BackgroundIDC in children, first reported by Baron in 1924, is very rare. OPLL of the cervical spine mainly affect people ages 50-70 years. The coexistence of IDC and OPLL in children is very rare, only six cases with 3 to 24 months' follow-up were reported to date.Case presentationA 6-year-old boy presented with complains of neck pain at July 2007. The boy was treated by conservative treatment and observed up for 9 years. Neck pain greatly improved after a one-month conservative treatment and never recur. Laboratory tests revealed elevated ESR and CRP at admission and found nothing abnormal a

BACKGROUND: Metabolomics is one of the "omics" technologies, and is a comprehensive analysis of small molecule metabolites which include amino acid, nucleotides, carbohydrates and fatty acid. The purpose of the present study was to compare the differences of metabolite profiling between patients with ossification of the posterior longitudinal ligament (OPLL) and control subjects.
METHODS: We analyzed plasma metabolites in patients with cervical OPLL (n = 10) and in control subjects (n = 10). Ionic metabolites were analyzed using capillary electrophoresis time-of-flight mass spectrometry (CE-T

INTRODUCTION: We report a case of acute tetraplegia, without any trauma or symptoms prior to onset, who presented with ossification of the posterior longitudinal ligament (OPLL) in the cervical spine with concomitant spinal cord infarction.
CASE PRESENTATION: A 64-year-old man with a number of risk factors for vascular disease was admitted to our hospital with progressive motor weakness in the bilateral upper and lower extremities. He had initially felt numbness in his left upper extremity and had no previous neurological symptoms or trauma. The night after the initial symptoms, he developed

Ossification of the posterior longitudinal ligament of the spine (OPLL) is a common disease in aging populations and sometimes results in serious neurological problems due to compression of the spinal cord and nerve roots. OPLL is a multi-factorial (polygenic) disease controlled by genetic and environmental factors. Studies searching for the genetic component of OPLL, using linkage and association analyses, are in progress and several susceptibility genes have been reported. This paper reviews the recent progress in the genetic study of OPLL and comments on its future task.

Although classically associated with patients of East Asian origin, ossification of the posterior longitudinal ligament (OPLL) may cause myelopathy in patients of any ethnic origin. Degeneration of the PLL is followed by endochondral ossification, resulting in spinal cord compression. Specific genetic polymorphisms and medical comorbidities have been implicated in the development of OPLL. Patients should be evaluated with a full history and neurologic examination, along with cervical radiographs. Advanced imaging with CT and MRI allows three-dimensional evaluation of OPLL. Minimally symptomati

This preliminary study assessed the association between ossification of the posterior longitudinal ligament (OPLL) and the transforming growth factor β receptor type 2 (TGFBR2) gene, with autoimmune disease examined as a possible underlying factor. Twenty-one patients diagnosed with OPLL and 42 control patients without OPLL (non-OPLL) were enrolled in the study. The TGFBR2 gene, composed of one promoter and seven exons, including the 5' untranslated region and flanking introns of each exon, was sequenced. Laboratory tests, including indirect immunofluorescence, were performed to detect autoimm

OBJECT: Ossification of the posterior longitudinal ligament (OPLL) is a progressive disease that causes cervical myelopathy. Because 2D evaluation of ossification growth with plain lateral radiographs has limitations, the authors developed a unique technique to measure ossification progression and volume increase by using multidetector CT scanning.
METHODS: The authors used serial thin-slice volume data obtained by multidetector CT scanning in 5 patients. The mean patient age was 63 years, and the mean follow-up duration was 3.1 years. First, a 3D model of OPLL was semiautomatically segmented

This is a review of the literature on the diagnosis and management of the ossification of the posterior longitudinal ligament (OPLL) of the cervical spine. The pathophysiology and clinical manifestations of cervical OPLL are related to the discogenic dynamic factor of the spine. The signs and symptoms of OPLL include numbness in the hands and/or legs, neck pain, awkward hand movements, and/or gait disturbance, which are not necessarily progressive. Apparent neurological deficit may be an indication of surgery in patients with cervical OPLL because of the low efficacy of conservative treatment

STUDY DESIGN: A longitudinal cohort study of 216 elderly patients with ossification of the posterior longitudinal ligament for an average of 13 years was performed.
OBJECTIVE: To know the quality of life experienced by patients after treatment.
SUMMARY OF BACKGROUND DATA: No report is available on the quality of life experienced by elderly patients with ossification of the posterior longitudinal ligament. Because the life prognosis of patients with this condition is relatively good, the quality of life experienced by elderly patients with this disease is an important subject.

