Risk Factors for Opioid-Related Adverse Drug Events Among Older Adults After Hospitalization for Major Orthopedic Procedures.

OBJECTIVES: Older adults undergoing orthopedic procedures are commonly discharged from the hospital on opioids, but risk factors for postdischarge opioid-related adverse drug events (ORADEs) have not been previously examined. We aimed to identify risk factors for ORADEs after hospital discharge following orthopedic procedures. METHODS: This is a retrospective cohort study of a national sample of Medicare beneficiaries 65 years or older, who underwent major orthopedic surgery during hospitalization in 2016 and had an opioid fill within 2 days of discharge. We excluded beneficiaries with hospic

Comparison of two postoperative complication grading systems following an elective orthopedic procedure in dogs.

INTRODUCTION: A standardized system for grading postoperative complications in veterinary orthopedics was proposed in 2010 by Cook et al. categorizing complications as minor, major, or catastrophic. Despite broad acceptance, complication rates still vary widely between studies. An alternative to the Cook system has been proposed; the adapted Clavien-Dindo (aCD) system, which categorizes five complication grades and distinguishes them from two other outcomes: 'sequelae' and 'failures-to-cure'. The objective of this study was to compare postoperative complication rates in dogs undergoing tibial

[The use of sonography as a complementary procedure in the orthopedic hip diagnosis in newborn infants (a pilot study)].

In this study we tried to complete the clinical Newborn-control by sonographically investigation at the Newborn-ward. The routine clinical and sonographical combination seems to be successful. Further investigation will be necessary for the sonographic analysis of the physiologic and dysplastic acetabulum. Here we must discuss, if radiological signs can be partially translated for sonography. At last the study checked further technical-sonographical details.

An analysis of the impact of a multimodal therapy order set on postoperative opioid prescribing after orthopedic shoulder procedures.

BACKGROUND: Opioids are effective for postoperative pain control but are no longer considered appropriate as the sole method for managing pain after surgery. Newer, multimodal approaches to pain control are increasingly being employed to decrease reliance on opioids, but patient-related outcomes are not consistently reported with these interventions. OBJECTIVE: This study evaluated the effect of implementing a new multimodal therapy order set, coupled with new patient education materials, on postoperative outcomes after complex shoulder surgery. METHODS: This retrospective cohort study compa

Description of Scatter Radiation Exposure to the Eye Lenses and Direct Exposure to the Hands in Orthopedic and Spinal Procedures.

INTRODUCTION: The increasing adoption of minimally-invasive spinal surgery has resulted in more frequent use of fluoroscopy, heightening concerns regarding radiation-induced cataracts and hand skin cancer in spinal surgeons. We investigated indirect and direct radiation exposure during spinal and orthopedic procedures to enhance radiation protection and explored the factors associated with high dose. METHODS: We used data from 60 procedures performed in the operating and fluoroscopy rooms of a hospital in Japan between January and July 2024. Radiation doses to the eye lenses of the surgeon an

Optimizing surgical instrument and implant utilization in orthopedic procedures: A lean methodology approach to cost reduction and efficiency improvement.

BACKGROUND: Operating rooms (ORs) are among the most resource-intensive areas in hospitals, and optimizing surgical efficiency is crucial for cost reduction. A key contributor to inefficiency is the overstocking of surgical instrument trays and implants, leading to increased sterilization costs, longer processing times, and workflow inefficiencies. Despite its impact, instrument tray optimization has received limited attention in orthopedic surgery. METHODS: This single-center prospective observational study analyzed the utilization rates of surgical instruments and implants in total knee art

Perioperative pain management of a complex orthopedic surgical procedure with double continuous nerve blocks in a burned child.

UNLABELLED: The use of catheters for continuous nerve blocks has been established in children, although in most series only one catheter was used. We report a case of a 3-yr-old child who underwent a toe-to-finger transfer managed with 2 regional catheters: axillary and sciatic. A pain score of 0 was noted during the entire study period. The total dose of bupivacaine was limited to an acceptable range, and the child recovered completely. This report adds to growing evidence in favor of the safety and efficacy of continuous peripheral nerve blocks in pediatric patients. IMPLICATIONS: Double co
