthalmic complications are rare following maxillary osteotomies. Potential complications include a decrease in visual acuity, extraocular muscle dysfunction, neuroparalytic keratitis, and nasolacrimal problems involving both an increase or a decrease in tearing. Ophthalmic injuries appear to be primarily mediated through indirect injuries to neurovascular structures occurring from traction, compression, or contrecoup injuries from forces transmitted during the pterygomaxillary dysjunction using an osteotome or from fractures extending to the base of the skull or orbit associated with the pte

Incidence of Otalgia After Orthognathic Surgery.

BACKGROUND: In orthognathic surgery, it is well known that maxillary osteotomies and displacements sometimes affect auditory function. Thus, this study examined the relationship between the direction of maxillary displacement and postoperative otalgia. METHODS: Twenty consecutive patients underwent Le Fort I maxillary osteotomy using advancement, impaction, setback, or a combination of these procedures. The direction of movement and incidence of otalgia were investigated. Patients provided informed consent preoperatively, and postoperative reassurance was prudent. RESULTS: Pure-tone average

Surgical Ciliated Cysts in Orthognathic Surgery.

Surgical ciliated cysts (SCC) are rare entities that were first described in the Japanese literature after treatment for chronic maxillary sinusitis. Classically associated with surgery to the maxillary sinus, they have also been reported in association with orthognathic surgery, bone grafting, and trauma. These lesions can develop from a few months up to several years after the surgical procedure. The authors report 2 new cases of SCC after a LeFort I osteotomy for class III dentofacial deformity correction. One of them presented with bilateral lesions and suffered from a recurrence after 5 y

Real-time navigation-assisted orthognathic surgery.

BACKGROUND: One limitation of orthognathic surgery is the narrow surgical field, which makes it difficult to view the operative site directly. Thus, many perioperative complications can occur. In this study, we evaluated the usefulness of computer-aided navigation techniques in orthognathic surgery. METHODS: We enrolled 10 patients (3 men and 7 women) with facial deformities who were treated between July 2010 and February 2011. A Le Fort I osteotomy, sagittal split ramus osteotomy, and reduction malarplasty were performed with guided navigation. RESULTS: All 10 patients were treated successf

[Orthognathic surgery. Study of nerve injuries].

The correction of oral and maxillo-facial mal-formations causes lesions to sensitive nerves due to the invasive and traumatic surgery techniques required. Generally neuro-sensory and cutaneous-mucosal defects are reversible, with full recovery within 6-12 months. In a follow-up study (2 and 8 years) we found altered tactile and pain sensitivity in 43% of patients at subjective evaluation, with objective alteration in 17% of the group. Dental sensitivity evaluated by a pulp test showed an increase in for all teeth tested; values were higher for superior than for inferior teeth. Despite the incr

Orthognathic surgery in craniomaxillofacial fibrous dysplasia.

In craniomaxillofacial fibrous dysplasia, jaw involvement often causes facial asymmetry, an occlusal cant, and loss of teeth. Although conservative management of fibrous dysplasia affecting the jaws is widely practiced, orthognathic surgery is indicated in such cases to restore occlusion and correct dentofacial deformity brought on by the disease process. Since 1981, the Craniofacial Center at Chang Gung Memorial Hospital in Taiwan has treated a total of 84 patients with craniomaxillofacial fibrous dysplasia. Of these, 55 (65%) had fibrous dysplasia affecting the jaws (Zone 4). Between 1988 an

Occlusal plane alteration in orthognathic surgery.

This article discusses correction of functional and esthetic deformities by surgically increasing or decreasing the occlusal plane angulation. Clockwise rotation, or increase of the occlusal plane angulation, is a well-accepted technique in orthognathic surgery. However, counterclockwise rotation (decrease of the occlusal plane angulation), has not been a well-accepted treatment approach. The functional and esthetic characteristics of the low occlusal plane and the high occlusal plane facial types are presented, specific surgical approaches to alteration of the occlusal plane are described, an

Porous block hydroxyapatite in orthognathic surgery.

Seventy-six nonconsecutive patients undergoing orthognathic surgery, in whom blocks of porous hydroxyapatite were implanted into osteotomy gaps in lieu of autogenous bone grafts, are the subjects of this report. Surgical procedures include inferior maxillary repositioning (10 patients), maxillary advancement (24 patients), transverse maxillary expansions (17 patients) and inferior repositioning of the chin (25 patients). A total of 140 anatomic sites were implanted. Eleven patients later consented to open biopsy of the implant material at a mean 10.2 months following implantation. At the time