Ossification of the posterior longitudinal ligament (OPLL) in the cervical spine warrants unique clinical, radiographic, and surgical management. OPLL patients presenting with severe myelopathy require full assessment with both magnetic resonance imaging (MRI) and computed tomography-based (noncontrast CT, myelo-CT, three-dimensional CT) examinations to document the full extent of their disease. Whether better surgical outcomes are attained after anterior resection (diskectomy/corpectomy), rather than posterior decompression (laminectomy/laminoplasty) of OPLL remains controversial. However, ou

Spinal Stenosis Syndrome (SSS) causes varying radiculopathies and myelopathies by compressing the spinal cord, rootlets and nerves within the bony canal. An enlarged and ossified posterior longitudinal ligament, a rare cause of the syndrome, can occupy up to 80% of the cervical spinal canal resulting in severe, sometimes permanent myelopathy. The clinical and the radiographic findings are discussed.

OBJECTIVE: To investigate the imaging findings correlated with C5 root palsies in the patients undergoing laminectomy and lateral mass screw fixation for ossification of posterior longitudinal ligament (OPLL), and clarify its pathogenic mechanism.
METHODS: The series included 49 patients with OPLL. Characteristics of preoperative and postoperative x-ray, computed tomography, and magnetic resonance images were compared between the patients with and those without C5 root palsies.
RESULTS: Postoperative C5 root palsies occurred in 9 patients 6 to 64 hours postoperatively. They tended to have in…

OPLL is a progressive process that can result in spinal cord compression and myelopathy. Various surgical approaches for the management of OPLL in the cervical spine exist. Our goal is to present our institution's experience in the management of OPLL over the last 20 years. Sixty-eight patients underwent surgery for cervical OPLL. Mean age at surgery was 56.9 years. No differences between demographic characteristics and surgical approach were identified. There were no significant differences between the approaches regarding the mean estimated blood loss, occurrence of durotomy, reoperation rat

Ossification of the Posterior Longitudinal Ligament (OPLL) is a disease that is characterized by the ectopic calcification of the ligament; however, the pathogenesis of OPLL remains to be investigated. We attempted to identify the in vivo role of Runx2, a master regulator of osteoblast differentiation and skeletal mineralization, in the pathogenesis of OPLL. The expression of Runx2 in the ligament was examined using in situ hybridization and immunohistochemistry and by monitoring the activity of a LacZ gene that was inserted into the Runx2 gene locus. To investigate the functional role of Runx

Ossification of the posterior longitudinal ligament is a common cause of cervical myelopathy, and controversy remains regarding surgical options. Between January 2004 and December 2007, a total of 164 patients with ossification of the posterior longitudinal ligament in the cervical spine who underwent surgical treatment at the authors' institution were included in this study. The choice of surgical option was based on pathological extent and cervical alignment. Short-segment pathology was treated via the anterior approach and long-segment pathology via the posterior approach. When the posterio

This is a case report of tetraparesis associated with extraordinarily severe ossification of the posterior longitudinal ligament (OPLL) of the cervical spine. There was no history of trauma. The object of this paper is to show that OPLL can progress relentlessly to a nearly complete quadriplegia even without trauma, but that adequate decompression can produce almost complete recovery.

Neurourological studies were performed on 39 patients with cervical myelopathy due to ossification of the posterior longitudinal ligament. Micturitional history revealed that 30 patients (77%) had voiding symptoms: obstructive in 8, irritative in 11, and both obstructive and irritative symptoms in 11. Ten patients had urge incontinence and 4 had urinary retention. Urodynamic studies revealed that 7 of 19 had abnormal uroflowmetrogram, 16 of 35 had residual urine, 18 of 39 had detrusor hyperreflexia, and 10 of 39 had detrusor-sphincter dyssynergia. There were no statistical differences in lower

Ossification of the posterior longitudinal ligament (OPLL) is a progressive disease. The bridging of ossified lesions to the vertebral body gradually increases, thereby decreasing the mobility of the cervical spine; thus, cervical spine function may decrease over time. However, cervical spine function in patients with cervical OPLL has not been evaluated in large prospective studies. Therefore, we conducted a prospective multicenter study to clarify whether ossification spread can influence cervical spine function and quality of life (QOL) in patients with cervical OPLL. In total, 238 patients
